[Association between high vitamin B12 levels and one year mortality in older people admitted to the hospital].

BACKGROUND: Supplementation of vitamin B12 in older adults is a common practice to avoid vitamin B12 insufficiency. However, there is a paucity of information about the effects of cobalamin excess. AIM: To asses any potential effects of high levels vitamin B12 on mortality on adults aged ≥ 65 years admitted to an internal medicine service. MATERIAL AND METHODS: We Prospectively studied patients admitted to an internal medicine service of an academic hospital from September 2017 to September 2018, who were able to give their consent and answer questionnaires. We tabulated age, gender, medical

The influence of low social support and living alone on premature mortality among aging Canadians.

OBJECTIVES: Among older Canadians, we examined the influence of low social support, a weak sense of belonging, and living alone to understand their role on mortality risk in Canada. METHODS: We conducted a retrospective cohort study of older Canadians surveyed in 2001 from the Canadian Community Health Survey and followed up with death events through December 31, 2011 from the Canadian Vital Statistics Database. Analyses were stratified by age: 55-64 and 65 and older. Social support was assessed using measures developed for the Medical Outcomes Study. A sense of belonging, a binary measure, m

One-year mortality after hip fracture surgery and prognostic factors: a prospective cohort study.

Older adult patients with hip fractures are 3-4 times more likely to die within one-year after surgery than general population. The study aimed to identify independent predictive factors associated with one-year mortality after hip fracture surgery. A prospective prognostic cohort study was performed. All patients aged ≥65 years, consecutively admitted in three Italian hospitals with a diagnosis of fragility hip fracture were included. Patients with periprosthetic or pathological fractures were excluded. Multivariate analysis was used to determine variables that significantly increased the ris

Skeletal muscle wasting in chronic heart failure.

Patients suffering from chronic heart failure (CHF) show an increased prevalence (~20% in elderly CHF patients) of loss of muscle mass and muscle function (i.e. sarcopenia) compared with healthy elderly people. Sarcopenia, which can also occur in obese patients, is considered a strong predictor of frailty, disability, and mortality in older persons and is present in 5-13% of elderly persons aged 60-70 years and up to 50% of all octogenarians. In a CHF study, sarcopenia was associated with lower strength, reduced peak oxygen consumption (peak VO2 , 1173 ± 433 vs. 1622 ± 456 mL/min), and lower e

Low serum creatinine is a type 2 diabetes risk factor in men and women: The Yuport Health Checkup Center cohort study.

AIM: Type 2 diabetes (T2D) is a risk factor for muscle loss and subsequent frailty. The reverse association, however, may also happen. This study examined whether serum creatinine level, an indicator of muscle mass, predicted diabetes development. In addition, a role for body mass index (BMI) as an effect modifier of creatinine levels was evaluated. METHODS: This cohort study included 9667 subjects without diabetes or hypertension and with normal creatinine levels at baseline. Multiple-adjusted hazard ratios (HRs) for associations between baseline creatinine and diabetes development were esti

The impact of transcatheter aortic valve implantation on quality of life: results from the German transcatheter aortic valve interventions registry.

OBJECTIVES: Transcatheter aortic valve implantation (TAVI) has proven a survival benefit in patients with severe symptomatic aortic stenosis (AS). Recently published data from the German TAVI registry reported a 1-year mortality rate of 19.9 %. However, there are limited data for the effect of TAVI on quality of life (QoL), especially in real life settings. METHODS: The German TAVI registry was launched to evaluate safety, effectiveness and QoL of TAVI in daily clinical routine. We analyzed health-related QoL, using the EQ-5D questionnaire. RESULTS: Quality-of-life data were eligible for 460

Impact of sarcopenia on outcomes following intra-arterial therapy of hepatic malignancies.

BACKGROUND: Assessment of patient performance status is often subjective. Sarcopenia--measurement of muscle wasting--may be a more objective means to assess performance status and therefore mortality risk following intra-arterial therapy (IAT). METHODS: Total psoas area (TPA) was measured on cross-sectional imaging in 216 patients undergoing IAT of hepatic malignancies between 2002 and 2012. Sarcopenia was defined as TPA in the lowest sex-specific quartile. Impact of sarcopenia was assessed relative to other clinicopathological factors. RESULTS: Indications for IAT included hepatocellular ca

Association of measured physical performance and demographic and health characteristics with self-reported physical function: implications for the interpretation of self-reported limitations.

BACKGROUND: Self-reported limitations in physical function often have only weak associations with measured performance on physical tests, suggesting that factors other than performance commonly influence self-reports. We tested if personal or health characteristics influenced self-reported limitations in three tasks, controlling for measured performance on these tasks. METHODS: We used cross-sectional data on adults aged >/= 60 years (N = 5396) from the Third National Health and Nutrition Examination Survey to examine the association between the repeated chair rise test and self-reported diff

The Onset of ADL Difficulties and Changes in Health-Related Quality of Life.

BACKGROUND: The effect of the onset of difficulties with activities of daily living (ADLs) on the health-related quality of life (HRQoL) of older adults is not well understood. We identified strong longitudinal associations between ADL onset and HRQoL changes for older adults in Medicare Advantage Organizations (MAOs). METHODS: We analyzed 473,282 age-eligible MAO beneficiaries in the 2008-2013 Medicare Health Outcomes Surveys (M-HOS) who reported no ADL difficulties at baseline and completed their two-year follow-ups in 2010-2015. The four HRQoL measures were the physical and mental health c

Handling missingness in electronic health record laboratory data for secondary use: a decision-making process.

Electronic health record (EHR) systems have become widely adopted, prompting a growing interest in leveraging EHR data for secondary use to improve clinical knowledge and enhance patient care, for example, by prediction modelling. A common challenge in EHR-derived healthcare datasets is missing data, which is frequently insufficiently addressed in publications. Missing data must be handled appropriately to ensure the quality of prediction models. We propose that combined data-centred and clinical domain expertise is needed for this. This paper demonstrates a stepwise decision-making process fo

Multikingdom Gut Microbiome and Immune Aging: A Host-Microbial Signaling Perspective.

Immune aging comprises heterogeneous changes in hematopoietic output, immune-cell function, inflammatory regulation, and tissue-specific immune states. In parallel, the gut microbiome undergoes ecological and functional remodeling across the bacteriome, mycobiome, virome, and archaeome, but these changes do not define a single "aged microbiome" and vary with host health, frailty, diet, medication exposure, and environmental context. Here, we examine how multikingdom gut microbiome remodeling intersects with distinct trajectories of immune aging. Rather than treating dysbiosis as a uniform upst

Integrated physical and mental healthcare for older adults in Europe: from concept to implementation-a joint EuGMS and EAGP perspectives paper.

Older adults commonly experience coexisting mental and physical multimorbidity, alongside frailty and polypharmacy. This clinical complexity is associated with poorer health outcomes, including higher rates of acute hospital admission, longer lengths of stay, increased dependency at discharge and higher mortality. Despite this, physical and mental health services for older adults are fragmented across Europe, often operating in a parallel rather than integrated manner. As a result, many patients receive poorly coordinated care. In response, there is growing interest in the development of integ

Fitness beyond age: Multidisciplinary expert opinion on redefining fitness for targeted therapies in chronic lymphocytic leukemia.

Advancements in the understanding of chronic lymphocytic leukemia (CLL) have transformed patient care, leading to the development of novel targeted therapies. Traditionally, patient fitness for treatment was based on the tolerability of chemoimmunotherapy such as fludarabine-cyclophosphamide-rituximab. As the CLL patient population is predominantly older, age has historically been a major factor in how a physician selects a patient's treatment. However, as a patient's fitness goes beyond age, the definition of patient fitness should also evolve. Here, we provide suggestions for the current bes

Caring for older adults in the Emergency Department: a Delphi-based national consensus by the Academy of Emergency Medicine and Care and the Italian Society of Gerontology and Geriatrics.

PURPOSE: The number of older adults attending Emergency Departments (EDs) is increasing rapidly in Italy. This trend poses substantial challenges to services that are not structured to address multidimensional needs. Despite the availability of international guidance, implementation of geriatric emergency care models remains inconsistent in Italy. This study aimed to establish a national, inter-specialty expert consensus on priorities for improving geriatric emergency care. METHODS: A two-round Delphi process was conducted involving 32 experts (16 geriatricians and 16 emergency physicians) fr

Dynamic profiling in inflammatory Bowel disease: A manifesto for personalized care.

Inflammatory bowel disease (IBD), encompassing Crohn's disease and ulcerative colitis, is a heterogeneous chronic condition whose clinical management has been advanced by the availability of molecularly targeted therapies. Despite this progress, marked inter- and intra-patient variability limits the effectiveness of a one-size-fits-all treatment approach. Precision medicine is a promising solution, but its implementation in routine care is limited. In this manifesto, we propose a multidimensional conceptual framework for the dynamic clinical profiling of IBD patients to be used in real-world c
