The impact of an education program on hand hygiene compliance and nosocomial infection incidence in an urban neonatal intensive care unit: an intervention study with before and after comparison.

BACKGROUND: Nosocomial bloodstream infections are a major cause of morbidity and mortality in neonatal intensive care units. Appropriate hand hygiene is singled out as the most important measure in preventing these infections. However, hand hygiene compliance among healthcare professionals remains low despite the well-known effect on infection reduction. OBJECTIVES: We studied the effectiveness of a hand hygiene education program on the incidence of nosocomial bloodstream infections. DESIGN: Observational study with two pretests and two posttest measurements and interrupted time series analy

One-year outcome of auditory-tactile-visual-vestibular intervention in the neonatal intensive care unit: effects of severe prematurity and central nervous system injury.

Thirty-seven infants with severe central nervous system injury or extreme prematurity were randomly assigned to a multisensory (auditory-tactile-visual-vestibular) intervention or control group. Intervention began in the hospital at 33 weeks' postconceptional age and continued twice daily in the home until 2 months' corrected age. Mother-infant interactions during feedings were videotaped, and the Bayley Scales of Infant Development were administered. Control mothers stimulated their infants more during feeding, but these significant differences dissipated by 4 months. The presence of perivent

Birth weight-specific mortality for extremely low birth weight infants vanishes by four days of life: epidemiology and ethics in the neonatal intensive care unit.

BACKGROUND: The persistent differences between those who question the appropriateness of aggressive resuscitative measures for many extremely low birth weight (ELBW) infants (birth weight < 1001 g) and those who generally initiate such treatment has been a source of ongoing tension for physicians, parents, judges, and policymakers. We believe that much of this tension may be a result of the way the issue is framed. We began this study with the intuition that although many ELBW infants die, most succumb quickly. Were this true, discussions that considered only survival rates might miss the poin

Consensus document on the management of patients with accidental hypothermia from the Spanish Society of Anesthesiology, Resuscitation and Pain Management (SEDAR), the Spanish Society of Intensive Care Medicine, Critical Care and Coronary Units (SEMICYUC), and the Spanish Society of Emergency Medicine (SEMES).

Accidental hypothermia is an underdiagnosed condition with a significant impact on public health. With the aim of providing the basis for a comprehensive care framework, 12 panelists from 3 Spanish scientific societies (SEDAR, SEMES and SEMICYUC) developed this multidisciplinary consensus document. Through a systematic search and the final synthesis of 281 articles including 1,704,632 patients, 37 recommendations were developed through a 2-round Delphi process, distributed across 8 application domains based on the organization of a rescue chain: prehospital prevention, initial evaluation, mana

The effect of three-way stopcock and closed vascular access systems on central line-associated bloodstream infection in the pediatric cardiac surgery intensive care unit.

OBJECTIVE: Central line-associated bloodstream infection (CLABSI) is a major cause of morbidity in pediatric cardiac intensive care units. Closed vascular access systems are modern devices used to prevent both needle-stick injuries and CLABSI. This study aimed to compare CLABSI rates between three-way stopcock (TWS) systems and closed vascular access systems (CVAS) in a pediatric cardiovascular surgery intensive care unit (PCSICU). METHODS: This study included patients with central venous catheters (CVC) between September 1, 2024, and October 31, 2025. CVC bundle practices were used in this s

Lung ultrasound-driven improvements in pneumonia detection, antibiotic stewardship, and ventilation outcomes: A three-era study in a pediatric cardiac surgical intensive care unit.

BACKGROUNDS: Postoperative pneumonia significantly prolongs mechanical ventilation (MV) and intensive care unit (ICU) stay after pediatric cardiac surgery. Lung ultrasound (LUS) offers superior diagnostic accuracy compared with chest radiography, but its longitudinal impact on clinical outcomes remains unclear. We evaluated the effect of structured LUS implementation on pneumonia detection, antibiotic stewardship, and ventilatory outcomes in a pediatric cardiac surgical ICU (PCSICU). METHODS: This retrospective three-era study included all bronchoalveolar lavage (BAL) episodes performed for s

"From the moment I started standing again, I was worried about falls": fear of falling in intensive care unit survivors over 12 months.

RATIONALE: Post-intensive care syndrome is a significant challenge for survivors of critical illness. However, little is understood about fear of falls and the concern for falls. OBJECTIVE: This study sought to quantify the prevalence of fear of falls within the first year after hospital discharge and identify factors associated with high fear of falls. METHODS: Using a mixed-methods approach, fear of falls was assessed using the Falls Efficacy Scale International short-form questionnaire with participants dichotomized into low/moderate (7-12) and high (13-28) fear of falls. Persistence was

Use of a Mixed Agent Prophylactic Laxative Regimen in the Medical Intensive Care Unit Leads to Reduced Length of Stay: A Single-Center Process Improvement Initiative.

Impaired GI tract function is a common and often underappreciated symptom in the Medical Intensive Care Unit (MICU). The most visible manifestation of this dysfunction is reduced bowel movements. The occurrence of impaired GI function is associated, both independently and in association with delayed enteral feeding, with increased length of stay, morbidity and mortality. We report on a process implementation project conducted at a single academic center designed to: (I) increase visibility of bowel movement charting and (II) transition from a reactive to a proactive laxative administration reg

Acceptability and Feasibility of an Educational Intervention to Improve Researcher-Participant Interactions in a Neonatal Intensive Care Unit Clinical Trial: Research Team Feedback on the BRIEF Intervention.

OBJECTIVE: Interactions with families are essential to successful recruitment conversations that promote informed decision-making about clinical research enrollment. However, there is little evidence about how to implement communication-oriented recruitment training among pediatric clinical research teams. Our objective was to evaluate the feasibility and acceptability of Better Research Interactions for Every Family (BRIEF), a multipart educational intervention to improve relationship-based conversations about clinical trial enrollment with families in the neonatal setting. Outcome of Children With Systemic Rheumatologic and Autoinflammatory Diseases Admitted to a Pediatric Intensive Care Unit: A Retrospective Cohort Study With Development of a Prognostic Risk-stratification Tool.

OBJECTIVE: Children with systemic rheumatologic diseases (SRD) and autoinflammatory diseases can experience life-threatening deterioration requiring intensive care. We conducted a retrospective cohort study with 2 complementary components: (1) a descriptive analysis of clinical presentations and outcomes; and (2) an exploratory derivation of a bedside risk-stratification tool, intended as a candidate severity overlay on established pediatric intensive care unit (PICU) scoring systems rather than as a fully validated general-purpose prognostic model. DESIGN: Retrospective observational cohort

Time to Initiation of Antibiotic Therapy in Critically Ill Patients with Suspected Sepsis and Its Effect on Intensive Care Unit Mortality: Analysis Using Bayesian Approach.

BACKGROUND AND AIMS: Sepsis is a leading cause of intensive care unit (ICU) admission and mortality. Although early antibiotic therapy in the emergency department improves outcomes, the clinical impact of delays in ICU-based antibiotic escalation is not well established. This study evaluated the association between time to ICU-based initiation of newly prescribed antibiotics and ICU mortality using a Bayesian analytical framework. PATIENTS AND METHODS: A prospective observational study (2019-2021) was conducted in a tertiary care hospital, including adult patients admitted to the ICU. New ant

Prognostic Value of the Advanced Lung Cancer Inflammation Index for Predicting in-Hospital Mortality in Intensive Care Unit Patients with Septic Shock: A Retrospective Cohort Study.

BACKGROUND: The Advanced Lung Cancer Inflammation Index (ALI), initially developed for advanced non-small cell lung cancer, has shown prognostic value across cardiovascular and oncologic conditions. Although ALI was originally developed for lung cancer prognosis, this study evaluates its applicability in general septic shock patients without active malignancy. However, its role in predicting outcomes in septic shock remains unclear. Conventional scoring systems like APACHE II and SOFA are limited by complexity and insufficient assessment of inflammatory and nutritional status. METHODS: We con
