Proteinase 3, Wegener's autoantigen: from gene to antigen.

Proteinase 3 (PR3) is one of four serine protease homologues in the azurophilic granules of neutrophils and granules of monocytes. It is of importance that anti-neutrophil cytoplasmic antibodies (ANCA) in patients with Wegener's granulomatosis (WG) are mainly directed against PR3 only. Furthermore, PR3 is overexpressed in a variety of acute and chronic myeloid leukemia cells. Cytotoxic T lymphocytes specific for a PR3-derived peptide have been shown to specifically lyse leukemia cells that overexpress PR3. This review will focus on PR3 and the characteristics of PR3 that might implicate this p

Next-generation CAR-T therapy for acute myeloid leukemia: bridging innovation with clinical translation.

Acute myeloid leukemia (AML) is a high-risk hematologic malignancy with poor long-term survival and frequent relapse, sustained by leukemic stem cells, antigenic heterogeneity, and an immunosuppressive bone marrow niche. Although chimeric antigen receptor (CAR) T-cell therapy achieves durable responses in B-cell malignancies, its application in AML is restricted by on-target myelotoxicity from antigen overlap with normal progenitors, heterogeneous and dynamic antigen expression, rapid T-cell exhaustion in suppressive microenvironments, limited manufacturing windows with compromised T-cell qual

Cardiac Toxicity Associated with Cancer Immunotherapy and Biological Drugs.

Cancer immunotherapy significantly contributed to an improvement in the prognosis of cancer patients. Immunotherapy, including human epidermal growth factor receptor 2 (HER2)-targeted therapies, immune checkpoint inhibitors (ICI), and chimeric antigen receptor-modified T (CAR-T), share the characteristic to exploit the capabilities of the immune system to kill cancerous cells. Trastuzumab is a monoclonal antibody against HER2 that prevents HER2-mediated signaling; it is administered mainly in HER2-positive cancers, such as breast, colorectal, biliary tract, and non-small-cell lung cancers. Human mesenchymal stromal cells attenuate graft-versus-host disease and maintain graft-versus-leukemia activity following experimental allogeneic bone marrow transplantation.

We sought to define the effects and underlying mechanisms of human, marrow-derived mesenchymal stromal cells (hMSCs) on graft-versus-host disease (GvHD) and graft-versus-leukemia (GvL) activity. Irradiated B6D2F1 mice given C57BL/6 BM and splenic T cells and treated with hMSCs had reduced systemic GvHD, donor T-cell expansion, and serum TNFα and IFNγ levels. Bioluminescence imaging demonstrated that hMSCs redistributed from lungs to abdominal organs within 72 hours, and target tissues harvested from hMSC-treated allogeneic BMT (alloBMT) mice had less GvHD than untreated controls. Cryoimaging m

Leukemia diagnosis and testing of complement-fixing antibodies for bone marrow purging in acute lymphoid leukemia.

In this paper a microplate method is described for diagnosing acute leukemia and for investigating the reactivity of monoclonal antibodies (MoAbs) against membrane antigens in combination with rabbit or murine antibodies to nuclear terminal transferase (TdT). The speed of this method facilitates the investigation of fresh leukemic cells from individual patients and assesses the cytolytic efficacy of the relevant MoAbs in the presence of complement (C'). Lymphoblasts (TdT+) are mixed in equal proportions with known numbers of "inert" cells, eg, RBC or nonleukemic bone marrow (BM). Following inc

Characterization of Immunophenotypic Aberrancies in Adult and Childhood Acute Lymphoblastic Leukemia: Lessons from Regional Variation.

BACKGROUND & OBJECTIVE: Although the antigen expression patterns of acute lymphoblastic leukemia (ALL) are well known, this study attempted to evaluate commonly used immune markers for immunophenotyping of acute leukemia to set the minimum of necessary diagnostic panels by flow cytometry. METHODS: This study evaluated 89 patients referred from all over the country to the Iranian Blood Transfusion Organization (IBTO) in Tehran from 2013 to 2015. We compared the immunophenotype patterns of childhood and adult ALLs including 69(77.5%) B-cell lymphoblastic lymphoma (B-LBL), 2(2.2%) Burkitt's lymp

Notch Partners in the Long Journey of T-ALL Pathogenesis.

T-cell acute lymphoblastic leukemia (T-ALL) is an aggressive hematological disease that arises from the oncogenic transformation of developing T cells during T-lymphopoiesis. Although T-ALL prognosis has improved markedly in recent years, relapsing and refractory patients with dismal outcomes still represent a major clinical issue. Consequently, understanding the pathological mechanisms that lead to the appearance of this malignancy and developing novel and more effective targeted therapies is an urgent need. Since the discovery in 2004 that a major proportion of T-ALL patients carry activatin

Immunological and Clinical Impact of Manipulated and Unmanipulated DLI after Allogeneic Stem Cell Transplantation of AML Patients.

Allogeneic stem cell transplantation (allo-SCT) is the preferred curative treatment for several hematological malignancies. The efficacy of allo-SCT depends on the graft-versus-leukemia (GvL) effect. However, the prognosis of patients with relapsed acute myeloid leukemia (AML) following allo-SCT is poor. Donor lymphocyte infusion (DLI) is utilized after allo-SCT in this setting to prevent relapse, to prolong progression free survival, to establish full donor chimerism and to restore the GvL effect in patients with hematological malignancies. Thus, there are different options for the administra

Asymmetric anti-CLL-1×CD3 bispecific antibody, ABL602 2+1, with attenuated CD3 affinity endows potent antitumor activity but limited cytokine release.

BACKGROUND: Acute myeloid leukemia (AML) is a type of leukemia in adults with a high mortality rate and poor prognosis. Although targeted therapeutics, chemotherapy, and hematopoietic stem cell transplantation can improve the prognosis, the recurrence rate is still high, with a 5-year survival rate of approximately 40%. This study aimed to develop an IgG-based asymmetric bispecific antibody that targets CLL-1 and CD3 for treating AML. METHODS: ABL602 candidates were compared in terms of binding activity, T-cell activation, and tumor-killing activities. ABL602-mediated T-cell activation and tu

Identification and validation of pyroptosis patterns in AML via comprehensive bioinformatics analysis.

Pyroptosis, a lytic inflammatory cell death mechanism, plays dual roles in tumorigenesis, but its clinical relevance in acute myeloid leukemia (AML) remains poorly understood. Through an integrative analysis of 40 pyroptosis-related genes in newly diagnosed AML patients (TCGA, n = 151) and healthy controls (GTEx, n = 386), we identified 32 genes with aberrant expression. Among these, 9 genes were found to be significant prognostic markers, including ELANE (protective), and CASP1, CHMP4B, BAK1, and CHMP2A (risk), which retained their prognostic significance after adjusting for age and gender. Expression of T-cell receptor alpha and beta variable genes in normal and malignant human T cells.

A PCR method was developed to analyse each of 29 families of the T cell receptor V alpha gene and 20 families of the V beta gene at the mRNA level in heterogenous cell populations. All V alpha and V beta families were detectable in blood mononuclear cells from four of six healthy donors. In two donors only V alpha 22 was missing, and all other V alpha and V beta families were detected. V beta family expression was observed in T-leukaemic cell lines Jurkat, HSB, Molt-3 and Molt-4. In contrast, V alpha family expression was not detectable in any cell line except Jurkat cells. In T-cell malignanc

The challenge to further improvements in survival of patients with T-ALL: Current treatments and new insights from disease pathogenesis.

Survival rates for children and adult patients with T-cell acute lymphoblastic leukemia (T-ALL) have improved during the past decade due to optimization of frontline multiagent chemotherapy regimens. The outcome for relapsed T-ALL after initial intensive chemotherapy is frequently fatal, however, because no effective salvage regimens have been developed. Immunotherapy and small molecule inhibitors are beginning to be tested in T-ALL and have the potential to advance the treatment, especially the frontline regimen by eradicating minimal residual disease thus inducing more durable remissions. Vitamins as modulators of Deltaretrovirus infection: implications for HTLV-1 and bovine leukemia virus-associated diseases.

Deltaretroviruses, particularly human T-cell leukemia virus type 1 (HTLV-1) and bovine leukemia virus (BLV), are oncogenic retroviruses responsible for severe lymphoproliferative and inflammatory diseases, including adult T-cell leukemia/lymphoma (ATLL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Despite decades of research, effective curative therapies remain limited, and disease progression is strongly influenced by host immune and metabolic factors. Accumulating evidence indicates that vitamins play critical roles in modulating viral pathogenesis, immune dysregu

T-lymphocyte recovery and function after cord blood transplantation.

The Szabolcs laboratory is focused on understanding the biology of donor-derived cellular immunity in recipients of allogeneic hematopoietic cell transplantation that can be translated into new immunotherapy strategies. To this end, we are focused on developing novel laboratory approaches to analyze and augment immune recovery for high risk patient cohorts without increasing graft-versus-host disease. Much of our work has focused on unrelated cord blood transplantation as the dominant clinical scenario and laboratory model. Our overarching goal is to minimize transplant-related mortality and m

Refractory Clinical Course of Acute Myeloid Leukemia With t(8; 10; 21)(q22; q22; q22.1): A Case Report and Literature Review of This Variant Form.

Acute myeloid leukemia (AML) with t(8;21)(q22;q22.1)/RUNX1::RUNX1T1 is usually associated with a good prognosis and is categorized as a favorable-risk group in the European Leukemia Net recommendations. A variant form involving a three-way translocation is rarely observed in this AML subtype, and the prognostic impact of this form remains unclear. A 43-year-old man presented with anemia and thrombocytopenia. He had diabetes mellitus and class III obesity according to the WHO criteria. After being diagnosed with AML with t(8;10;21)(q22;q22;q22.1), he received standard induction therapy consisti

A molecular and functional analysis of large granular lymphocyte expansions in patients with chronic myelogenous leukemia treated with tyrosine kinase inhibitors.

Tyrosine kinase inhibitor (TKI) therapy has become the standard treatment for chronic myelogenous leukemia (CML). Off-target kinase inhibition has been implicated in the appearance of unique adverse effects, such as colitis and pleural effusions. In addition, some patients present oligoclonal expansions of large granular lymphocytes (LGLs). We sought to further investigate this phenomenon in 64 patients treated with five different TKIs. Clonal expansions of cytotoxic T lymphocytes (CTLs) were identified in all TKI-treated patient groups, but only in dasatinib-treated patients were these expans

HTLV-I infection of WE17/10 CD4+ cell line leads to progressive alteration of Ca2+ influx that eventually results in loss of CD7 expression and activation of an antiapoptotic pathway involving AKT and BAD which paves the way for malignant transformation.

Adult T-cell leukemia/lymphoma (ATLL) is a malignancy slowly emerging from human T-cell leukemia virus type 1 (HTLV-I)-infected mature CD4(+) T-cells. To characterize the molecular modifications induced by HTLV-I infection, we compared HTLV-I-infected WE17/10 cells with control cells, using micro-arrays. Many calcium-related genes were progressively downmodulated over a period of 2 years. Infected cells acquired a profound decrease of intracellular calcium levels in response to ionomycin, timely correlated with decreased CD7 expression. Focusing on apoptosis-related genes and their relationshi

Different chromosomal breakpoints impact the level of LMO2 expression in T-ALL.

The t(11;14)(p13;q11) is presumed to arise from an erroneous T-cell receptor delta TCRD V(D)J recombination and to result in LMO2 activation. However, the mechanisms underlying this translocation and the resulting LMO2 activation are poorly defined. We performed combined in vivo, ex vivo, and in silico analyses on 9 new t(11;14)(p13;q11)-positive T-cell acute lymphoblastic leukemia (T-ALL) as well as normal thymocytes. Our data support the involvement of 2 distinct t(11;14)(p13;q11) V(D)J-related translocation mechanisms. We provide compelling evidence that removal of a negative regulatory ele

Rapid development of myeloproliferative neoplasm in mice with Ptpn11D61Y mutation and haploinsufficient for Dnmt3a.

PTPN11 gain-of-function mutation is the most common mutation found in patients with juvenile myelomonocytic leukemia and DNMT3A loss occurs in over 20% of acute myeloid leukemia patients. We studied the combined effect of both Ptpn11 gain-of-function mutation (D61Y) and Dnmt3a haploinsufficiency on mouse hematopoiesis, the presence of which has been described in both juvenile myelomonocytic leukemia and acute myeloid leukemia patients. Double mutant mice rapidly become moribund relative to any of the other genotypes, which is associated with enlargement of the spleen and an increase in white b
