A North Carolina newborn screening pilot for mucopolysaccharidosis II: Evaluating endogenous nonreducing end glycosaminoglycan analysis and IDS sequencing as higher-tier testing options.

PURPOSE: Mucopolysaccharidosis II (MPS II, OMIM 309900) is a lysosomal disorder recommended for newborn screening (NBS) in the United States. This study evaluated outcomes of high-throughput NBS for MPS II and use of 2 reflex testing methods to improve sensitivity and specificity. METHODS: We implemented a first-tier liquid chromatography tandem mass spectrometry laboratory-developed test measuring iduronate-2-sulfatase (I2S) enzyme activity in dried blood spots. Newborns with activity ≤10% of the daily median underwent reflex testing for endogenous nonreducing end (NRE) glycosaminoglycan (GA

Prevalence and trends of major congenital anomalies in Brazil: A study from 2011 to 2020.

BACKGROUND: Congenital anomalies contribute significantly to morbidity and mortality among newborns and infants. In Brazil, the estimated prevalence of malformations in newborns is < 1%, which is comparatively lower than that recorded in other regions worldwide. This study aimed to analyze the prevalence of congenital anomalies in Brazil over a 10-year period and to identify potential associations of this prevalence with socioeconomic, gestational, and regional factors by performing an analysis using data sourced from the Live Birth Information System (Sistema de Informações sobre os Nascidos

Long-Term Follow-Up Cares and Check Initiative: A Program to Advance Long-Term Follow-Up in Newborns Identified with a Disease through Newborn Screening.

In the United States and around the world, newborns are screened on a population basis for conditions benefiting from pre-symptomatic diagnosis and treatment. The number of screened conditions continues to expand as novel technologies for screening, diagnosing, treating, and managing disease are discovered. While screening all newborns facilitates early diagnosis and treatment, most screened conditions are treatable but not curable. Patients identified by newborn screening often require lifelong medical management and community support to achieve the best possible outcome. To advance the long-

Predischarge transcutaneous bilirubin screening reduces readmission rate for hyperbilirubinaemia in diverse South African newborns: A randomised controlled trial.

BACKGROUND: In South Africa (SA), healthy term newborns are usually discharged ˂72 hours after delivery. Discharged babies remain at risk for severe hyperbilirubinaemia if it is not identified early. Hyperbilirubinaemia is an important cause of readmission, and also leads to neonatal mortality and morbidity. Use of transcutaneous bilirubin (TcB) screening before hospital discharge has been controversial. OBJECTIVES: To test the clinical benefits of TcB screening of healthy newborns before discharge for the outcomes of readmission for jaundice and severe hyperbilirubinaemia in a randomised con

Population-based newborn screening for severe combined immunodeficiency: steps toward implementation.

Severe combined immune deficiency (SCID) has been identified as a disorder of high priority for population-based newborn screening. Most affected infants are not brought to medical attention until they develop serious infectious complications, and SCID is fatal if untreated. Effective treatment with allogeneic hematopoietic stem cell transplantation is widely established. The best outcome for SCID, as with many other conditions for which newborn screening is now done, is achieved if hematopoietic stem cell transplantation is performed in the first months of life, ideally before clinical presen

Can anesthesiologic strategies for caesarean section influence newborn jaundice? A retrospective and prospective study.

RATIONALE AND OBJECTIVES: Neonatal jaundice is a frequent problem in neonatology and can be influenced by many factors. Our study arose from the clinical observation that among all newborns delivered by caesarean section in our center, some had a more intense physiological jaundice. We began by reviewing clinical anesthesiological case-sheets to ascertain whether this difference was linked to the use of different anesthesiologic strategies. We then performed a prospective study on healthy preterm and term newborns to verify this hypothesis. STUDY DESIGN: We retrospectively considered all heal

Serum 3,5,3'-triiodothyronine, thyroxine, and thyrotropin in hypothyroid infants with congenital goiter and the response to iodine.

Iodine deficiency in adults caused preferred synthesis of T3; this observation has not been reported in iodine-deficient hypothyroid newborns. Serum total T4, total T3, and TSH have been determined in nine full term newborns with congenital hypothyroid goiter before and after cutaneous application of iodine. The mothers of these infants had untreated euthyroid goiter and lived during pregnancy in the area of Göttingen, West Germany, known as an iodine-deficient region. Mean total T4 in the newborns was 6.3 +/- 1.6 (mean +/- SD) micrograms/dl compared to 16.6 +/- 3.4 micrograms/dl in normal new

Characteristics and outcomes of respiratory distress among term infants born in a regional setting.

INTRODUCTION: Respiratory distress is the leading cause of admission to neonatal units and is a common indication for medical retrieval. Whilst approximately 25% of births in NSW occur in regional centres, there is a paucity of neonatal research in these settings. OBJECTIVE: To describe the characteristics and outcomes of term neonates admitted with respiratory distress to two regional special care nurseries (SCNs) and identify variables associated with the need for medical retrieval. DESIGN: We describe a cohort of 629 term infants admitted to the SCN in two regional hospitals, 2015-2019. W

Study protocol for multicentre randomized controlled trial of HeLP (Heat Loss Prevention) in the delivery room.

INTRODUCTION: Immediate postnatal hypothermia is an independent risk factor for death in premature newborns. Three randomized controlled trials (RCTs) and five historical controlled trials show statistically significant differences in admission temperature between infants wrapped in occlusive skin wrap and unwrapped infants. This paper presents a study protocol for The Vermont Oxford Network (VON) Heat Loss Prevention (HeLP) Trial, a multicentre RCT of two interventions (standard of care vs. occlusive wrap) that investigates the effect of polyethylene occlusive wrap applied immediately after b
