OBJECTIVE: The objective of this study of distal radius fractures was to determine if a subchondral haematoma in an unfractured compartment predicts secondary osteoarthritis.
METHODS: In 1995-1997, 41 patients, 22 women, a median age of 41 years (20-57 years) with a displaced distal radius fracture underwent diagnostic wrist arthroscopy in addition to the fracture treatment. In 12 patients (7/12 women), subchondral haematomas were identified in a joint compartment not involved in the fracture.
RESULTS: At 13-15 years, 37 patients were still alive. Twenty-eight patients attended the follow-up

INTRODUCTION: Injuries of the hand can cause significant functional impairment, diminished quality of life and delayed return to work. However, the incidence and functional outcome of hand injuries in polytrauma patients is currently unknown. The aim of this study was to determine the incidence, distribution and functional outcome of fractures and dislocation of the hand in polytrauma patients.
METHODS: A single centre retrospective cohort study was performed at a level 1 trauma centre. Polytrauma was defined as patients with an Injury Severity Score of 16 or higher. Fractures and dislocation

BACKGROUND: Wrist hemiarthroplasty has emerged as a motion-sparing option for severe wrist arthritis. It is technically easy with advantages of limited bone resection and no risk of nonunion. Given the relative infancy of the procedure in clinical practice, there are limited data on patient outcomes.
METHODS: Eleven patients were treated with wrist reconstructive hemiarthroplasty. Indications included 1 patient with scaphoid nonunion advanced collapse, 9 patients with scapholunate advanced collapse, and 1 patient with capitolunate arthritis. Average age was 63 years; average follow-up was 4 y

Swan neck deformity, or hyperextension of the proximal interphalangeal joint, may occur secondary to trauma, rheumatoid arthritis, cerebral palsy, or Ehlers-Danlos syndrome, and can be treated with tenodesis of one slip of the flexor digitorum sublimis tendon. This technique has several variations, differing primarily in the specific location and method that a single slip of the flexor digitorum sublimis tendon is secured, but they all serve to create a static volar restraint against hyperextension. Options include tunneling the tendon through the bone of the proximal phalanx, attaching the te

Ulnar nerve palsy results in significant loss of sensation and profound weakness, leading to a dysfunctional hand. Typical clinical findings include loss of key pinch, clawing, loss of normal flexion sequence of the digits, loss of the metacarpal arch, and abduction of the small finger. Further deficits in hand/wrist function are seen in high-level ulnar nerve palsy, including loss of ring- and small-finger distal interphalangeal flexion, decreased wrist flexion, and loss of dorsal sensory innervation. This article reviews the clinical findings seen in low and high ulnar nerve palsies, and rev

The aim of this prospective, randomised, controlled trail was to compare two methods of rehabilitating extensor tendon repairs in zones V-VII. Patients who incurred simple and complete lacerations of their extensor tendons in zones V-VII enrolled into the study and underwent either static splinting (n = 25) or dynamic splinting (n = 27) after primary acute repair of tendons. Extension lag, flexion deficit, total active motion (TAM), grip strength, and functional status of upper extremities were measured. TAM was improved in the dynamic group when compared with the static group in the injured d

UNLABELLED: Factors, such as high repetition, high force and gripping play a role in the development of upper extremity work-related musculoskeletal disorders. The purpose of this study was to systematically examine the effects of push load and frequency on muscle activity with and without concurrent gripping. A total of 10 men and 10 women performed a cyclic bimanual pushing task. All combinations of three push loads (1 kg, 2 kg, 4 kg), three frequencies (4/min, 8/min, 16/min) and two grip conditions (no required grip and 30% of maximum grip force) were performed in randomised order. The musc

OBJECTIVE: Treatment methods in Kienböck's disease remain controversial despite the long-term awareness of the disease by orthopedic surgeons. Currently, excisional arthroplasty with a palmaris longus tendon ball is one of the most frequently used treatment methods. The aim of this study was to evaluate the long-term results of thirty-eight patients who had been treated with excisional arthroplasty and palmaris longus tendon ball, as well as the effect of this method on carpal height.
METHODS: In this study, we retrospectively evaluated 38 patients (mean age: 30.4 years; range: 17-64 years) w

Based on anatomic study, the vascularized capitate transposition to replace excised necrotic lunate was designed and applied in 40 cases of advanced Kienböck disease. It includes excision of the necrotic lunate and proximal shift of the vascularized capitate. The blood supply of the transposed capitate is provided by the dorsal branch of the anterior interosseous artery. Bone union occurred radiographically, and no postoperative capitate necrosis occurred in all cases after 6 weeks. Twenty-three cases were followed up for 1 year. No residual wrist pain existed in the range of motion, but limit

We experienced 16 patients with a fracture of the hook of the hamate. The routine posteroanterior view raised the suspicion of fracture in four of 13 patients (31%), the carpal tunnel view showed the actual fractures in six of 14 patients (43%), and the supine oblique radiographic view of the wrist showed fractures in eight of ten patients (80%). Computed tomography corroborated the fracture in all eight patients (100%). Computerized tomography proved most useful for accurate diagnosis of fracture of the hook of the hamate.

Six children with radial club hand had distraction lengthening of the ulna using the Ilizarov technique at the mean age of 10 years. The mean lengthening achieved was 4.7 cm (46% of original ulna length). Complications included nocturnal pain, pin track infection and callus fracture or delayed union. Distraction lengthening of the ulna can enhance the ability to perform normal activities of daily living, such as reaching the perineum or driving a car, but complications are extremely common. The high rate of callus fracture in this series reinforces the need for regular radiographic review duri

BACKGROUND: Bony and soft tissue hand injuries are common and their ensuing management can result in adhesions between tendons and the surrounding bone, soft tissues or inserted hardware. Knowledge of the patterns of motion restriction imparted by different adhesions has implications for rehabilitation protocols following injury or surgery. The aim of this study was to quantify the degree and pattern of digital motion loss due to adhesions at various levels of the flexor apparatus in a cadaveric model.
METHODS: Ten cadaveric limbs underwent range of motion assessment of the digits in a standa
