An Incidental Finding of a Talonavicular and Talocalcaneal Joint Coalition After a Tibial Pilon Fracture: A Case Report.

It has been proposed that patients with talocalcaneal and talonavicular coalitions have decreased ankle joint range of motion. It has also been reported that rotational forces regularly absorbed by the talocalcaneal joint are transferred to the ankle joint in patients with coalitions, increasing the stress on the ankle joint after trauma. To the best of our knowledge, only 1 reported study has detailed the increased stress placed on the ankle joint secondary to a coalition. We present a case study of a 53-year-old female who experienced a traumatic fall and subsequent right ankle fracture. Adv

Unilateral Dislocation of the Posterior Tibialis Tendon (PTT) and Flexor Digitorum Longus Tendon With Contralateral PTT Subluxation in a Patient With Congenitally Shallow Flexor Groove.

INTRODUCTION: Flexor tendon dislocation from the flexor tendon groove posterior of the medial malleolus has been previously described, and may be difficult to diagnose initially, but is amendable to surgical treatment with good outcomes. We present a unique case of unilateral dislocation of the posterior tibialis and flexor digitorum longus tendons with contralateral flexor digitorum longus subluxation that was treated surgically with a good outcome. CASE PRESENTATION: A 37-year-old active duty male sustained a dislocation and subluxation of the flexor tendons bilaterally after a forced dorsi

Responsiveness of the foot and ankle ability measure (FAAM) in individuals with diabetes.

BACKGROUND: The impact of diabetes on physical function pose a challenge in assessing clinical outcomes. OBJECTIVE: The purpose of this study was to provide evidence of responsiveness for the foot and ankle ability measures (FAAM) in individuals with diabetes mellitus. METHODS: The two most recent FAAM scores of 155 diabetic patients treated for foot/ankle pathology were analyzed. Based on physical component summary (PCS) scores of the SF-36, subjects were categorized as improved (>7-point positive change), worsened (>7-point negative change), or unchanged (<7-point change). Analyses of the

Surgical management of severe deformity resulting from posterior tibial tendon dysfunction.

A retrospective case study format was used to evaluate the results of surgical treatment of severe deformities due to long-standing tibialis posterior tendon dysfunction. To be included in the study, the peri-talar (talonavicular) subluxation on a weight-bearing AP X-ray should have been at least 75%. This criterion limited the field to patients with long-standing, rigid, severe deformities. Surgical correction was obtained through a two-incision approach, dorsomedial and dorsolateral. Significant shortening of the medial column and extensive removal of adaptive bone formation was necessary to

Double arthrodesis in the adult.

Twenty-four double arthrodeses (24 patients) were evaluated at an average of 56 months postoperatively. Sixteen arthrodeses were performed for adult acquired flatfoot attributable to posterior tibial tendon insufficiency (16 patients), and results were compared with the results of eight patients undergoing arthrodesis for other diagnoses. The overall satisfaction rate was 83%, with 76% of patients having good and excellent results. Considerable improvements were observed in pain and function indices, with similar outcomes observed in the patients with and without acquired flatfoot. However, co

Postmortem computed tomography and histopathology of distal limb lesions in giraffe (Giraffa spp.) under managed care.

Foot structure and function are essential to the health and mobility of giraffe (Giraffa spp.), yet comprehensive evaluations of distal limb pathology remain limited. This retrospective necropsy-based study used postmortem computed tomography (CT) of frozen distal limbs with targeted histopathology to characterize osseous and soft-tissue changes in a historical cohort of giraffe under managed care. Seventy-three distal limbs from 21 giraffe housed at 12 North American zoos (necropsies performed between 2004-2023) were evaluated. Lesions were graded on a scale of 0-3, and localized to either th

Total ankle arthroplasty via lateral transfibular approach with and without concomitant subtalar arthrodesis : radiological alignment, patient-reported outcomes, and range of motion at ≥ five-year follow-up.

AIMS: Concomitant subtalar pathology may require arthrodesis at the time of total ankle arthoplasty (TAA), yet direct comparisons of isolated TAA versus TAA + subtalar joint arthrodesis (STJA) via a lateral transfibular approach, using the same implant, remain limited. We compared ≥ five-year radiological, functional, and range of motion (ROM) outcomes. METHODS: This retrospective single-centre cohort included 119 patients from January 2015 to December 2020 with ≥ five-year follow-up: 50 underwent TAA with concomitant subtalar arthrodesis and 69 underwent isolated TAA. Outcomes were Foot and

Clinical Outcomes of Ilizarov Bone Transport for Distal Tibial Defects in Refractory Osteomyelitis: A Retrospective Study.

BACKGROUND: Distal tibial defects following refractory osteomyelitis pose a significant orthopedic challenge, necessitating effective reconstruction. The Ilizarov bone transport technique is a potential treatment option, but its clinical outcomes specifically for distal tibial infectious defects need further evaluation. METHODS: In this retrospective observational single-arm study, we analyzed the clinical data of 22 patients who had undergone multiple debridements for refractory distal tibial osteomyelitis (DTO) and had associated bone defects, and were subsequently treated with the Ilizarov

Clinical Outcomes and Return to Preinjury Sports After Anatomic Reconstruction With a Gracilis Autograft Versus the Modified Broström Procedure in Patients With Generalized Joint Laxity.

BACKGROUND: Generalized joint laxity (GJL) is a risk factor for inferior outcomes after the modified Broström procedure for chronic lateral ankle instability, while anatomic reconstruction with tendons is more inclined to be recommended. However, whether anatomic reconstruction could achieve better results than the modified Broström procedure in patients with GJL is unknown. PURPOSE: To compare clinical outcomes and return to sports between anatomic reconstruction and the modified Broström procedure in patients with GJL. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Patients wi

Prevention of Equinus Deformity after External Fixation for Open Tibia Fractures: A Comparison between Plaster of Paris Backslab and Passive Ankle Physiotherapy.

BACKGROUND: Ankle equinus deformity is a common complication of prolonged external fixator use in tibia fractures with an incidence of 15%-16%. It affects gait and may lead to foot, leg, and back problems. Treatment of equinus deformity increases the cost of treatment of open tibia fractures and increases the time spent off work. Several preventive modalities have been suggested in literature with variable success, but no standard protocol exists. OBJECTIVES: The aim of this study was to determine the most appropriate method of preventing equinus deformity of the ankle joint during the manage

Burden of Chronic Musculoskeletal Disorders amongst Patients Attending Orthopaedic Outpatients Clinic of a Tertiary Hospital, North-Western Nigeria.

BACKGROUND: Musculoskeletal disorders often pose significant limitation to activities of daily living due to pain or disability. These disorders often arise as a result of the normal aging process, but most have some predisposing factors that are lifestyle-related, especially in younger people as a result of repeated mal- posturing/ misuse of parts of the body in the course of their jobs or recreations. These disorders cause more functional limitations in the older adult population than any other group of disorders. Thus, with increasing aging of most populations of the world, it is expected t

Comparative Study of Intramedullary Hammertoe Fixation.

BACKGROUND: Temporary Kirschner wire fixation (K-wire) is a widely used, low-cost fixation method for the correction of hammertoe deformity. Reported complications associated with K-wires prompted the development of new implants over the past decade. However, there is a lack of literature on comparative studies analyzing functional outcomes using validated questionnaires. The purpose of this study was to analyze functional outcomes in patients who had undergone proximal interphalangeal joint fusion using 2 types of intramedullary implant, the Smart Toe and the TenFuse, and to compare them with
