Platelet-Rich Plasma for the Treatment of Low Back Pain: a Comprehensive Review.

PURPOSE OF REVIEW: Back pain is a growing problem worldwide, incurring enormous economic costs and disability. Current treatment modalities often provide adequate relief but fail to address underlying conditions. Regenerative cellular modalities aim to restore anatomical function in degenerative conditions which may cause low back pain. Platelet-rich plasma (PRP) consists of an increased concentration of autologous platelets suspended in a small amount of plasma. PRP can be administered via injection or topically and is prepared using various techniques. RECENT FINDINGS: While a unifying mech

The biomechanical effect of single-level laminectomy and posterior instrumentation on spinal stability in degenerative lumbar scoliosis: a human cadaveric study.

OBJECTIVEDegenerative lumbar scoliosis, or de novo degenerative lumbar scoliosis, can result in spinal canal stenosis, which is often accompanied by disabling symptoms. When surgically treated, a single-level laminectomy is performed and short-segment posterior instrumentation is placed to restore stability. However, the effects of laminectomy on spinal stability and the necessity of placing posterior instrumentation are unknown. Therefore, the aim of this study was to assess the stability of lumbar spines with degenerative scoliosis, characterized by the range of motion (ROM) and neutral zone

A Microstructural Investigation of Disc Disruption Induced by Low Frequency Cyclic Loading.

STUDY DESIGN: Microstructural investigation of low frequency cyclic loading and flexing of the lumbar disc. OBJECTIVE: To explore micro-level structural damage in motion segments subjected to low frequency repetitive loading and flexing at sub-acute loads. SUMMARY OF BACKGROUND DATA: Cumulative exposure to mechanical load has been implicated in low back pain and injury. The mechanical pathways by which cyclic loading physically affects spine tissues remain unclear, in part due to the absence of high quality microstructural evidence. METHODS: The study utilized seven intact ovine lumbar spin

Comparison of pure lateral and oblique lateral inter-body fusion for treatment of lumbar degenerative disk disease: a multicentric cohort study.

PURPOSE: The most effective interbody fusion technique for degenerative disk disease (DDD) is still controversial. The purpose of our study is to compare pure lateral (LLIF) and oblique lateral (OLIF) approaches for the treatment of lumbar DDD from L1-L2 to L4-L5, in terms of clinical and radiological outcomes. MATERIALS AND METHODS: 45 patients underwent lumbar interbody fusion for pure lumbar DDD from  L1-L2 to L4-L5 through LLIF (n = 31, mean age 62.1 years, range 45-78 years) or OLIF (n = 14, mean age 57.4 years, range 47-77 years). Clinical evaluations were performed with ODI and SF-36 t

Radiologic Factors Associated With the Dynamic Change of Dural Sac Diameter in Lumbar Spine: A Kinematic MRI Study.

STUDY DESIGN: This is a cross-sectional radiologic study. OBJECTIVE: The purpose of this study was to assess radiologic features related to dynamic changes of dural sac diameter using kinematic magnetic resonance imaging. SUMMARY OF BACKGROUND DATA: It is well known that the cross-sectional area of the lumbar spinal canal decreases during movement from flexion to extension. However, the radiologic characteristics related to dynamic changes of the lumbar spinal canal have not been elucidated. METHODS: A total of 453 consecutive patients with clinically symptomatic low back pain or radiculopa

Adjacent segment degeneration after intervertebral fusion surgery by means of cervical block vertebrae.

BACKGROUND: The prevalence of cervical block vertebrae is unknown. Furthermore, there is no consensus on the cause of adjacent segment degeneration (ASD) after cervical fusion. QUESTIONS/PURPOSES: (1) What is the incidence of cervical block vertebrae? Is there a gender difference? (2) Among cervical block vertebrae cases, is ASD related to age and segment? And what is the specific relationship? (3) What are the imaging findings and characteristics of this disease? PATIENTS AND METHODS: We retrospectively diagnosed cervical block vertebrae cases with bony fusions in any segments of the cervic

Thoracic disc herniation: Surgical treatment.

Thoracic disc herniation is rare and mainly occurs between T8 and L1. The herniation is calcified in 40% of cases and is labeled as giant when it occupies more than 40% of the spinal canal. A surgical procedure is indicated when the patient has severe back pain, stubborn intercostal neuralgia or neurological deficits. Selection of the surgical approach is essential. Mid-line calcified hernias are approached from a transthoracic incision, while lateralized soft hernias can be approached from a posterolateral incision. The complication rate for transthoracic approaches is higher than that of pos

Progressive Brown-Séquard syndrome: A rare manifestation of cervical disc herniation.

Brown-Séquard syndrome (BSS) is a rare syndrome and is often described in association with spinal cord injury resulting from hemisection of the spinal cord. BSS due to cervical disc herniation is rare with often delayed diagnosis. The importance of early recognition with imaging cannot be over-emphasized, as the prognosis is better when compared to traumatic and vascular etiologies of BSS. We report a rare case of BBS in an 86-year-old man secondary to cervical disc herniation who had dramatic improvement after surgical intervention. This case highlights the unusual presentation from a very co

Dynamic stabilization using the Dynesys system versus posterior lumbar interbody fusion for the treatment of degenerative lumbar spinal disease: a clinical and radiological outcomes-based meta-analysis.

OBJECTIVE The Dynesys, a pedicle-based dynamic stabilization (PDS) system, was introduced to overcome the drawbacks of fusion procedures. Nevertheless, the theoretical advantages of PDS over fusion have not been clearly confirmed. The aim of this study was to compare clinical and radiological outcomes of patients who underwent PDS using the Dynesys system with those who underwent posterior lumbar interbody fusion (PLIF). METHODS The authors searched PubMed, Embase, Web of Science, and the Cochrane Database. Studies that reported outcomes of patients who underwent PDS or PLIF for the treatment

Biportal Endoscopic Lumbar Decompression for Lumbar Disk Herniation and Spinal Canal Stenosis: A Technical Note.

Background and Study Aims Endoscopic lumbar diskectomy through the interlaminar window is gaining recognition. Most of the literature describes these endoscopic procedures using specialized uniportal multichannel endoscopes. However, a single portal limits the motion of the instruments and obscures visualization of the operating field. To overcome this limitation, we propose a new technique that utilizes two portals to access the spinal canal. The biportal endoscopic lumbar decompression (BELD) technique uses two portals to treat difficult lumbar disk herniations and also lumbar spinal stenose

Percutaneous endoscopic lumbar discectomy for a huge herniated disc causing acute cauda equina syndrome: a case report.

Microsurgery for lumbar disc herniation that requires surgical intervention has been well described. The methods vary from traditional open discectomy to minimally invasive techniques. All need adequate preanesthetic preparation of patients as general anesthesia is required for the procedure, and nerve monitoring is necessary to prevent iatrogenic nerve injury. Conventional surgical techniques sometimes require the removal of the corresponding lamina to assess the nerve root and herniated disc, and this may increase the risk for posterior instability of the vertebral body. Should this occur, f

Routine Upright Imaging for Evaluating Degenerative Lumbar Stenosis: Incidence of Degenerative Spondylolisthesis Missed on Supine MRI.

STUDY DESIGN: A retrospective cohort. BACKGROUND: Degenerative spondylolisthesis (DS) with lumbar stenosis is a well-studied pathology and diagnosis is most commonly determined by a combination of magnetic resonance imaging (MRI) and standing radiographs. However, routine upright imaging is not universally accepted as standard in all practices. To the best of our knowledge, there has been no study investigating the incidence of missed diagnosis of DS evident only on standing lateral or dynamic radiographs when compared with sagittal alignment on MRI. OBJECTIVE: The authors hypothesize that s
