Long-term seizure outcomes and the likelihood of antiseizure medication withdrawal in patients with juvenile absence epilepsy: A 10-year follow-up study.

BACKGROUND AND OBJECTIVE: Juvenile absence epilepsy is a common idiopathic generalized epilepsy syndrome that often requires long-term antiseizure medications (ASMs). However, it remains unclear whether and when such long-term medication is necessary. In this study, we aimed to explore the long-term seizure outcomes and the feasibility of ASMs withdrawal in patients with JAE. Additionally, we aimed to investigate the evolution of EEG patterns over time. METHODS: We conducted a retrospective study on patients diagnosed with JAE according to the 2022 ILAE at Xijing Hospital between March 2014 a

Monitoring Changes in Oxygen Muscle during Exercise with High-Flow Nasal Cannula Using Wearable NIRS Biosensors.

Exercise increases the cost of breathing (COB) due to increased lung ventilation (V˙E), inducing respiratory muscles deoxygenation (∇SmO2), while the increase in workload implies ∇SmO2 in locomotor muscles. This phenomenon has been proposed as a leading cause of exercise intolerance, especially in clinical contexts. The use of high-flow nasal cannula (HFNC) during exercise routines in rehabilitation programs has gained significant interest because it is proposed as a therapeutic intervention for reducing symptoms associated with exercise intolerance, such as fatigue and dyspnea, assuming that

Cardiorespiratory hysteresis during incremental high-altitude ascent-descent quantifies the magnitude of ventilatory acclimatization.

NEW FINDINGS: What is the central question of this study? We assessed the utility of a new metric for quantifying ventilatory acclimatization to high altitude, derived from differential ascent and descent steady-state cardiorespiratory variables (i.e. hysteresis). Furthermore, we aimed to investigate whether the magnitude of cardiorespiratory hysteresis was associated with the development of acute mountain sickness. What is the main finding and its importance? Hysteresis in steady-state cardiorespiratory variables quantifies ventilatory acclimatization to high altitude. The magnitude of cardio

Ammonia as a stimulant to ventilation in rainbow trout Oncorhynchus mykiss.

Ammonia is the third most important respiratory gas in ammoniotelic fish after oxygen and carbon dioxide. We here investigated the effects of elevated plasma ammonia on ventilation in freshwater rainbow trout. Intact trout fitted with indwelling dorsal aortic catheters were given injections (over 5 min) of Cortland saline, isotonic high ammonia solutions (NH(4)HCO(3), (NH(4))(2)SO(4), NH(4)OH at pH 8.0, and NH(4)OH at pH 9.0), and other solutions as controls for acid-base effects, while ventilatory rate (VR) and buccal pressure amplitude (DeltaP(buccal)) were recorded. All high ammonia solutio

Indications for routine repeat head computed tomography (CT) stratified by severity of traumatic brain injury.

BACKGROUND: Controversy exists as to the role of a routine repeat head computed tomography (CT) for patients with traumatic brain injury and an initially abnormal head CT. The specific aim of this study is to identify patients with head injuries who would benefit from a routine repeat head CT. METHODS: This was a 2-year (2003 and 2004) prospective study of all patients with blunt trauma admitted to an urban, Level I trauma center that presented with an abnormal head CT. Results of initial head CT and indications for repeat head CT (routine vs. neurologic change) were recorded. Interventions w

Red blood cell pH, the Bohr effect, and other oxygenation-linked phenomena in blood O2 and CO2 transport.

The discovery of the S-shaped O2 equilibrium curve and the Bohr effect in 1904 stimulated a fertile and continued research into respiratory functions of blood and allosteric mechanisms in haemoglobin (Hb). The Bohr effect (influence of pH/CO2 on Hb O2 affinity) and the reciprocal Haldane effect (influence of HbO2 saturation on H+/CO2 binding) originate in the Hb oxy-deoxy conformational change and allosteric interactions between O2 and H+/CO2 binding sites. In steady state, H+ is passively distributed across the vertebrate red blood cell (RBC) membrane, and intracellular pH (pHi) changes are r

Ethosuximide, sodium valproate or lamotrigine for absence seizures in children and adolescents.

BACKGROUND: Absence seizures are brief epileptic seizures which present in childhood and adolescence. They are characterised by sudden loss of awareness and an electroencephalogram (EEG) typically shows generalised spike wave discharges at three cycles per second. Ethosuximide, valproate and lamotrigine are currently used to treat absence seizures. This review aims to determine the best choice of anticonvulsant for a child with typical absence seizures. OBJECTIVES: To review the evidence for the effects of ethosuximide, valproate and lamotrigine as treatments for children and adolescents with

Muscle fiber type distribution and nonlinear .VO(2)-power output relationship in cycling.

PURPOSE: We examined whether reported deviations from linearity of the oxygen uptake (.VO(2))-to-power output (W) relationship during intense cycling exercise correlated with the percentage Type II fibers in the exercising muscle. METHODS: Twelve trained young men with known fiber type distribution in the vastus lateralis muscle performed step-increment exercise (40 W.3 min(-1)) to exhaustion. RESULTS: .VO(2) increased linearly with W up to about 50% .VO(2max) with a regression equation of .VO(2) (mL.min-1) = 661 + 9.73 W and a correlation coefficient (r) of 1.000. Subsequent .VO(2) values w

Neurophysiologic monitoring in the neuroscience intensive care unit.

Neurologically critically ill patients, more often than others, are unable to communicate and, for a crucial period of time, have the vital functions of their brains hidden in the "black box" of the cranial vault behind a curtain of ambiguity and immobility. Customarily--and naively--we have relied upon beside clinical observations to pierce these barriers. Recent insights lead us to conclude that these "neurochecks" no longer suffice. This article has surveyed four major monitoring systems relied upon by neurointensivists to evaluate the pathophysiology of their patients. Of these, ICPM has t

[Capnography for bronchoscopy with rigid technique using high frequency jet ventilation (HFJV)].

OBJECTIVES AND METHODS: Rapid bronchoscopy in general anaesthesia still has its precise indications, where the high frequency jet ventilation technique offers several advantages. The monitoring of ventilation, however, has been rather unsatisfactory up to date. We therefore studied capnography in 60 bronchoscopies during HFJV (rate: 100/min; I:E = 0.33; driving pressure: 0.08-0.14 MPa) using a rigid bronchoscope with a distally located sampling port. Continuous capnograms were recorded. End-tidal partial pressures of carbon dioxide (petCO2), however, were obtained from 2-3 single breaths by in

Applied physiology of diving.

Recreational diving is a popular sport, although human ability to stay in and under water is severely limited physiologically. An understanding of these limitations enhances safety and enjoyment of sports diving. Breath-hold diving involves head-out water immersion, apnoea and submersion, exercise, cold stress, and pressure exposure. Each of these components, by itself, elicits prominent and specific physiological effects. Combination of these factors produces a unique and interesting physiological response generally known as diving reflex. Humans display weak diving responses, but exhibit no

[General anesthesia in infants].

It is generally accepted, that the progress in surgical treatment of congenital malformation is closely related to the progress in highly-specialised methods of newborn anaesthesia. The safe methods of anaesthesia have to be adapted to anatomical and physiological peculiarities of the earliest days of life as well as to different reaction to drugs and anaesthetics. The preoperative preparation in newborns used to be often very short, because most of the problems of the neonatal period are emergency surgical interventions and there is no time for treatment even of the serious disturbances of ba

[Respiratory behavior].

The notion of respiratory behaviour is grounded, among other approaches, on studies of neuronal mechanisms of voluntary breathing, clinical data, conditioning experiments and respiratory sensations. The interactions between cortical centres of voluntary breathing and respiratory neurones in the brain stem are poorly understood: voluntary control operates through the direct action of corticomotor centres on respiratory motoneurones; however these cortical structures may directly act on bulbopontine centres, and therefore indirectly on respiratory motoneurones. Recordings in animals of brain ste
