Risk factors for failure of manipulation under anesthesia after total knee arthroplasty.

BACKGROUND: Manipulation under anesthesia (MUA) is a well-established treatment for stiffness after total knee arthroplasty (TKA). Risk factors for failure of MUA remain largely unknown. The primary aim of this study was to identify risk factors for failure of MUA after TKA. METHODS: We performed a retrospective cohort study including 470 patients who underwent MUA after primary TKA with minimum 2 year follow-up. Patients were grouped into success (n = 412) or failure (n = 58) cohorts; failure was defined as flexion < 90° at most recent follow-up or revision for stiffness. The increase in fle

Soft tissue can absorb surprising amounts of energy during knee exoskeleton use.

Soft tissue at the human-exoskeleton interface can deform under load to absorb, return and dissipate the mechanical energy generated by the exoskeleton. These soft tissue effects are often not accounted for and may mislead researchers on the actual joint assistance an exoskeleton provides. We assessed the effects of soft tissue by quantifying the performance and energy distribution of a knee exoskeleton under different assistance strategies using a synthetic lower limb phantom. The phantom emulated knee kinematics and soft tissue deformation at the exoskeleton interface. We loaded the exoskele

Complete Release of the Superficial Medial Collateral Ligament in Total Knee Arthroplasty.

BACKGROUND: Proper soft-tissue balance was essential in total knee arthroplasty (TKA). Superficial medial collateral ligament (sMCL) release has been recommended in correction of severe varus knee. However, it has concerns of overcorrection. This study aimed to analyze coronal plane laxity in sMCL-released TKA patients. METHODS: We prospectively collected data from TKA patients who were operated from January 2015 to November 2018. All patients went through the same surgical steps; however, sMCL was left intact in mild-to-moderate deformity (sMCL-intact), while it was completely released in pa

Challenges and Complications in Treating Total Knee Arthroplasty in Morbidly Obese Patients.

The number of total hip and total knee replacements has increased steadily each year. Today, they are routinely performed all over the world. The increasing prevalence of obesity has led to an increase in the number of total knee arthroplasties (TKAs) performed in patients with a higher body mass index (BMI) and also has a higher rate of complications. Possible complications are infection, instability, misalignment, osteolysis, or fracture. Revision arthroplasty can be challenging due to bone defects, ligament instability, and difficulty in fixation. We present the case of a 66-year-old morbid

Soft tissue laxity is highly variable in patients undergoing total knee arthroplasty.

BACKGROUND: One major goal of total knee arthroplasty (TKA) is to achieve balanced medial and lateral gaps in flexion and extension. While bone resections are planned by the surgeon, soft tissue laxity is largely intrinsic and patient-specific in the absence of additional soft tissue releases. We sought to determine the variability in soft tissue laxity in patients undergoing TKA. METHODS: We retrospectively reviewed 113 patients undergoing TKA. Data on preoperative knee deformity were collected. Data from a dynamic intraoperative stress examination were collected by a robotic tracking system

Management of Femoral Condyle Insufficiency Fractures Following Total Knee Arthroplasty (FCIF-TKA).

BACKGROUND: Femoral condyle insufficiency fractures following total knee arthroplasty (FCIF-TKA) are rare but significant complications. These fractures, characterized by atraumatic bone insufficiency near the femoral component, present unique challenges in postoperative care, often necessitating femoral component revision. METHODS: This study retrospectively reviewed 835 primary total knee arthroplasties performed by a single surgeon, identifying six cases of FCIF-TKA. Patient demographics, implant details, radiographic findings, and clinical outcomes were analyzed with up to four years of f

Severe inwardly pointing knee after medial patellofemoral ligament reconstruction: a case report.

We report the case of a 26-year-old woman who presented with a profound gait disturbance and total disability following a medial patellofemoral ligament (MPFL) reconstruction for recurrent patellar dislocation. It is common knowledge that patellar instability is associated with multiple risk factors, including but not limited to loss of the MPFL, trochlear dysplasia, patella alta, an abnormally placed tibial tuberosity on the tibia, quadriceps contracture, genu valgum, excess of femoral anteversion, excess of external tibial torsion, and foot pronation. Since the relative importance of each is

Diet influences knee osteoarthritis osteophyte formation via gut microbiota and serum metabolites.

Osteophyte formation, a key indicator of osteoarthritis (OA) severity, remains poorly understood in its relation to gut microbiota and metabolites in knee osteoarthritis (KOA). We conducted 16S rDNA sequencing and untargeted metabolomics on fecal and serum samples from 20 healthy volunteers, 80 KOA patients in Guangdong, and 100 in Inner Mongolia, respectively. Through bioinformatics analysis, we identified 3 genera and 5 serum metabolites associated with KOA osteophyte formation. Blautia abundance negatively correlated with meat, cheese, and bean consumption. The 5 serum metabolites negativel

Knee osteonecrosis after SARS-CoV-2 infection: a systematic case-based review.

BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the virus responsible for coronavirus disease 2019 (COVID-19). Patients with COVID-19 manifested symptoms mainly related to the respiratory system, but also the musculoskeletal system can be involved. COVID-19 has been described as a possible cause of knee osteonecrosis (ON). A systematic review was performed to investigate the hypothetical correlation between COVID-19 and knee ON. METHODS: Inclusion criteria were all articles reporting cases of knee ON after a diagnosis of SARS-CoV-2 infection. Considering that COVID

Unveiling distinct kinematic profiles among total knee arthroplasty candidates through clustering technique.

BACKGROUND: Characterizing the condition of patients suffering from knee osteoarthritis is complex due to multiple associations between clinical, functional, and structural parameters. While significant variability exists within this population, especially in candidates for total knee arthroplasty, there is increasing interest in knee kinematics among orthopedic surgeons aiming for more personalized approaches to achieve better outcomes and satisfaction. The primary objective of this study was to identify distinct kinematic phenotypes in total knee arthroplasty candidates and to compare differ

Association between venous leg ulcers and knee osteoarthritis: A Mendelian randomization study.

To gain a clearer understanding of the relationship between venous leg ulcers (VLUs) and knee osteoarthritis (KOA), we performed a two-sample, bidirectional Mendelian randomization (MR) analysis in this study. The present MR was carried out using summary data from publicly available genome-wide association studies. After filtering single-nucleotide polymorphism (SNP), we applied a variety of MR methods including inverse variance weighted (IVW), MR egger, weighted mode, and weighted median. IVW analysis revealed that the genetic association between VLUs and KOA was not significant (β = -0.017;

Complications of Stem Cell-Based Injections for Knee Osteoarthritis: A Systematic Review.

Knee osteoarthritis (OA) remains a common cause of knee pain and dysfunction. Stem cell-based injections have been widely used for the treatment of knee OA, but the types and rates of post-injection complications are not well characterized. We sought to characterize the type and severity of adverse events and quantify the frequency of adverse events associated with stem cell injections used to treat knee OA. We conducted a systematic review that followed the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. We searched the PubMed and the Cochrane library d

Tuina Intervention in Sodium Monoiodoacetate Injection-Induced Rat Model of Knee Osteoarthritis.

Knee osteoarthritis (KOA), a common degenerative joint disorder, is characterized by chronic pain and disability, which can progress to irreparable structural damage of the joint. Investigations into the link between articular cartilage, muscles, synovium, and other tissues surrounding the knee joint in KOA are of great importance. Currently, managing KOA includes lifestyle modifications, exercise, medication, and surgical interventions; however, the elucidation of the intricate mechanisms underlying KOA-related pain is still lacking. Consequently, KOA pain remains a key clinical challenge and
