Cryptococcal choroiditis.

Two patients with acquired immune deficiency syndrome presented with headaches and fevers. A diagnosis of cryptococcal meningitis was made by lumbar puncture and elevated cryptococcal antigens. Complaints of decreased vision in both patients led to the diagnosis of optic disc edema and cryptococcal choroiditis with yellow-white choroidal infiltrates noted in both eyes of the two patients. Systemic treatment with amphotericin B and 5' flucytosine led to resolution of the choroidal infiltrates. Late visual acuity loss was believed to be secondary to optic atrophy.

Retinal pigment epithelial tear in reactive lymphoid hyperplasia of uvea.

A 76-year-old patient developed cystic exudative retinal detachment with many yellowish subretinal precipitates and a large retinal pigment epithelial (RPE) tear in midperiphery of the right eye in the course of polyclonal hypergammaglobulinemia. The left eye showed localized RPE detachment in the posterior pole which gradually extended. Systemic steroid administration reduced the subretinal precipitates to some extent, but suspicion of malignancy led to enucleation of the right eye after unsuccessful diagnostic vitrectomy. A histopathological study revealed massive infiltration of the uvea wi

Vitrectomy for complicated retinal detachments secondary to branch retinal vein occlusions.

Combined rhegmatogenous and traction retinal detachments associated with branch vein occlusions occurred in five eyes of five patients between Jan. 1, 1986 and Dec. 31, 1987. Four patients underwent surgery with pars plana vitrectomy and intravitreal gas, with or without scleral buckling. One patient refused treatment. All operated on eyes had attached retinas at a mean follow-up of seven months. Because posterior traction plays an important role in these unusual detachments, consideration should be given to pars plana vitrectomy and air-fluid exchange rather than scleral buckling alone.

Central retinal-artery occlusion in a patient with circulating lupus anticoagulant.

The circulating lupus anticoagulant is a spontaneously acquired immunoglobulin that has been associated in vivo with an increased risk of thrombotic episodes. We describe the case of a young woman with central retinal-artery occlusion associated with circulating lupus anticoagulant in the absence of systemic lupus erythematosus or any other autoimmune or collagen vascular disease. In cases of central retinal-artery occlusion without apparent cause, the presence of the lupus anticoagulant should be excluded especially in cases that have been exposed to the human immunodeficiency virus.

Bilateral retinal vascular hypoplasia associated with persistence of the primary vitreous: a new clinical entity?

Three infant boys with a bilateral congenital retinopathy characterized by retinal vascular hypoplasia and persistence of the primary vitreous are described. The infants were healthy and had no non-ocular physical abnormalities. None had histories of prematurity or low birth weight. Family histories were negative to similar retinopathy, and the fundi of all examined blood relatives were normal. The described congenital retinopathy is compared and contrasted with persistent hyperplastic primary vitreous, Norrie's disease, idiopathic retinal dysplasia, familial exudative vitreoretinopathy, retin

Anterior ischemic optic neuropathy. VIII. Clinical features and pathogenesis of post-hemorrhagic amaurosis.

Visual disturbance after marked and/or recurrent blood loss has been known for at least 25 centuries, since Hippocrates; however, so far its clinical features have been controversial and its pathogenesis enigmatic. The author studied seven patients, four of whom were seen soon after the visual loss and followed prospectively. A detailed review of the extensive literature and analysis of the cases provide relevant information on the subject. The blood loss is usually from the gastrointestinal (GI) tract, less often from other sites. There is typically a time lag between the bleeding and the ons

Familial optic atrophy with sex-influenced severity. A new variety of autosomal-dominant optic atrophy?

A family is described with 20 members in three successive generations affected by optic atrophy without other ocular or extraocular manifestations. The anomaly was transmitted as an autosomal-dominant character. There was a clearly bimodal distribution of severity: 4 male patients complained of severe impairment of vision since childhood while 16 other subjects (7 males and 9 females) were completely asymptomatic. This family could be an example of a new variety of autosomal dominant optic atrophy characterized by sex-influenced severity.

Ocular manifestations of adult Niemann-Pick disease: a case report.

Niemann-Pick Disease Type B in a 33-year-old man is described. No family, racial or hereditary factors were noted in this case as in other reports in the literature. The ocular abnormalities in our patient were mild myopia, a mild generalized colour abnormality, and fundi showing a lipid ring-form opacity about the foveolas resulting in minimal visual impairment. Sphingomyelinase deficiency was found with the resulting deposition of phospholipid occurring most probably in the Müller cell. No neurological features were present and the patient was otherwise well.

Isolated retinal telangiectatic masses.

The clinical findings of 20 patients with an isolated peripheral retinal mass secondary to retinal telangiectasis were reviewed. The important clinical features were subretinal exudation, vitreous haemorrhage and retinal detachment. No associated systemic abnormalities were identified. Early results suggest that cryotherapy is effective for the reduction of subretinal exudation but not for macular oedema. Patients with extensive retinal detachment complicated by vitreous haemorrhage and epiretinal membrane formation were treated with vitreoretinal surgery.

Reversible visual loss in pseudotumor cerebri.

A patient with chronic pseudotumor cerebri experienced precipitous visual loss as a result of a central retinal vein occlusion associated with an intracranial pressure elevation of 520 mm of CSF and a fall of mean systemic arterial pressure. Optic nerve sheath decompression was associated with substantial improvement of vision.

Posttraumatic syndrome as a sequel to less severe cranio-cerebral injuries.

By means of clinical examinations, the acid/base balance, angiography, scintigraphy of the brain, EEG and other methods, the author examined 95 patients presenting the respective signs and deals with the etiology, the course and the treatment of a clinical syndrome that appears in the way of a cerebral contusion after minor craniocerebral traumas. Cortical congestion of the blood flow in the region of the brain exposed to the injury were found. Because of a vascular dilatation, the volume of the respective region increases resulting in sings of the intracranial pressure increase. A typical sig

Retinal complications of optic disc drusen.

Following reports by Sanders et al. and Wise et al. of optic disc drusen associated with retinal bleeding, an additional 19 cases were studied and followed up for 1 to 10 years (average 5 years). Four of them showed haemorrhages at the optic disc and its borders only, but 15 showed extensive retinopathy with macular involvement. In 3 cases retinopathy was serous and not haemorrhagic. Apart from cases in which bleeding was limited to the disc and its immediate vicinity the main clinical features of the syndrome were as follows: (1) the presence of optic disc drusen; (2) extensive retinal haemor

Microsurgical management of macular epiretinal membranes (macular pucker).

Surgical management of selected macular epiretinal membranes is possible through the use of microsurgical pars plana vitreoretinal membranectomy. Visual improvement can be significant with resolution of traction retinal detachment and retinal distortion. Complications are similar to other forms of vitreous surgery. In a series of 9 selected cases with severe reduction of visual acuity secondary to macular pucker, microsurgical membranectomy was performed with encouraging results.

Bilateral juxtapapillary subretinal neovascularization associated with pseudotumor cerebri.

A 32-year-old obese woman with hypertension and a three-year history of pseudotumor cerebri developed bilateral juxtapapillary subretinal neovascular membranes. To our knowledge, this is the first reported case of bilateral subretinal neovascular membranes complicating the course of this disease. The subretinal neovascular membrane in the left eye spontaneously involuted, but because the membrane in the right eye threatened the foveola, the patient underwent argon-laser photocoagulation. The subretinal fluid and hemorrhage progressively resolved, the membrane was replaced by fibrous tissue, an

Retrolental Fibroplasia. II. Pathologic correlation.

Retinas of 142 eyes from 71 premature infants on whom autopsies had been done were studied by gross observation and by using PAS-stained whole mount, trypsin-digest, and conventional histological techniques. With these techniques, vascular pathologic condition was correlated with fluorescein angiograms in living infants with acute retrolental fibroplasia. The specific lesions seen in acute phase were a major arteriovenous shunt in the eye, microvascular changes including tufting, and obliteration of capillaries around arteries and veins. Regression occurred by vascular budding from the anterio

Diagnostic features of the Favre-Goldmann syndrome.

Extensive retinal vascular disease was noted in three patients from two families with Favre-Goldmann syndrome. In addition to classical features they had pronounced leakage from some retinal vessels. Vessels were either opaque ('sclerotic') or non-perfused. Cystoid macular oedema was a contributing cause of decreased vision. Two of the three patients showed a discrepancy on electroretinography between single-flash photopic amplitudes and flicker fusion frequency. This may be characteristic of Favre-Goldmann syndrome.
