Effect of Low-Intensity Transcranial Focused Ultrasound Stimulation on Neuropathic Pain: Protocol for a Randomized, Single-Blind, Placebo-Controlled, 3-Arm, Parallel-Group Trial.

BACKGROUND: Neuropathic pain (NP) remains difficult to manage because conventional pharmacological therapies often have limited efficacy and intolerable side effects. Noninvasive neuromodulation techniques have emerged as potential alternatives with fewer adverse effects. Meta-analyses of randomized controlled trials suggest that high-frequency repetitive transcranial magnetic stimulation over the primary motor cortex has analgesic effects; however, its efficacy is modest and restricted by the inability to stimulate deeper brain regions. Low-intensity transcranial focused ultrasound stimulatio

Effect of add-on therapy with leukotriene receptor antagonists and Chungsangboha-tang in patients with asthma: a protocol for a randomized, placebo-controlled, parallel, multicenter trial.

BACKGROUND: Asthma is a chronic disease characterized by airway inflammation and obstruction. Treatment aims to control symptoms with minimal medication, using disease-controlling and symptom-relieving drugs. Inhaled steroids and beta2 agonists are common treatments; however, their long-term use can cause side effects. Leukotriene receptor antagonists (LTRAs) are used in combination with inhaled steroids to manage asthma because of their anti-inflammatory and bronchodilatory effects. Combining LTRAs with Chungsangboha-tang (CSBHT), a Korean medicine, may enhance their efficacy. This study aime

Design and rationale for a pilot trial of D-alanine supplementation to reduce the triglyceride to high-density lipoprotein cholesterol ratio in patients with chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) is an increasingly prevalent disease that adversely affects patients' quality of life and medical expenses. While its progression is linked to lifestyle-related diseases, its association with dyslipidemia remains unclear. We previously showed a significant association between a high triglyceride to high-density lipoprotein cholesterol (TG/HDL-C) ratio and CKD progression and cardiovascular disease in CKD patients. Recent reports indicate that D-alanine regulates circadian rhythms, participates in gluconeogenesis, and exhibits renoprotective effects. Thi

Comparing Analgesic Regimen Effectiveness and Safety after Surgery (CARES): protocol for a pragmatic, international multicentre randomised trial.

INTRODUCTION: Acute pain is commonly experienced by millions of patients who undergo outpatient surgical procedures. Moreover, an increasing number of procedures are performed on an outpatient basis, requiring greater postoperative planning to ensure effective pain management. Analgesic approaches commonly involve prescription opioids and non-steroidal anti-inflammatory drugs (NSAIDs), but an optimal regimen that balances pain and adverse effects has not been identified. In addition, critical gaps in evidence exist regarding how opioids and NSAIDs compare as analgesic regimens after surgery.

Feasibility and acceptability of a personalised self-care support programme for primary care patients with diabetic foot ulcers delivered by wound care nurses: the HEALing study protocol.

INTRODUCTION: Diabetic foot ulcers (DFUs) are highly prevalent and recurrent complications of diabetes mellitus that have significant health and cost implications. Self-care is critical for preventing or delaying DFU and promoting healing, yet adherence to self-care recommendations is low. Interventions using motivational interviewing (MI) have been effective in supporting behaviour change and emotional adjustment, but evidence for DFU is scarce. This study will assess the acceptability, feasibility and preliminary efficacy of an MI-guided programme, Healing DFU through Empowerment and Active

Extreme pulse dosing of 10 kHz spinal cord stimulation: how low can you go?

BACKGROUND: Pulse dosing of high frequency spinal cord stimulation at 10 kHz (10 kHz SCS) may offer comparable clinical benefits as continuous 10 kHz SCS, but extreme pulse dosing (EPD) has not been studied. METHODS: Patients using an implantable pulse generator (IPG) with 10 kHz SCS to treat chronic back or leg pain were enrolled. After baseline assessments, patients underwent "EPD titration" starting at an EPD setting of 3%. Patients who preferred the EPD tried progressively lower EPD settings (0.6%, 0.3%, 0.14%, 0.06%), each for 7-10 days, until reaching an EPD they did not prefer over tha

Study protocol for a single-blind, parallel-group, randomized, controlled superiority trial of intensive versus weekly delivered prolonged exposure for adults with post-traumatic stress disorder.

BACKGROUND: Prolonged exposure (PE) therapy is widely recognized as an effective treatment for post-traumatic stress disorder (PTSD) and is often considered one of the primary options for addressing this condition. Nevertheless, a significant proportion of patients (30-51%) fail to demonstrate clinically significant symptom changes. One of the reasons is that a high proportion of patients drop out from treatment, which often lasts for a minimum of 3-4 months. Hence, there is an urgent need for PTSD treatments that can be delivered to decrease dropout rates. A more intensive PE treatment approa
