Considering Valproate as a Risk Factor for Rapid Exacerbation of Complex Movement Disorder in Progressed Stages of Late-Infantile CLN2 Disease.

Neuronal ceroid lipofuscinosis type 2 (CLN2 disease, OMIM 204500) is a rare autosomal-recessive lysosomal storage disorder. It is one of the most common neurodegenerative disorders in childhood. Symptoms include epilepsy, rapid motor and language regression, dementia, visual loss, and a complex movement disorder in later stages of the disease. We report on two children with genetically confirmed late-infantile CLN2 disease who developed a severe exacerbation of their complex movement disorder leading to hyperthermia, hyper-CK-emia and decreased level of consciousness over several weeks despite

Carinal resection and reconstruction for respiratory tumors using Miyamoto's technique.

PURPOSE: Surgical processes that involve the carina pose a serious challenge to thoracic surgeons. Although techniques to allow resection and reconstruction have been developed, few institutions have accumulated sufficient experience to achieve meaningful results. There is still a debate about the indications and the morbidity and mortality rates for this type of surgery. METHODS: We have operated on six patients using a modified version of the tracheobronchial end-to-end and bronchial end-to-side anastomosis technique that was developed by Miyamoto and coworkers and reported in the English-l

The validity of Barlow's 1877 case of acquired childhood aphasia: case notes versus published reports.

In 1877, Barlow described a ten-year-old boy with right hemiplegia and aphasia, quick recovery of language function, and subsequent left hemiplegia and aphasia, who was shown to have symmetrical left and right Broca's area lesions at autopsy. The report of this case motivated many writers in the second half of the nineteenth century to develop theories on localization, laterality, equipotentiality and development of specialization, recovery of function, and the role of the right hemisphere (see Finger et al., 2003, for review). This paper presents an analysis of the original archived case note

Fracture and dislocation classification compendium - 2007: Orthopaedic Trauma Association classification, database and outcomes committee.

The purpose of this new classification compendium is to republish the Orthopaedic Trauma Association's (OTA) classification. The OTA classification was originally published in a compendium of the Journal of Orthopaedic Trauma in 1996. It adopted The Comprehensive Classification of the Long Bones developed by Müller and colleagues and classified the remaining bones. In this compendium, the introductory chapter reviews new scientific information about classifying fractures that has been published in the last 11 years. The classification is presented in a revised format that is easier to follow.

A case of multiple congenital anomalies in association with Rett syndrome confirmed by MECP2 mutation screening.

Rett syndrome (RTT) is a severe neurodevelopmental disorder. Apparently normal at birth, girls with RTT undergo developmental regression and acquire a neurological and behavioural phenotype that has been used to define clinical diagnostic criteria for the disorder. Recently mutations in the methyl-CpG binding protein 2 gene (MECP2), located on Xq28 have been identified in females with RTT. We report a girl whose clinical course was complicated by congenital abnormalities of the respiratory tract and gastrointestinal system. In addition neurological abnormalities were evident in the newborn per

Conduction aphasia in a 3-year-old with a left posterior cortical/subcortical abscess.

A 3-year-old, right-handed girl developed a conduction-type aphasia following a second generalized seizure in the setting of a developing abscess involving left subcortical and cortical angular gyrus and arcuate fasciculus, and the posterior corpus callosum. The language disorder was fluent, characterized by age appropriate mean length of utterance and syntax, but with markedly reduced spontaneity of output, rapid rate of speech and mild dysarthria. Comprehension was relatively, but not completely spared. Naming, repetition, and reading (letters) were initially markedly impaired. Improvements

An ECG storage and retrieval system embedded in client server HIS utilizing object-oriented DB.

In the University of Tokyo Hospital, the improved client server HIS has been applied to clinical practice and physicians can order prescription, laboratory examination, ECG examination and radiographic examination, etc. directly by themselves and read results of these examinations, except medical signal waves, schema and image, on UNIX workstations. Recently, we designed and developed an ECG storage and retrieval system embedded in the client server HIS utilizing object-oriented database to take the first step in dealing with digitized signal, schema and image data and show waves, graphics, an

TRAPPC6B biallelic variants cause a neurodevelopmental disorder with TRAPP II and trafficking disruptions.

Highly conserved transport protein particle (TRAPP) complexes regulate subcellular trafficking pathways. Accurate protein trafficking has been increasingly recognized to be critically important for normal development, particularly in the nervous system. Variants in most TRAPP complex subunits have been found to lead to neurodevelopmental disorders with diverse but overlapping phenotypes. We expand on limited prior reports on TRAPPC6B with detailed clinical and neuroradiologic assessments, and studies on mechanisms of disease, and new types of variants. We describe 29 additional patients from 1

Assessment of 2-Year Neurodevelopmental Outcomes in Extremely Preterm Infants Receiving Opioids and Benzodiazepines.

IMPORTANCE: Extremely preterm (EP) infants frequently receive opioids and/or benzodiazepines, but these drugs' association with neurodevelopmental outcomes is poorly understood. OBJECTIVES: To describe the use of opioids and benzodiazepines in EP infants during neonatal intensive care unit (NICU) hospitalization and to explore these drugs' association with neurodevelopmental outcomes at 2 years' corrected age. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was a secondary analysis of data from the Preterm Erythropoietin Neuroprotection (PENUT) Trial, which was conducted among infants b

Patient and Parent Decision-Making in the Setting of Chemotherapy-Induced Sensorineural Hearing Loss.

OBJECTIVES: Children with malignancies may be exposed to ototoxic therapies resulting in sensorineural hearing loss (SNHL). There is no consensus as to when intervention with amplification is necessary due to a variety of factors such as disease status, speech and language development, perceived difficulty with communication, and limitations of technology to fit these challenging losses. The decision to proceed with amplification after cancer can be difficult for patients and families. The purpose of this study is (1) to understand the decision-making (DM) process of childhood cancer survivors

Developing an Antenatal Intervention to Improve Vaccine Uptake Using Parent, Patient and Public Involvement and Engagement in Northern Ireland.

INTRODUCTION: Patient and public involvement and engagement (PPIE) is increasingly recognised as important in health care research, yet reporting of how involvement is undertaken and how it informs intervention development remains inconsistent. In maternal health, information about antenatal vaccination is required early in pregnancy, highlighting the value of involving parents and communities in shaping how such information is developed and shared. This paper describes the use of Parent, Patient and Public Involvement and Engagement (PPPIE) to inform the development of an intervention to supp

Home-Based Learning Opportunities, Responsive Caregiving and the Development of Preschool-Aged Children in Low- and Middle-Income Countries of the East Asia and Pacific Region: A Systematic Review.

BACKGROUND: Optimal early childhood development predicts lifelong health and well-being. A child's immediate environment, especially the home, shapes cognitive, physical, language, motor, social and emotional development. Contextually relevant data on the proximal settings that support preschool-aged children are lacking in low- and middle-income countries (LMICs) across the East Asia and Pacific region. This systematic review synthesises evidence on associations between home-based learning opportunities and caregiving experiences and the cognitive and overall development of children aged 2-5

Review and Developmental Model: Early Childhood Emotion Regulation and Co-Regulation in Autism.

OBJECTIVE: Emotion regulation (ER) and dysregulation (ED) have significant impacts on the mental health and quality of life of autistic individuals and their families, yet little is known about ER development in early childhood autism. This paper proposes a developmental model of disruptions to early ER development, emphasizing parent-child co-regulation, to guide future research and clinical care. METHOD: Empirical research on ER, ED, and parent-child co-regulation in young autistic children (mean age <6 years) was summarized. Insights from theoretical and empirical work on typical ER develo

Association of phthalate exposure with precocious and delayed pubertal timing in girls and boys: a systematic review and meta-analysis.

Exposure to phthalate derivatives has adverse effects on the health and development of humans, especially for children. A growing body of evidence supports the idea that exposure to phthalates can change an individual's physiological set point and the time of puberty in both genders. In this systematic review and meta-analysis, recent studies were evaluated to obtain systematic and regulation results in relation to puberty status and phthalate exposure in girls and boys. We searched English-language papers using Scopus, ISI, and PubMed databases as search engines, with no restriction of time,

[Follow-up study of extremely low birth weight preterm infants to preschool age in the light of perinatal complications].

INTRODUCTION: Owing to the rapid progress of the medical science and technology, the chances of survival of the extremely low birth weight (<1000 g) preterm babies have dramatically improved. Nevertheless, the research findings on their long-term developmental outcome are inconsistent. AIM: Our study has attempted to contribute to the understanding of the developmental mechanisms in the extremely low birth weight preterm infants and to the prediction of the developmental outcomes taking into account of the risk factors of development. METHOD: 34 preterm children who were free of any major ce
