New Treatment Paradigms in Primary Biliary Cholangitis.

Primary biliary cholangitis (PBC) is an archetypal autoimmune disease. Chronic lymphocytic cholangitis is associated with interface hepatitis, ductopenia, cholestasis, and progressive biliary fibrosis. People living with PBC are frequently symptomatic, experiencing a quality-of-life burden dominated by fatigue, itch, abdominal pain, and sicca complex. Although the female predominance, specific serum autoantibodies, immune-mediated cellular injury, as well as genetic (HLA and non-HLA) risk factors, identify PBC as autoimmune, to date treatment has focused on cholestatic consequences. Biliary ep

Influence of socioeconomic factors on liver transplant survival outcomes in patients with autoimmune liver disease in the United States.

INTRODUCTION AND OBJECTIVES: Autoimmune liver diseases (AILDs): autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC) have different survival outcomes after liver transplant (LT). Outcomes are influenced by factors including disease burden, medical comorbidities, and socioeconomic variables. MATERIALS AND METHODS: Using the United Network for Organ Sharing database (UNOS), we identified 13,702 patients with AILDs listed for LT between 2002 and 2021. Outcomes of interest were waitlist removal, post-LT patient survival, and post- LT graft surviv

Camrelizumab-Related Lethal Arrhythmias and Myasthenic Crisis in a Patient with Metastatic Thymoma.

Immune checkpoint inhibitors (ICIs) have emerged in recent years as promising treatment options for several malignant tumors. However, ICI therapy has also been associated with various immune-related adverse events (irAEs), especially for patients with preexisting autoimmune status, which sometimes can be life-threatening. A 68-year-old woman diagnosed with metastatic thymoma was treated with camrelizumab, a new ICI, as her antitumor protocol. Eleven days after the first dose of camrelizumab, the patient was admitted to our hospital with symptoms of dyspnea, fatigue, and poor appetite. Workups

Rate of non-response to ursodeoxycholic acid in a large real-world cohort of primary biliary cholangitis patients in Italy.

Background and aim: Response to ursodeoxycholic acid (UDCA) is crucial for the prediction of primary biliary cholangitis (PBC) prognosis, and different response criteria were validated and proposed by reference centers for PBC. To date, rates of non-response to UDCA from real-world series are lacking. Methods: Hepatology/Gastroenterology centers belonging to 'Club Epatologi Ospedalieri' (CLEO) and 'Associazione Italiana Gastroenterologi Ospedalieri' (AIGO) were invited to participate in the study, and asked to extract all patients followed for PBC, without any selection or exclusion, and fill i

Redefined clinical features and diagnostic criteria in autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy.

Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED) is a rare primary immunodeficiency disorder typically caused by homozygous AIRE mutations. It classically presents with chronic mucocutaneous candidiasis and autoimmunity that primarily targets endocrine tissues; hypoparathyroidism and adrenal insufficiency are most common. Developing any two of these classic triad manifestations establishes the diagnosis. Although widely recognized in Europe, where nonendocrine autoimmune manifestations are uncommon, APECED is less defined in patients from the Western Hemisphere. We enrol

Chronic graft-versus-host-disease in CD34(+)-humanized NSG mice is associated with human susceptibility HLA haplotypes for autoimmune disease.

Chronic graft-versus-host disease (cGVHD) is a significant hurdle to long-term hematopoietic stem-cell transplantation success. Insights into the pathogenesis and mechanistical investigations of novel therapeutic strategies are limited as appropriate animal models are missing. The immunodeficient NSG mouse - when humanized with human bone marrow, fetal liver and thymus (BLT NSG) - is prone for cGVHD, yet mainly affects the skin. In contrast, the NSG mouse humanized exclusively with CD34(+)-selected, CD3(+)-depleted stem cells (CD34(+)NSG) has neither been described for acute nor chronic GVHD s

Associations between selected immune-mediated diseases and tuberculosis: record-linkage studies.

BACKGROUND: Previous studies have suggested that there may be an association between some immune-mediated diseases and risk of tuberculosis (TB). METHODS: We analyzed a database of linked statistical records of hospital admissions and death certificates for the whole of England (1999 to 2011), and a similar database (the Oxford Record Linkage Study (ORLS)) for a region of southern England in an earlier period. Rate ratios for TB were determined, comparing immune-mediated disease cohorts with comparison cohorts. RESULTS: In the all-England dataset, there were significantly elevated risks of T

[Primary sclerosing cholangitis: diagnosis, prognosis and treatment].

Primary sclerosing cholangitis is a chronic cholestatic disease characterized by inflammation with fibrosis and obliteration of the intra- and extrahepatic bile ducts. This disease is usually associated with ulcerative colitis. The process of chronic cholestasis eventually leads to biliary cirrhosis. The prevalence of primary sclerosing cholangitis is low in southern Europe but is especially high in Scandinavian countries. The etiopathogenesis is unknown but immune disorders, potential toxic agents or intestinal infections, ischemic injury to the bile ducts, and possibly alterations in hepatob

Immune-related adverse events from checkpoint inhibitors: An evidence-based management review.

Immune checkpoint inhibitors (ICIs) improve cancer outcomes but can cause immune‑related adverse events (irAEs) in any organ. Corticosteroids remain first‑line, achieving remission in ∼70-85%: grade 2 toxicities typically use oral prednisone 0.5-1 mg/kg/day, while grade 3-4 require IV methylprednisolone 1-2 mg/kg/day (or pulses for life‑threatening disease). However, 20-40% of severe irAEs are steroid‑refractory and need rapid, organ‑directed escalation-mycophenolate for hepatitis/pneumonitis; infliximab or vedolizumab for colitis; IVIG and plasma exchange for neurologic syndromes; and investi

Endobronchial tuberculosis revealing extensive multiorgan dissemination in a severely malnourished immunocompetent adult.

A woman in her mid-30s presented with a 3-month history of dry cough, profound weight loss and anorexia. Evaluation revealed pulmonary tuberculosis microbiologically confirmed by bronchoalveolar lavage cartridge-based nucleic acid amplification testing (CBNAAT) together with histopathological evidence from endobronchial biopsy, with extensive dissemination involving the pleura, pericardium, peritoneum, omentum, liver and colon. Bronchoscopy demonstrated multiple nodular endobronchial lesions while bronchoalveolar lavage CBNAAT detected Mycobacterium tuberculosis without rifampicin resistance.

Low-background Near-infrared Fluorescent Probe for Real-time Monitoring of β-Glucuronidase Activity in Inflammation and Therapy.

β-Glucuronidase (GUS) is an acidic hydrolase enzyme overexpressed in various inflammatory diseases, making it a promising biomarker for inflammation. However, current tools for real-time, in situ imaging of GUS activity are hindered by background interference, which reduces their effectiveness in dynamic biological environments. To address this challenge, we developed Ox-GUS, a GUS-specific fluorescent probe with a unique molecular design featuring a disrupted conjugated structure. This design provided Ox-GUS with near-zero background optical properties, a significantly enhanced signal-to-nois

Nimbolide Targeting SIRT1 Protects Against Acetaminophen-Induced Acute Liver Injury by Regulating Oxidative Stress and Endoplasmic Reticulum Stress.

Acetaminophen (APAP) is a major cause of acute liver injury (ALI), and N-acetylcysteine is the only approved detoxification drug. Nimbolide (Nim), which is isolated from the neem tree (Azadirachta indica), possesses protective properties against multiple diseases, including pancreatitis, autoimmune hepatitis, arthritis, and diabetic cardiomyopathy. Here, we investigated the protective effect of nimbolide on APAP-induced ALI. Male C57BL/6J mice were used to establish an ALI model via APAP administration (500 mg/kg, i.p.). All the mice received nimbolide (20 mg/kg, i.p.) or a vehicle 2 h before

The evolving scenario of HCV-related mixed cryoglobulinemia and B-cell lymphoma in the era of direct-acting antivirals.

INTRODUCTION: Hepatitis C virus (HCV) infection represents a significant global health burden, particularly due to its extrahepatic immune-mediated manifestations, such as mixed cryoglobulinemia, associated vasculitis (CryoVas), and non-Hodgkin B-cell lymphoma (B-NHL), which pose significant challenges. The advent of direct-acting antiviral (DAA) has changed the therapeutic landscape for HCV-related complications. AREAS COVERED: This review explores the evolving epidemiology and management of HCV extrahepatic manifestation and lymphoproliferative disorders in the era of DAAs. It examines the

The role of immune regulation in HBV infection and hepatocellular carcinogenesis.

Hepatitis B virus (HBV) infection is a well-documented independent risk factor for developing hepatocellular carcinoma (HCC). Consequently, extensive research has focused on elucidating the mechanisms by which HBV induces hepatocarcinogenesis. The majority of studies are dedicated to understanding how HBV DNA integration into the host genome, viral RNA expression, and the resulting protein transcripts affect cellular processes and promote the malignant transformation of hepatocytes. However, considering that most acute HBV infections are curable, immune suppression potentially contributes to t
