Patient experience with cancer care in low- and middle-income Asian countries: a cross-sectional study of patients with advanced cancer.

INTRODUCTION: Despite the disproportionate burden of cancer morbidity and mortality in low- and middle-income countries (LMICs), little is known about patients' care experiences in these settings. This study assesses which aspects of physician communication and care coordination most influence patients' overall experience with care, and factors associated with patient experience ratings, to inform quality improvement and improve cancer care experiences in LMICs. METHODS: In a cross-sectional study of 1933 patients with advanced cancer recruited at 10 major public hospitals in seven LMICs in A

Readmission and Resource Utilization in Patients From Socioeconomically Distressed Communities Following Lumbar Fusion.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To determine whether: (1) patients from communities of socioeconomic distress have higher readmission rates or postoperative healthcare resource utilization and (2) there are differences in patient-reported outcome measures (PROMs) based on socioeconomic distress. SUMMARY OF BACKGROUND DATA: Socioeconomic disparities affect health outcomes, but little evidence exists demonstrating the impact of socioeconomic distress on postoperative resource utilization or PROMs. METHODS: A retrospective review was performed on patients who underwent lum

Differentiated service delivery: a qualitative study of people living with HIV and accessing care in a tertiary facility in Ghana.

BACKGROUND: In 2014, the Joint United Nations Program on HIV/AIDS (UNAIDS) set out a treatment target with the objective to help end the AIDS epidemic by 2030. This was supported by the UNAIDS '90-90-90' target that by 2020, 90% of all people living with HIV (PLHIV) will know their HIV status; 90% of all those diagnosed with HIV will be on sustained antiretroviral therapy; and 90% of all people receiving antiretroviral therapy will have viral suppression. The concept of offering differentiated care services using community-based models is evidence-based and is suggested as a means to bring thi

Dysphonia in Hospitalized Patients.

Dysphonia, or impaired voice quality, is common in hospitalized patients and can impair communication, signal underlying disease, and worsen outcomes. Ultimately, this negatively impacts quality of life, increases resource utilization, and often coexists with dysphagia. Causes include surgical injury, intubation, infection, neurologic disease, and malignancy. Diagnosis relies on history and physical examination, often inclusive of laryngoscopy performed by otolaryngologists. Treatment is multidisciplinary, involving speech-language therapy, medical services, and surgery. Early ear, nose, and t

Is Chlamydia to Blame for Koala Reproductive Cysts?

A significant threat to koala populations is infection from Chlamydia, which results in disease and death. Wild koalas with Chlamydia infections are admitted to wildlife hospitals and treated with antibiotics; however, up to 50% of koalas that present to wildlife hospitals do not survive. A major contributor to high mortality is the development of reproductive cysts, resulting in female infertility and euthanasia. However, the diagnosis of reproductive disease is limited to ultrasound with no further investigations. This communication highlights reports of histological and microbiological find

Isolation of methicillin-resistant Staphylococcus aureus from a postoperative wound infection in a horse.

Methicillin-resistant Staphylococcus aureus (MRSA) was isolated from a postoperative wound infection in a horse. Methicillin-resistant S aureus infections in animals have been reported. In human beings, MRSA is an important cause of hospital-acquired (nosocomial) infections. Infections caused by MRSA respond poorly to beta-lactam treatment, and resistance of MRSA to multiple antimicrobials, including aminoglycosides, macrolides, clindamycin, and tetracyclines, is common. Identification of MRSA by routine susceptibility testing may be difficult; therefore, techniques for MRSA detection should b

A two-year clinical and economic follow-up of patients on clozapine.

The long-term efficacy of clozapine therapy and its effect on health care costs were examined over a two-year period. Patients on clozapine showed marked clinical improvement as measured by the Brief Psychiatric Rating Scale. They also had significantly lower rates of rehospitalization and hospitalization costs than a comparison group of schizophrenic patients who received standard neuroleptic treatment and who were considerably less psychotic at hospital admission. By the second year of the study, savings on mental health care costs averaged $20,000 for each patient on clozapine therapy. The

Impact of rapid molecular testing on diagnosis, treatment and management of community-acquired pneumonia in Norway: a pragmatic randomised controlled trial (CAPNOR).

BACKGROUND: Community-acquired pneumonia (CAP) causes a large burden of disease. Due to difficulties in obtaining representative respiratory samples and insensitive standard microbiological methods, the microbiological aetiology of CAP is difficult to ascertain. With a few exceptions, standard-of-care diagnostics are too slow to influence initial decisions on antimicrobial therapy. The management of CAP is therefore largely based on empirical treatment guidelines. Empiric antimicrobial therapy is often initiated in the primary care setting, affecting diagnostic tests based on conventional bact

Incidence and antimicrobial resistance trends in bloodstream infections caused by ESKAPE and Escherichia coli at a large teaching hospital in Rome, a 9-year analysis (2007-2015).

The proportion of antimicrobial resistance (AMR) among the ESKAPE and Escherichia coli (ESKAPEEc) pathogens causing bloodstream infection (BSI) increased worldwide. We described longitudinal trends in ESKAPEEc BSI and AMR over 9 years (2007-2015) at a large teaching hospital in Italy. Of 9720 unique BSI episodes, 6002 (61.7%) were caused by ESKAPEEc pathogens. The majority of these episodes (4374; 72.9%) were hospital-onset infections. The most frequent pathogen was E. coli (32.8%), followed by Staphylococcus aureus (20.6%), Klebsiella pneumoniae (16.1%), and Pseudomonas aeruginosa (11.6%). Th

Obstetric care navigation: a new approach to promote respectful maternity care and overcome barriers to safe motherhood.

BACKGROUND: Disrespectful and abusive maternity care is a common and pervasive problem that disproportionately impacts marginalized women. By making mothers less likely to agree to facility-based delivery, it contributes to the unacceptably high rates of maternal mortality in low- and middle-income countries. Few programmatic approaches have been proposed to address disrespectful and abusive maternity care. OBSTETRIC CARE NAVIGATION: Care navigation was pioneered by the field of oncology to improve health outcomes of vulnerable populations and promote patient autonomy by providing linkages ac

A structured communication model for advance care planning in Chinese oncology nursing: a mixed-methods participatory action research study.

BACKGROUND: Despite the global recognition of advance care planning as a critical component of patient-centred end-of-life care, its implementation remains challenged by skill-based deficiencies (e.g., inadequate training), cultural and communication barriers, and system-level structural impediments within healthcare settings. This study aimed to develop and implement a structured advance care planning communication model to improve nurses' communication practices and facilitate patient engagement in end-of-life care discussions. METHODS: A participatory action research design with embedded m
