Identification of an Ultra-Rare GLA Frameshift Variant in a South African Family With Hypertrophic Cardiomyopathy: A Case Report.

Fabry disease (FD) is an X-linked deficiency in glycosphingolipid metabolism caused by pathogenic variation in GLA. FD can mimic hypertrophic cardiomyopathy (HCM). Here, we present a South African patient of European ancestry with HCM where subsequent genetic analysis led to a diagnosis of FD. He was diagnosed with HCM at the age of 53 when he presented with new-onset atrial fibrillation (AF) and a left occipital cerebral infarction. He reported receiving treatment for recurrent pneumothoraxes in his mid-20s and suffered a transient ischemic attack (TIA) seven years prior to his diagnosis. Dur

Bardet-Biedl syndrome: a rare cause of end-stage kidney disease. Case report.

INTRODUCTION: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive disorder. Diagnosis can be established with specific symptom criteria, confirmed by genetic testing. Early detection and monitoring are crucial due to the high risk of serious organ damage in affected children. We report a case of BBS at younger age admitted to our training program for severe renal failure. Our case is the first reported observation in our country, which will contribute to scientific research in Morocco and in Africa. CASE PRESENTATION: An 18-year old female born to consanguineous parents, with a history

High urinary CXCL10/Cr with onset of Burkitt lymphoma in a pediatric kidney transplant recipient.

BACKGROUND: Urinary CXCL10/Cr is a promising diagnostic tool for early detection of TCMR in pediatric transplant recipients, and most studies focus on its utility in the context of localized allograft inflammation thus far. Other sources of inflammation that may be detected by CXCL10 are less clear. METHODS: We present a case review of a patient with BL, who was enrolled in a prospective trial of urinary CXCL10 monitoring. To evaluate the potential confounding, we tested for association of CXCL10/Cr and EBV viral load in a prospective cohort of pediatric transplant recipients with serial test

Consequences of chronic kidney disease in chronic obstructive pulmonary disease.

BACKGROUND: The combination of chronic obstructive pulmonary disease (COPD) and chronic kidney disease (CKD) is associated with a higher prevalence of comorbidities and increased mortality. The impact of kidney function on patient-centered outcomes in COPD has not been evaluated. METHODS: Patients from the German COPD and Systemic Consequences - Comorbidities Network (COSYCONET) cohort COPD were analysed. CKD was diagnosed if the estimated glomerular filtration rate (eGFR) measurements were < 60 mL/min/1.73m2 at study inclusion and six month later. The effect of CKD, on comorbidities, symptom

Donor-derived strongyloidiasis after organ transplantation in Norway.

Strongyloides stercoralis is an intestinal helminth which in humans can cause asymptomatic chronic infection maintained for decades through its auto-infective cycle. During solid organ transplantation, recipients may unintentionally receive an organ infected with strongyloides. This is a very rare complication but may have deadly outcome if not detected. We hereby report two transplant recipients whom developed Strongyloides hyperinfection syndrome after organ transplantation from the same deceased donor. Recipient 1 was kidney transplanted and presented at day 65 post engraftment with diarrhe

Antibody-Mediated Rejection in Sensitized Nonhuman Primates: Modeling Human Biology.

We have established a model of sensitization in nonhuman primates and tested two immunosuppressive regimens. Animals underwent fully mismatched skin transplantation, and donor-specific antibody (DSA) response was monitored by flow cross-match. Sensitized animals subsequently underwent kidney transplantation from their skin donor. Immunosuppression included tacrolimus, mycophenolate, and methylprednisolone. Three animals received basiliximab induction; compared with nonsensitized animals, they showed a shorter mean survival time (4.7 ± 3.1 vs. 187 ± 88 days). Six animals were treated with T cel

Effects of curcumin and captopril on the functions of kidney and nerve in streptozotocin-induced diabetic rats: role of angiotensin converting enzyme 1.

Oxidative stress and inflammation are involved in the development and progression of diabetes and its complications. The renin-angiotensin system also plays an important role in the pathogenesis of diabetes and its complications. We hypothesized that curcumin and captopril would restore the kidney and nerve functions of diabetic rats through their angiotensin converting enzyme 1 (ACE1) inhibiting activity as well as their antioxidant and anti-inflammatory effects. Diabetes was induced by a single intraperitoneal injection of streptozotocin (100 mg·kg(-1) body weight). One week after induction

Protective efficacy of antigenic fractions in mouse models of cryptococcosis.

Infections due to the encapsulated fungus Cryptococcus neoformans are a significant cause of morbidity and mortality in patients with impaired T-cell function, particularly those with AIDS. Presumably then, T-cell responses to cryptococcal antigens are critical for protection against this ubiquitous fungus. To test the protective efficacy of these antigens as vaccine candidates, secreted cryptococcal antigens were separated by concanavalin A affinity chromatography into adherent (mannoprotein [MP]) and nonadherent (flowthrough [FT]) fractions, and the fractions were tested in murine models of

Case Report: Falsely Elevated Tacrolimus Concentrations in a Kidney Transplant Recipient.

Tacrolimus (TAC) is a cornerstone of immunosuppressive therapy after kidney transplantation but has a narrow therapeutic window, requiring strict therapeutic drug monitoring. Several analytical methods are available for serum concentration measurement and methodological interference may lead to clinically misleading results. We report the case of a 67-year-old female kidney transplant recipient who presented with persistently elevated tacrolimus concentrations despite drug discontinuation. TAC concentrations were initially measured using the affinity column-mediated immunoassay (ACMIA), showin

Concurrent Anti-glomerular Basement Membrane (Anti-GBM) Disease and Immunoglobulin A Nephropathy (IgAN) Presenting With Severe Pulmonary-Renal Syndrome: A Case Report.

Anti-glomerular basement membrane (anti-GBM) disease is a rare, aggressive autoimmune condition that typically presents as rapidly progressive glomerulonephritis and can be complicated by pulmonary haemorrhage. The co-occurrence of anti-GBM disease and IgA nephropathy (IgAN) is extremely uncommon, though increasingly recognised as a distinct clinical and pathological entity. We describe a 51-year-old Sudanese man who presented with acute hypoxaemic respiratory failure, haemoptysis, severe anaemia, and acute kidney injury. Testing showed high levels of anti-GBM antibodies, with negative anti-ne

Neutrophil extracellular traps in the animal model of adenine-induced chronic kidney disease.

Neutrophil extracellular traps (NETs) are suggested to play a role in chronic kidney disease (CKD). Whether they are indeed involved in the pathogenesis of animal models of CKD has not been proved. This study tested the hypothesis that a genetic deficiency of peptidylarginine deiminase 4 (PAD4) - a regulator of NETs production will protect mice from developing adenine-induced nephropathy as a model of CKD. Adult male Padi4-/- mice and their wild-type counterparts received 100 mg/kg adenine or saline i.p. daily for 14 days. Markers of renal function and NETs-related biomarkers were assessed dur

Renal Pathology of Ciliopathies.

Renal ciliopathies are a group of genetic disorders that affect the function of the primary cilium in the kidney, as well as other organs. Since primary cilia are important for regulation of cell signaling pathways, ciliary dysfunction results in a range of clinical manifestations, including renal failure, cyst formation, and hypertension. We summarize the current understanding of the pathophysiological and pathological features of renal ciliopathies in childhood, including autosomal dominant and recessive polycystic kidney disease, nephronophthisis, and Bardet-Biedl syndrome, as well as skele

Molecular approaches to chronic kidney disease.

PURPOSE OF REVIEW: The progression of chronic kidney disease to terminal renal failure remains a major challenge in nephrology. Definition of the dynamic differences in gene regulation, protein interaction and protein function in this process might allow the development of rationally designed management strategies for the individual patient. Current approaches to identifying the molecular markers required to implement this 'personalized medicine' concept in progressive renal failure will be presented in this review. RECENT FINDINGS: In small populations, molecular fingerprints derived from re

Retrieval of renal function after renal artery stenting improves event-free survival in a subgroup analysis of the Cardiovascular Outcomes in Renal Atherosclerotic Lesions trial.

OBJECTIVE: The Cardiovascular Outcomes in Renal Atherosclerotic Lesions (CORAL) trial, a multicenter randomized controlled trial with 947 patients, concluded that there was no benefit of renal artery stenting (RAS) over medical therapy. However, patients with chronic kidney disease (CKD) were not analyzed separately in the CORAL trial. CKD is a risk factor for cardiovascular and renal morbidity. We hypothesized that improved renal function after RAS would be associated with increased long-term survival and a lower risk of cardiovascular and renal events in patients with CKD. METHODS: This pos

Role of Cardiac Troponins in Predicting Adverse Outcomes in Acute Coronary Syndrome With Renal Dysfunction.

A substantial global cause of mortality as well as disability is acute myocardial infarction (AMI). It is also widespread knowledge that patients with chronic kidney disease (CKD) possess greater mortality and cardiovascular disease risks than the rest of the population. A vital biomarker for the diagnosis of AMI is high-sensitive cardiac troponin T (hs-cTnT). Individuals afflicted with severe CKD frequently exhibit increased hs-cTnT levels, which can pose a significant diagnostic challenge in cases of non-ST elevation acute coronary syndrome (NSTE-ACS) necessitating revascularization. [Extremely severe pulmonary lymphangioleiomyomatosis with combined brain, liver, kidney, and lung involvement: a case report].

We reported a case of extremely severe tuberous sclerosis complex-associated lymphangioleiomyomatosis (TSC-LAM) involving the brain, liver, kidneys, and lungs that was successfully treated with sirolimus, with a complete 17-year follow-up. The patient was diagnosed in 2008 and experienced rapid progression; by 2012, the patient had developed respiratory failure, severe anemia, renal insufficiency, and extreme abdominal distension. After 3 days of treatment with sirolimus, the aforementioned symptoms improved significantly, and respiratory failure resovled. Over the following 7 years of regular

HypoxamiR regulation and function in ischemic cardiovascular diseases.

SIGNIFICANCE: MicroRNAs (miRNAs) are deregulated and play a causal role in numerous cardiovascular diseases, including myocardial infarction, coronary artery disease, hypertension, heart failure, stroke, peripheral artery disease, kidney ischemia-reperfusion. RECENT ADVANCES: One crucial component of ischemic cardiovascular diseases is represented by hypoxia. Indeed, hypoxia is a powerful stimulus regulating the expression of a specific subset of miRNAs, named hypoxia-induced miRNAs (hypoxamiR). These miRNAs are fundamental regulators of the cell responses to decreased oxygen tension. Certain

Growth suppression of glioma cells by PTEN requires a functional phosphatase catalytic domain.

Deletions of all or part of chromosome 10 are the most common genetic alterations in high-grade gliomas. The PTEN gene (also called MMAC1 and TEP1) maps to chromosome region 10q23 and has been implicated as a target of alteration in gliomas and also in other cancers such as those of the breast, prostate, and kidney. Here we sought to provide a functional test of its candidacy as a growth suppressor in glioma cells. We used a combination of Northern blot analysis, protein truncation assays, and sequence analysis to determine the types and frequency of PTEN mutations in glioma cell lines so that

Alloplastic replacement of the urinary bladder.

PURPOSE: The loss of a functioning bladder is nowadays mostly managed by replacement with gastrointestinal segments. Management of these urinary diversions is difficult and the long-term effects are unknown. We developed a silicone rubber prosthesis for alloplastic replacement of the urinary bladder and tested it in an animal experiment. MATERIAL AND METHODS: Two artificial bladders were implanted into subcutaneous pockets on each side of the anterior abdominal wall. Cystectomy was performed and each prosthesis was connected to one kidney. Distally, a Y-shaped tube sutured to the urethral stu

A three-year randomized, double-blind, placebo-controlled study of lanreotide in stage 2/3 autosomal dominant polycystic kidney disease.

INTRODUCTION: Somatostatin analogues reduce liver and kidney cyst volume in autosomal dominant polycystic kidney disease (ADPKD), but randomized trials have not demonstrated a consistent benefit on kidney function decline. We tested whether the analogue lanreotide slows glomerular filtration rate (GFR) decline over three years in adults with ADPKD stages 2/3. METHODS: In this randomized, double-blind, placebo-controlled trial, we enrolled adults with ADPKD and measured GFR (mGFR) 30-89 ml/min/1.73 m2. Patients were randomized to receive monthly injections of lanreotide 120 mg or 0.9% sodium c
