Risk Factors for the Development of Postoperative Acute Kidney Injury in Patients Undergoing Joint Replacement Surgery: A Meta-Analysis.

A new-onset acute kidney injury (AKI) after arthroplasty impairs rehabilitation and outcome. A prior knowledge of risk factors contributes to a planned preventive management and prognostication. Although many studies have addressed the issue, our objective was to perform a meta-analysis to bring a consensus on the perioperative risk factors promoting AKI postoperatively. We conducted a systematic review and meta-analysis of observational studies reporting risk factors with odds of development of AKI according to the existing criteria after hip or knee replacement surgery. We searched the PubMe

Associations between single-leg postural control and drop-landing mechanics in healthy women.

CONTEXT: Impaired postural control in single-limb stance and aberrant drop-landing mechanics have been implicated separately as risk factors for noncontact anterior cruciate ligament (ACL) injury, but associations between these variables has not been reported. OBJECTIVE: To determine whether there are associations between single-limb postural control and drop-landing mechanics. SETTING: University motion-analysis laboratory. DESIGN: Single-leg-landing kinematic and kinetic data were collected after participants dropped from a hang bar. Postural-control variables COP excursion and velocity w

Does chronic medial collateral ligament laxity influence the outcome of anterior cruciate ligament reconstruction?: a prospective evaluation with a minimum three-year follow-up.

We have shown in a previous study that patients with combined lesions of the anterior cruciate (ACL) and medial collateral ligaments (MCL) had similar anteroposterior (AP) but greater valgus laxity at 30° after reconstruction of the ACL when compared with patients who had undergone reconstruction of an isolated ACL injury. The present study investigated the same cohort of patients after a minimum of three years to evaluate whether the residual valgus laxity led to a poorer clinical outcome. Each patient had undergone an arthroscopic double-bundle ACL reconstruction using a semitendinosus-graci

MR imaging of patellar instability: injury patterns and assessment of risk factors.

First-time patellar dislocation typically occurs with twisting knee motions, during which the medial ligamentous stabilizers rupture, and the patella strikes against the lateral femoral condyle. The typical injury pattern is a tear of the medial patellofemoral ligament (MPFL) and bone bruises of the patella and the lateral femoral condyle. Additionally, complex injuries to bone, cartilage, and ligaments may occur. The ensuing loss of medial restraint favors future patellar dislocations, especially if additional risk factors are present. Recurrent patellar dislocations usually occur in individu

Anterior Cruciate Ligament Injuries in Female Athletes.

Female athletes are at a significantly greater risk of anterior cruciate ligament injury when compared to males. The cause of this is multifactorial, due to interactions of extrinsic, anatomic, genetic, hormonal, and neuromuscular factors. Careful preoperative consideration of the patient's anatomy, biomechanics, and goals is crucial when discussing graft type and adjunct procedures. Female athletes are also significantly less likely to return to sport than males, often due to fear of re-injury and/or distrust of their knee. To optimize success in female athletes, it is important to take a mul

Bifocal Avulsion of the Patellar Tendon From the Distal Patella and Tibial Tuberosity in a Skeletally Immature Patient: A Case Report.

CASE: A 13-year-old boy suffered a sports injury of the right knee. The clinical examination showed swelling, inability to bear weight, and pain involving the inferior apex of the patella and the anterior tibial tuberosity. Imaging studies showed an avulsion fracture, sleeve-like, from the patella apex and avulsion of the tibial tuberosity. The patient underwent open reduction and internal fixation. At the 24-month follow-up, no pain, limitation, or complication was reported. CONCLUSIONS: Bifocal patellar tendon avulsion in a skeletally immature patient is an extremely rare injury, and to our

Popliteal artery reconstruction secondary to minor trauma in a 19-year-old morbidly obese woman.

Obesity is an increasing burden on health-care globally. Significant obesity is presenting at a younger age, with pathology that has not been previously seen. This case report illustrates the catastrophic consequences which may occur when minor trauma occurs in a young person who suffers from morbid obesity. A 19-year-old woman with BMI 50 tripped over an uneven curb, and suffered complete dislocation of the knee with associated popliteal artery injury. She required femoro-popliteal bypass using vein. This case reports the youngest person to suffer from this injury and the first in the UK.

The "reverse Segond" fracture: association with a tear of the posterior cruciate ligament and medial meniscus.

OBJECTIVE: We describe three patients who presented with radiographic findings of a fragment on the medial side of the tibial plateau of the knee that represented an avulsion of the deep portion of the medial collateral ligament. These findings were all associated with disruption of the posterior cruciate ligament and a peripheral medial meniscal tear-the so-called reverse Segond fracture. CONCLUSION: Avulsion fracture at the tibial insertion of the deep component of the medial collateral ligament is a rare finding. When this type of injury is diagnosed, the radiologist should consider poster

Salvage of a below knee amputation stump with a free sensate total sole flap preserving continuity of the posterior tibial nerve.

We illustrate the use of a free sole flap with intact posterior tibial nerve in the coverage of a below knee amputation 6 months after an explosive injury to a 12-year-old child. We discuss the indications for lower leg amputation in children, modalities of reconstruction of the amputation stump and possible reason for the observed change of colour of the flap persisting for 14 days.

Saphenous nerve injury. An unusual arthroscopic complication.

The surgeon should use strict technical vigilance to prevent intraoperative arthroscopic complications. An expert understanding of the anatomy, along with visualization of all instruments during surgery, should prevent inadvertent injury to neighboring structures. This case report represents one unusual complication associated with arthroscopic knee surgery and should serve to heighten the surgeon's awareness that arthroscopy is not a benign procedure.

In Vitro Evaluation of Tibiofemoral Contact Stress and Graft Loading After Modified MacIntosh Anterior Cruciate Ligament Reconstruction.

BACKGROUND: The modified MacIntosh anterior cruciate ligament reconstruction (ACLR) consists of a combined intra- and extra-articular construct using iliotibial band autograft. It is used in skeletally immature patients to avoid physeal injury. However, its effect on articular cartilage contact mechanics, rotational laxity, and graft loading is poorly understood. HYPOTHESIS: Relative to the intact knee, a modified MacIntosh ACLR would (1) increase and anteriorize lateral tibiofemoral contact stress, (2) decrease internal rotation (IR) and varus laxity, and (3) offload the intra-articular graf

Creatine Supplementation in Pediatric Orthopedic Rehabilitation: A Translational Review and Proposed Clinical Trial Framework.

INTRODUCTION: Knee arthroscopy is a commonly performed minimally invasive orthopedic procedure in pediatric and adolescent populations, particularly for sports-related injuries such as ligament tears and meniscal pathology. Optimization of post-operative recovery is essential in this age group to preserve muscle mass, restore neuromuscular control, and facilitate safe return to activity during a critical period of musculoskeletal development. Perioperative nutritional strategies represent a modifiable adjunct to rehabilitation; however, their role in pediatric orthopedic recovery remains insuf

WDR63 enhances the chondrogenic differentiation and regenerative potential of stem cell from apical papilla by facilitating vimentin function to promote mitochondrial fission.

BACKGROUND: Research on cartilage repair in the knee joint is crucial for treating knee arthritis or injuries. The application of mesenchymal stem cells (MSCs) for cartilage tissue regeneration represents a promising therapeutic approach. Among the critical aspects in cartilage formation, the enhancement of MSC chondrogenic differentiation stands as a pivotal challenge. WDR63 is a cytoplasmic dynein that plays a significant role in promoting stem cell differentiation and is closely associated with the cytoskeleton and energy metabolism processes. In the current study, our objective is to eluci

Associations between the meniscal spatial-specific signal indexes of T2-weighted images and the presence of radiographic knee osteoarthritis.

RATIONALE AND OBJECTIVE: Volume and signal intensity of meniscus in T2-weighted images are typical manifestations of meniscal injuries, which are risk factors for the presence of radiographic knee osteoarthritis (RKOA). The objective of this study was to predict the presence of RKOA by proposing six meniscal spatial-specific signal (MSS) indexes. MATERIALS AND METHODS: Ninety subjects with symptomatic KOA were divided into non-RKOA and RKOA groups with a cut-off of Kellgren and Lawrence grade ≥2. Lateral and medial menisci were automatically segmented, and meniscus pixels were categorized int

Extended Anterolateral Approach for Complex Lateral Tibial Plateau Fractures.

Complex fractures of the lateral tibial plateau may extend to the posterior rim of the knee and to the tibial spines. Displaced fractures of the posterolateral corner of the tibial plateau may result in joint incongruity and instability, especially with the knee in flexion. Anatomical reduction of the joint surface and containment of the tibial rim are the primary goals of the treatment in such cases. Dedicated surgical approaches including dissection of the peroneal nerve, sometimes in association with an osteotomy of the fibular head are typically used to address these injuries. Some techniq
