The Hidden Connection - Split Notochord Syndrome with Dorsal Enteric Fistula.

Split notochord with dorsal enteric fistula is a rare congenital disease of the central nervous system. It can be associated with other anomalies. Communication with gastrointestinal tract is one of them. Less than 30 cases of such dorsal enteric fistula with split notochord have been reported till date. Here, the authors describe a unique case of a female infant with split notochord syndrome and dorsal enteric fistula. A comprehensive assessment is essential to plan management of this infrequent disease. She was treated with a multistage surgical repair and is currently thriving well.

An Unusual Variant of Distal Vaginal Agenesis: Congenital Vesicovaginal Fistula.

Congenital vesicovaginal fistula (VVF) with distal vaginal agenesis (DVA) is an extremely rare anomaly and isn't easy to diagnose. To detect VVF in DVA, waiting until adolescence for the operation and performing cystoscopy during the menstrual period will ensure a correct diagnosis. Here, a girl with this anomaly is presented.

Massive Upper Gastrointestinal Bleeding From a Mediastinal Tuberculous Esophageal Fistula.

Upper gastrointestinal bleeding (UGIB) is a common medical emergency, most often caused by peptic ulcer disease, variceal rupture, or severe erosive mucosal injury. However, rare etiologies like vascular malformations or infectious processes, including tuberculosis, may also result in severe and life-threatening hemorrhage. Mediastinal tuberculosis with esophageal involvement is an exceptionally rare cause of UGIB. We report a case of a 43-year-old man admitted for massive hematemesis and hemodynamic instability. Endoscopy revealed a large ulcer in the mid-esophagus with active bleeding. Endos

Endoscopic Closure of a Pancreatico-Colonic Fistula: A Case Report.

A known complication of severe pancreatitis is the development of localized collections of necrotic tissue, sometimes leading to the formation of abnormal connections (fistulas) with nearby organs. Some fistulas, such as those involving the stomach or small intestine, may allow drainage of fluid naturally and might not require intervention. However, fistulas involving the colon can lead to serious complications ranging from bleeding to sepsis, with less chance of closure, often needing more invasive treatment. Traditionally, gastrointestinal fistulas were managed surgically, often through bowe

Uncommon Etiology for a Rare Post-Transplant Esophago-Lymphatic Fistula.

Esophageal tuberculosis (TB) is a rare finding, and esophageal TB fistulas are even rarer. In general, esophageal fistulas are an uncommon finding in everyday practice, but it is frequently seen in advanced endoscopy centers. It can be either congenital or acquired. The acquired type is most commonly a complication of advanced mediastinal malignancy. Other etiologies include trauma, radiation, infections, surgery, aortic aneurysms, or esophageal stents. The most common type of esophageal fistulas is trachea-esophageal. Other types include esophago-aortic, esophago-lymphatic, and esophago-respi

Laparoscopic treatment of ureteroduodenal fistula: Case report, technique, and review.

Ureteroduodenal fistula is extremely rare, with fewer than 20 reported cases. We describe a 50-year-old woman with recurrent flank pain and hematuria whose imaging showed proximal ureteral hydronephrosis, a 2-cm stone, and a migrated metallic foreign body. During laparoscopic ureterolithotomy, dense adhesions revealed a fistula caused by transduodenal needle penetration with associated stone formation. Laparoscopic removal of the stone and foreign body, duodenal repair, and ureteral suturing with stent placement were successfully performed. The patient recovered uneventfully and remained asymp

[Management strategies and research progress in pancreatic fistula after pancreaticoduodenectomy].

Pancreaticoduodenectomy is the standard surgical procedure for treating malignant tumors in the head and neck of the pancreas, the uncinate process, the ampulla of Vater, and the lower segment of the bile duct. The operation involves multi-organ resection and complex gastrointestinal reconstruction. Pancreatic fistula is a common complication after pancreaticoduodenectomy, often leading to infections, bleeding, and other complications, which can result in perioperative mortality. In recent years, with advancements in surgical techniques and concepts, as well as close collaboration among multid

Purpose: To describe a choroidal detachment resulting from an indirect carotid-cavernous fistula. Methods: An observational case report was evaluated, and a literature review was performed. Results: A 97-year-old woman presented with unilateral dilated episcleral vessels. A posterior segment examination showed serous choroidal detachments in all 4 quadrants with associated retinal hemorrhages. A subsequent cerebral angiogram showed a Barrow type D carotid-cavernous fistula. The patient was observed, and the serous choroidal detachments slowly improved. Conclusions: In a patient with a choroida

Extensive Arteriovenous Fistula Thrombosis With Glucagon-Like Peptide-1 Agonist.

Glucagon-like peptide-1 (GLP-1) receptor agonists are widely used due to their significant anti-atherosclerotic, cardiovascular, and renoprotective benefits. However, potential adverse effects must also be considered. Here, we present a rare case of GLP-1 receptor agonist-induced thrombotic complications in a middle-aged African American female, contributing to the limited existing literature on this topic. A 55-year-old African American female with a past medical history of deceased donor renal transplantation presented with pain, swelling, and erythema localized to the left arteriovenous (AV

Pancreatico-cutaneous fistula following complicated perforated peptic duodenal ulcer repair.

Pancreatic fistulas are a known complication following gastrointestinal surgeries, especially when there is disruption to pancreatic tissue or ducts. These fistulas occur due to leakage of pancreatic enzymes, which can lead to inflammation, infection, and other complications. While typically associated with pancreatic resections, fistulas can develop when the ulcer involves the pancreas leading to pancreatic ductal disruption. A 59-year-old female presented with hyperacute epigastric pain, hematemesis, septic shock with a blood pressure of 89/62, a Boey score of 3, and a Pulp score of 4. Imagi

Colo-cutaneous fistula: an unusual complication after hemilaminectomy and microdiscectomy.

Although the current literature points to surgical complication as the most common cause of entero-cutaneous fistulae, the formation of a colo-cutaneous fistula following a microdiscectomy is an unusual complication. Derivation of iatrogenic fistulae is rare, however, can carry increased rates of patient mortality and may require definitive surgical intervention. The authors present a clinical manifestation of a colo-cutaneous fistula between the descending colon and lumbosacral back wound from a previous microdiscectomy.

Acquired cutaneous fistula in the periocular area: A case report.

BACKGROUND: Lacrimal sac rhinosporidiosis, with nil or minor nasal extensions, rarely presents as an acquired cutaneous fistula in the periocular area. The correct diagnosis in such cases can be challenging, leading to repeated failure of conservative or surgical interventions. CASE SUMMARY: A 39-year-old female presented with a 6-year history of swelling in the periocular area, specifically in the left lacrimal sac area. Symptoms were limited to epiphora and constant mucoid discharge from the fistula, clinically mimicking chronic lacrimal sac fistula. She had a history of treatment with mult

Management of Congenital Palatal Fistula Associated With Wiskott-Aldrich Syndrome.

Submucous cleft palate (SMCP) is a rare form of cleft palate involving abnormal muscle development in the soft palate. Diagnosis is often delayed, with many cases identified after age 4. Symptoms like hypernasality and velopharyngeal insufficiency (VPI) may appear, though some remain asymptomatic. SMCP diagnosis typically relies on Calnan's triad: bifid uvula, hard palate notch, and zona pellucida. Wiskott-Aldrich syndrome (WAS) is a rare X-linked immune disorder marked by thrombocytopenia, eczema, and infections. Early diagnosis is key to effective treatment. Due to the rarity of both SMCP an

Concurrent Congenital Umbilicobiliary Fistula and Vesicourachal Diverticula in a Dog.

Congenital umbilicobiliary fistula is a rare disease reported in humans and dogs. A 2-month-old, intact, male French Bulldog presented with a greenish-yellow discharge dripping from the umbilicus since birth. Complete blood count and serum biochemistry were within normal limits, but serum alkaline phosphatase activity was mildly elevated. A positive contrast cystogram was performed to rule out a patent urachus and confirmed the presence of a vesicourachal diverticula, a type of urachal anomalies. An abdominal ultrasound and computed tomography fistulogram demonstrated a communication between t

Aortoesophageal fistula as a rare cause of upper gastrointestinal hemorrhage.

An aortoesophageal fistula (AEF) is an exceedingly rare and life-threatening cause of upper gastrointestinal bleeding, particularly in patients with vascular prostheses. Endoscopic findings in such cases are scarcely described. We describe a 73-year-old male admitted to the emergency department with hematemesis and altered consciousness. His history included ascending thoracic aortic dissection repair with endovascular prosthesis placement 25 years prior. Upon admission, he presented with signs of hypovolemic shock and hemoglobin of 6.5 g/dL. Following hemodynamic stabilization, upper gastroin

Video-assisted thoracoscopic surgery in pancreaticopleural fistula: A case report.

Pancreatico-pleural fistula (PPF) is sequelae of pancreatitis. It is more commonly seen in alcoholic pancreatitis with abdominal symptoms and signs. PPF presenting with respiratory symptoms and signs in the absence of abdominal signs is rare. Moreover, trivial trauma which went unrecognised in a 14-year-old child for 3 months resulting in PPF has not been reported in the literature. This chronic PPF results in the formation of adhesions between the lung and pleura. These adhesions result in the incomplete expansion of the lungs even after thoracocentesis. The present case highlights the signif

Suspicion and Treatment of Perilymphatic Fistula: A Prospective Clinical Study.

Background: Since the discovery of the perilymphatic fistula (PLF), the diagnosis and treatment remain controversial. If successfully recognized, the PLF is surgically repairable with an obliteration of the fistula site. Successful treatment has a major impact on patient's quality of life with an improvement in their audiological and vestibular symptoms. Objective: To prospectively investigate patients' clinical and audiological evolution with PLF suspicion after middle ear exploration and obliteration of the round and oval window. Study Design: Prospective comparative study. Setting: Tertiary
