Perioperative management of the adult with cystic fibrosis.

Since cystic fibrosis (CF) was first differentiated from celiac disease in 1938, the medical care of patients with CF has substantially improved. These improvements have resulted in a significant increase in median survival and the quality of life experienced by patients. The resultant increase in survival has caused the "average" CF patient to be a young adult and not a child. The gene that causes CF was first identified in 1989 and is the first gene discovered by positional cloning. Unfortunately, gene therapy for CF has not been successful, although it continues to hold great promise for fu

Robotic Parenchyma-Preserving Pancreatic Resection for Focal Congenital Hyperinsulinism in an Infant.

BACKGROUND: Robot-assisted surgery has been increasingly adopted over traditional laparoscopic and open procedures. However, its application in neonates and infants remains limited. Restricted space, delicate tissues, and the need for special instruments complicate its use. This case report describes the application of robot-assisted surgery in an infant with focal congenital hyperinsulinism. MATERIALS AND METHODS: We describe the management of focal congenital hyperinsulinism in an infant using robotic surgery with indocyanine green injection, intraoperative ultrasound, and a suture-based re

A preoperative risk-scoring model for predicting intraoperative hemorrhage during endoscopic submucosal dissection for gastric neoplasms.

BACKGROUND: Intraoperative hemorrhage is the most frequent complication of endoscopic submucosal dissection (ESD). We aim to develop and externally validate a preoperative risk-scoring model for predicting intraoperative hemorrhage during ESD for gastric neoplasms. METHODS: Patients who underwent ESD for gastric neoplasms from four endoscopy centers were enrolled in this retrospective study, with 1123 patients in derivation cohort and 345 patients in validation cohort. In derivation cohort, univariable and multivariable logistic regression analysis were used to identify independent preoperati

Staged Reconstruction Is Not Necessary Following Oncologic Resection of Superficial Myxofibrosarcoma.

BACKGROUND: Myxofibrosarcomas are notoriously highly infiltrative soft-tissue sarcomas, making negative surgical margins difficult to obtain. Recently, vacuum-assisted closure (VAC) is used to delay wound closure until a negative margin has been achieved; however, this can delay care and increase costs. Our institution has historically performed single-stage resections with intraoperative frozen margin analysis and reconstruction in these patients. The purpose of this study is to report the outcomes of this technique. METHODS: We reviewed 112 patients (62 males, mean age 70 ± 14 years) with s

Prophylactic bilateral internal iliac balloon occlusion to control hemorrhage in placenta accreta spectrum: a boon for obstetricians.

BACKGROUND: Placenta accreta spectrum (PAS) is abnormal placental adhesion beyond superficial myometrium, which may lead to severe life-threatening hemorrhage requiring massive blood transfusions in the peripartum period. Prophylactic balloon catheterization of bilateral internal iliac arteries with or without additional embolization with Cesarean hysterectomy in patients with PAS prevent excessive intraoperative hemorrhage and may also obviate the need for hysterectomy. PURPOSE: To study the efficacy of intervention radiological procedures in controlling intraoperative hemorrhage in patients

Intraoperative versus postoperative rendezvous endoscopic retrograde cholangiopancreatography to treat common bile duct stones during cholecystectomy.

BACKGROUND AND AIM: The rendezvous postoperative endoscopic retrograde cholangiopancreatography (ERCP) technique has been introduced as a modification of the single-session rendezvous intraoperative ERCP procedure in the management of concurrent common bile duct stones during cholecystectomy. There are no reports on the impact of this modified technique on post-ERCP morbidity. The objective of the present study was to study and compare the rendezvous techniques in terms of procedure-associated morbidities, such as post-ERCP pancreatitis and postoperative infections. METHODS: The Swedish Natio

Awake Microvascular Decompression for Trigeminal Neuralgia: Concept and Initial Results.

BACKGROUND: In this initial series, we evaluated the use of microvascular decompression (MVD) under an awake anesthesia protocol ("awake" MVD) to assess whether intraoperative pain evaluation can identify and mitigate insufficient decompression of the trigeminal nerve, improving surgical outcomes, and possibly expand the indications of MVD in patients with comorbidities that would preclude the use of general endotracheal anesthesia (GEA). METHODS: An Institutional Review Board-approved prospective study of 10 consecutive adults who underwent MVD for trigeminal neuralgia (TN) was conducted. Th

Perioperative Bioccipital Watershed Strokes in Bilateral Fetal Posterior Cerebral Arteries During Spinal Surgery.

BACKGROUND: Vision loss due to cerebral infarction during spinal surgery is less described. Intraoperative hypotension would be a leading cause. Patients with variation of the circle of Willis could be more prone to present stroke in this context, but reports are lacking to sustain the theory. Bilateral occipital watershed ischemic strokes have never been described before. We report the case of a patient with a fetal origin of both posterior cerebral arteries (PCAs), presenting this particular anatomic stroke following lumbar laminectomy surgery for spinal stenosis during which intraoperative

Use of Peripheral Insertion Central Venous Catheter in Kidney Transplant Patients: An Alternative.

Kidney transplantation continues to be the treatment of choice in patients with end-stage chronic kidney disease. Unfortunately, most patients in the adult population have coexisting diseases. Anemia, platelet dysfunction, and changes to the cardiovascular system are not the exception in the evolution of the disease. This group of patients has an increased risk of intraoperative and postoperative complications associated with the general conditions of their body and other complications related to the surgical procedure. For this reason, adequate control of fluids and electrolytes under adequat

Perioperative Management of Spinal Cord Stimulators and Intrathecal Pain Pumps.

Spinal cord stimulators (SCSs) and intrathecal pain pumps (IPPs) are implantable devices used in the management of chronic pain or spasticity. Complications, such as infection, lead migration/failure, cerebrospinal fluid leak, neurologic injury, and other medical complications, can occur after placement and may require surgical intervention. Orthopaedic surgeons may encounter patients with these devices and should have a basic understanding of their function. In addition, they should be aware that patients may have residual stenosis or deformity contributing to their symptoms; thus, spine surg

Technique tip: percutaneous fixation of partial incongruous Lisfranc injuries in athletes.

Open reduction with screw fixation is considered the standard surgical approach for injuries of the Lisfranc complex in athletes. However, multiple incisions are required, which increase the risk for postoperative complications. We present a novel percutaneous reduction and solid screw fixation technique that may be a viable option to address partial incongruous injuries of the Lisfranc complex in athletes. At our institution, no intraoperative or postoperative complications have been encountered. Screw breakage did not occur. Reduction of the second metatarsal was considered anatomic across a

Pseudoexfoliation syndrome and cataract surgery by phacoemulsification.

Pseudoexfoliation syndrome is a systemic age-related disease in which abnormal extracellular material is produced and accumulates in many ocular tissues. Its ocular manifestations involve all of the structures of the anterior segment as well as conjunctiva and orbital structures. The presence of pseudoexfoliation should alert the physicians to the increased risks associated during and after cataract surgery. Increasing awareness of this condition are important in the detection and preoperative determination of patients inclined to be at greater risk for complications during surgery. Data regar

Single-Stage Versus Staged Fasciectomy for Severe PIPJ Contractures in Dupuytren Disease: A Retrospective Comparative Study.

PURPOSE: This study compared the clinical effectiveness and safety of single-stage fasciectomy versus a two-stage approach for the treatment of severe proximal interphalangeal joint contractures in Dupuytren disease. The two-stage approach involves first using an external extension torque device (EETD) to correct the deformity, followed by removal of the device and fasciectomy at the second stage. We sought to determine whether the staged approach would (1) improve intraoperative correction and reduce residual contracture, (2) decrease operative time for fasciectomy, and (3) reduce complicatio
