Cystic Fibrosis Modulator Therapies.

Cystic fibrosis (CF) is an inherited multisystemic disease that can cause progressive bronchiectasis, pancreatic endocrine and exocrine insufficiency, distal intestinal obstruction syndrome, liver dysfunction, and other disorders. Traditional therapies focused on the treatment or prevention of damage to each organ system with incremental modalities such as nebulized medications for the lungs, insulin for diabetes, and supplementation with pancreatic enzymes. However, the advent of highly effective modulator therapies that target specific cystic fibrosis transmembrane conductance regulator prot

The Relationship Between Diabetes Mellitus and Cancers and Its Underlying Mechanisms.

Epidemiological studies suggest associations between diabetes mellitus and some cancers. The risk of a number of cancers appears to be increased in diabetes mellitus. On the other hand, some cancer and cancer therapies could lead to diabetes mellitus. Genetic factors, obesity, inflammation, oxidative stress, hyperglycemia, hyperinsulinemia, cancer therapies, insulin and some oral hypoglycemic drugs appear to play a role in the crosstalk between diabetes mellitus and cancers. This review summarized the associations between various types of diabetes and cancers and updated available evidence of

Precision medicine for autoimmune hepatitis.

Autoimmune hepatitis (AIH) is an autoimmune liver disease induced by environmental factors in genetically susceptible individuals. AIH is characterized by hypergammaglobulinemia, elevation of serum autoantibodies and transaminases, and interface hepatitis. Personalized therapy is necessary in AIH because of its heterogeneity in clinical manifestations. Precision medicine is a recent and novel therapeutic pattern which ultimately aims to achieve personalized therapy. In this review we summarize the research progress of precision medicine to treat AIH by an exploration of the susceptible genes,

Comprehensive genetic analysis based on multi - omics reveals novel therapeutic targets for mitral valve prolapse and drug molecular dynamics simulation.

OBJECTIVE: Mitral valve prolapse (MVP), the most prevalent primary valvular disease, serves as a direct risk factor for multiple cardiovascular disorders and exhibits a high prevalence in the general population. As no specific pharmacological therapies currently exist for MVP, the identification of precise therapeutic targets is imperative. METHOD: We conducted comprehensive causal genetic inference by integrating genetic data from expression quantitative trait loci (eQTL) and genome-wide association studies (GWAS). Analytical approaches included Mendelian Randomization (MR), colocalization a

Metabolic enzyme-based cancer therapy: Mechanisms, delivery strategies, and translational challenges.

Cancer remains one of the leading causes of global mortality, underscoring the need for therapeutic strategies that are both effective and selective. Enzyme-based cancer therapy has emerged as a promising approach by exploiting tumor-specific metabolic dependencies, redox vulnerabilities, and nutrient auxotrophy. This review critically examines recent advances in anticancer enzyme therapy, focusing on amino acid-degrading and redox-modulating enzymes, their mechanisms of action, and translational potential. We integrate insights across enzyme classes to highlight shared therapeutic principles,

The combination of osimertinib and chemotherapy successfully enabled surgical resection of locally advanced lung adenocarcinoma: a Case Report.

We present the case of a 33-year-old female with a 4.3 × 3.2 cm high-density mass in the apical segment of the right upper lobe, detected on non-contrast chest CT, which showed suspected invasion of the chest wall and other tissues. CT-guided lung biopsy confirmed lung adenocarcinoma, and genetic testing revealed an EGFR exon 19 deletion. The patient received neoadjuvant therapy with oral Osimertinib combined with chemotherapy. After 4 months of treatment, she underwent thoracoscopic right upper lobectomy, mediastinal lymph node dissection, and pleural adhesion lysis. Postoperative management

ApoC-III Antisense Oligonucleotide Therapy Used in Glycogen Storage Disease Type 1a.

BACKGROUND: Glycogen storage disease type 1a (GSD1a) is an autosomal recessive disorder characterized by impaired glycogenolysis and gluconeogenesis. Individuals with GSD1a manifest significant hypertriglyceridemia refractory to dietary and lipid-lowering medication therapies. Apolipoprotein C-III antisense oligonucleotide (apoCIII-ASO) therapy is approved to reduce severe hypertriglyceridemia in familial chylomicronemia syndrome but is also effective in moderate hypertriglyceridemia of varied genetic backgrounds. EARLY REPORTS SUMMARY: We describe a 22-year-old with GSD1a and hypertriglyceri

Treatment of Epidermolysis Bullosa and Future Directions: A Review.

Epidermolysis bullosa (EB) comprises rare genetic disorders characterized by skin and mucosal membrane blistering induced by mechanical trauma. Molecularly, pathogenic variants affect genes encoding proteins crucial for epidermal-dermal adhesion and stability. Management of severe EB is multidisciplinary, focusing on wound healing support, ensuring that patients thrive, and complication treatment. Despite extensive research over 30 years, novel therapeutic approaches face challenges. Gene therapy and protein therapy struggle with efficacy, while regenerative cell-based therapies show limited e

Identification and Management of Aggressive Meningiomas.

Meningiomas are common primary central nervous system tumors derived from the meninges, with management most frequently entailing serial monitoring or a combination of surgery and/or radiation therapy. Although often considered benign lesions, meningiomas can not only be surgically inaccessible but also exhibit aggressive growth and recurrence. In such cases, adjuvant radiation and systemic therapy may be required for tumor control. In this review, we briefly describe the current WHO grading scale for meningioma and provide demonstrative cases of treatment-resistant meningiomas. We also summar

[Homozygous hypercholesterolemia - new therapeutic options in cases with complete lack of LDL- receptor].

Homozygous hypercholesterolemia is an extremely rare genetic disorder caused by mutations in the LDL receptor gene or occasionally by mutations in other genes like proprotein convertase subtilisin / kexin 9 (PCSK9). Gold standard of homozygous hypercholesterolemia therapy is apheresis, accompanied by high-dose statin and ezetimibe therapy. The cholesterol-lowering effect can be supported by new agents like inhibitors of microsomal triglyceride transfer protein (lomitapide), or by enhancing LDL catabolism through inhibition of the PCSK9 activity. We present the case of a young woman with homozy

[Repeated pulse corticosteroid therapy in Vogt-Koyanagi-Harada disease. A case report].

Vogt-Koyanagi-Harada disease is an autoimmune bilateral uveitis that occurs in people with genetic sensitivity. Diagnosis was based on the association of ocular inflammatory manifestations such as diffuse choroiditis, with or without anterior uveitis, and extraocular manifestations such as meningismus, tegumentary or auditory findings. Intravenous pulses of corticosteroid followed by oral corticosteroid is the mainstay therapy. We present the case of a woman who showed a first improvement of symptoms with three consecutive daily pulses of corticosteroid, but none during the following oral ther

Ischemic stroke in a man using estrogen.

Ischemic cerebrovascular disease has been associated with oral contraceptives but not standard hormone replacement in women. It is unknown whether or not estrogen therapy in a male is associated with an increased risk of stroke. We report a single case of a genetic male taking high doses of estrogen and progesterone who suffered a stroke. A 46-year-old man was using high doses of estrogen and progesterone while awaiting surgical gender reassignment. He had suffered a deep venous thrombosis 1 year prior to presentation. He developed an acute left visual field cut, left neglect, and left hemipar

Synthetically designed anti-defense proteins overcome barriers to bacterial transformation and phage infection.

Bacterial defense systems present considerable barriers to both phage infection and plasmid transformation. These systems target mobile genetic elements, limiting the efficacy of bacteriophage-based therapies and restricting genetic engineering applications. Here, we employ a de-novo protein design approach to generate proteins that bind and inhibit bacterial defense systems. We show that our synthetically designed proteins block defense, and that phages engineered to encode the synthetic proteins can replicate in cells that express the respective defense system. We further demonstrate that a

Lipoprotein(a) and Cardiovascular Disease: From Genetic Risk Factor to Therapeutic Target.

Lipoprotein(a) [Lp(a)] is a causal, genetically determined risk factor for atherosclerotic cardiovascular disease (ASCVD) and calcific aortic valve stenosis (CAVS). Although elevated Lp(a) affects approximately 20% of the global population, specific pharmacological options have long been unavailable, leaving a major gap in residual risk management. This review synthesizes current understanding of Lp(a) molecular architecture, genetics, and metabolism, and integrates mechanistic evidence linking Lp(a) to pro-atherogenic, pro-inflammatory, and pro-thrombotic pathways. We summarize epidemiologica

A patient-specific induced pluripotent stem cell and neural stem cell resource for angelman syndrome with 15q11.2-q13 deletion.

Angelman syndrome (AS) is a severe neurodevelopmental disorder most commonly caused by a maternal deletion of the 15q11.2-q13 region, resulting in loss of ubiquitin protein ligase E3A (UBE3A) function. The development of targeted therapies and a deeper understanding of AS pathogenesis require the availability of genetically accurate human models. Here, we generated and characterized a novel induced pluripotent stem cell (iPSC) line from peripheral blood mononuclear cells of a female pediatric patient with genetically confirmed deletion-type AS. The iPSC line displayed a normal karyotype, expre

GWAS meta-analysis of psoriasis identifies new susceptibility alleles impacting disease mechanisms and therapeutic targets.

Psoriasis is a common, debilitating immune-mediated skin disease. Genetic studies have identified biological mechanisms of psoriasis risk, including those targeted by effective therapies. However, the genetic liability to psoriasis is not fully explained by variation at robustly identified risk loci. To refine the genetic map of psoriasis susceptibility we meta-analysed 18 GWAS comprising 36,466 cases and 458,078 controls and identified 109 distinct psoriasis susceptibility loci, including 46 that have not been previously reported. These include susceptibility variants at loci in which the the

Genetics in Sarcoidosis and Its Burden as a Multisystem Disease.

Sarcoidosis is a complex, immune-mediated disease characterized by a broad spectrum of clinical and molecular phenotypes-often referred to as endophenotypes-some of which progress to chronic outcomes such as pulmonary fibrosis. Despite decades of research, the pathogenesis of sarcoidosis remains incompletely understood, primarily due to its clinical heterogeneity and the absence of robust preclinical models. Established risk factors include age, sex, ethnicity, geographic origin, and environmental exposures, all of which contribute to granuloma formation and the activation of profibrotic pathw
