[Implantation of a vibratory mass transducer on the round window: a report of two cases].

The Vibrant Soundbridge (VSB, Med-El, Austria) is a semiimplantable hearing aid usually attached to the long process of the incus to vibrate the ossicular chain in patients with moderate to severe mixed hearing loss. We implanted a VSB vibratory transducer on the round window membrane of the left ear in two cases not treated effectively by tympanoplastic surgeries. Pure tone audiography did not differ significantly in the two cases pre- or postoperatively, indicating that the small mass transducer does not adversely affect middle-ear vibration. Postoperative hearing thresholds with VSBs were s

Bone conduction in otosclerosis--operated versus non-operated ears.

OBJECTIVE: To examine whether stapes surgery improves long-term bone conduction in patients with otosclerosis. SETTING: Tertiary university hospital. METHODS: Thirty patients (52 ears) who underwent stapedectomy between 1988 and 1994. Hearing tests were performed pre-operatively and 5 years postoperatively. RESULTS: The operated ears showed significant improvement in speech reception threshold and air conduction (250-4,000 Hz). Over the follow-up period, there were no significant differences in bone conduction thresholds between the operated and non-operated ears and no deterioration during

Further delineation of the hypotrichosis-deafness syndrome.

We recently delineated a novel disorder characterized by hypotrichosis, nail dystrophy and sensorineural deafness and caused by a missense mutation in GJB2 (connexin26). The patient, a girl, was two years old when we first saw her. We had the opportunity to re-examine her at four years of age and found that the phenotype had changed appreciably. The hypotrichosis was less pronounced, but the nail dystrophy had worsened. Intriguingly, the phenotype now included a mucositis and skin lesions identical to those found in erythrokeratodermia variabilis. There is now a considerable overlap with other

[Cogan síndrome. Audio-vestibular findings in a patient and literature review].

Cogan Sindrome is described as the association of non-syphilic interstitial keratitis with vestibulo-auditory symptoms in its typical form. We are reporting a case corresponding to a 32 years old male who consulted at first place for ophthalmic clinic and, two months later, affected of bilateral neurosensorial hearing loss with tinnitus and vestibular dysfunction. In spite of the corticoid treatment our patient has not improved. His hearing disorder was even worse after one year-follow up. A bibliographic review has been performed related to this pathology whose etiology is still unknown.

Diagnostic implications of stimulus polarity effects on the auditory brainstem response.

The purpose of this study was to determine whether clicks presented in rarefaction or condensation modes produce more accurate diagnostic information. Subjects were 20 consecutive patients who were seen at the Mayo Clinic for unilateral acoustic neuromas. The nontumor ear served as a control to minimize intersubject variability in the latencies. A standard audiologic evaluation was followed by an auditory brainstem response (ABR) test for which the stimuli were rarefaction and condensation clicks. Responses were analyzed for the presence of waves I, III, and V; absolute latencies of waves I, I

Immune-mediated hearing loss: basic mechanisms and options for therapy.

Immune responses are an important component of the defense of the inner ear against infection. However, immunity appears to be restricted in the labyrinth, as the inflammation associated with immune responses can damage the delicate cellular structures that mediate hearing and balance. When immunoregulation is compromised, either experimentally or through disease, immune-mediated inner ear damage and hearing loss can result. Because there are few resident leukocytes within the labyrinth, immunity and inflammation are mediated primarily by cells that enter the inner ear following the activation

Chronic silent otitis media.

Otitis media occurs along a continuum. For example, otitis media with effusion characterized by fluid pathology can lead to chronic otitis media plus chronic mastoiditis, characterized by the presence of intractable tissue pathology such as cholesteatoma, cholesterol granuloma or granulation tissue. The literature defines chronic otitis media as having a tympanic membrane perforation and otorrhea. Amongst many other sequelae, which can result from the continuum, an important common one is chronic silent otitis media. This overlooked entity which includes pathology beneath an intact tympanic me

Precocious puberty, endometriosis, skeletal anomalies and mild hearing loss: a new autosomal dominant syndrome?

Two sisters presented at 13 and 15 years-of-age respectively with a history of precocious puberty, short stature, endometriosis and mild mixed hearing loss. They had mild learning difficulties and a number of skeletal features in common. Some of these features were present in their father, suggesting a new autosomal dominant entity.

New variant of familial cerebellar ataxia with hypergonadotropic hypogonadism and sensorineural deafness.

Cerebellar ataxia and hypergonadotropic hypogonadism comprise a rare and presumably heterogeneous association. Inheritance in most cases appears to be autosomal recessive, and associated features include deafness, intellectual impairment, and neuropathy. Typically, onset of ataxia is in the first decade and hypogonadism results in primary amenorrhoea in females. We describe two sisters with a previously undescribed pattern of adult onset progressive cerebellar ataxia and secondary amenorrhoea due to hypergonadotropic hypogonadism. Sensorineural deafness with vestibular hypofunction and periphe

Otopathology in a case of type I Waardenburg's syndrome.

We report a case of type I Waardenburg's syndrome that provides insight into the etiopathogenesis of sensorineural hearing loss (SNHL) in this syndrome. The subject, a 76-year-old woman with type I Waardenburg's syndrome (dystopia canthorum, heterochromia irides, and white hair), had congenital low-frequency SNHL in her right ear only, which had remained relatively stable throughout her life. Blood leukocyte DNA studies revealed a PAX-3 mutation with a 1 base pair C-to-A substitution in exon 5 at base 602. Light microscopic studies of the right cochlea showed intact neurosensory structures in

Magnetic resonance imaging of the temporal bone in patients with Ménière's disease.

Ménière's disease (MD) is still controversial in several aspects. The vestibular aqueduct, the osseous channel that carries the endolymphatic duct and sac, has previously been studied by tomography and computed tomography, with shortening and narrowing of this structure observed. These findings are apparently correlated to the development of the endolymphatic hydrops present in MD and related to its episodic symptoms. In studying the endolymphatic duct, the key structure in the pathology of this disease, magnetic resonance imaging (MRI) studies of the temporal bone were performed in 12 patient

Neuro-otologic findings in the lightning-injured patient.

Although lightning injuries are common, neuro-otologic sequelae are infrequently reported. The most common otologic injury encountered in the lightning strike victim is tympanic membrane rupture; the most common vestibular disturbance documented is transient vertigo. A variety of other clinical findings have been described in this population of patients. They include sensorineural hearing loss, conductive deafness, tinnitus, basilar skull fracture, avulsion of the mastoid bone, burns to the external auditory canal, and peripheral facial nerve palsy. The initial treatment of the lightning strik

Revision surgery for otosclerosis is analysed in 26 patients. Audiometric results are better in patients with recurrent rather than persistent transmission hearing loss. Total platinectomy with interposition may be a solution for patients with chronic tubal dysfunction.

Pathology of prelingual profound deafness: magnitude of labyrinthitis fibro-ossificans.

Quantitative histologic studies were performed on 15 temporal bones from eight adult persons who were known to have prelingual bilateral profound hearing loss. The pathologic changes are characterized by severe degeneration of the structures of the cochlear duct, often with degeneration of the vestibular sense organs, causing a reparative host response that features osteoneogenesis and fibrous proliferation followed by retrograde neuronal degeneration. The pathology is consistent with meningogenic bacterial or viral labyrinthitis that occurred subclinically or went undiagnosed. Bone and fibrou
