Machine learning-based prediction of prolonged length of stay in older patients with type 2 diabetes mellitus and cardiovascular disease.

BACKGROUND: Older patients with type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD) frequently experience prolonged length of stay (PLOS). This condition increases healthcare burden and worsens prognosis. However, no predictive model specifically addresses PLOS in this high-risk multimorbid population. METHODS: This single-center retrospective study included hospitalized older T2DM-CVD patients. PLOS was defined as hospital stay exceeding the 75th percentile of the training set population. Potential predictors were selected via LASSO regression. Eight machine learning (ML) models

Adverse-effect burden as the dominant correlate of antidepressant non-adherence in major depressive disorder: insights from a multicentre machine-learning analysis.

BACKGROUND: Non-adherence to antidepressants contributes to relapse and recurrence in major depressive disorder (MDD). Existing risk-prediction models are drawn largely from chronic conditions in high-income settings; none exist for Pakistani outpatient psychiatry. METHODS: We analysed cross-sectional, patient-reported data from 2513 adults with MDD attending outpatient clinics in Lahore, Rawalpindi and Quetta. The outcome was an Urdu Morisky-Green-Levine score of 3 or higher, with 31 binary predictors from the Urdu Drug Attitude Inventory and Urdu Antidepressant Side-Effect Checklist. Six cl

Pharmacogenomic impact and genetic architecture of toxicity in pediatric acute lymphoblastic leukemia induction therapy: an exploratory modeling approach.

Induction chemotherapy for pediatric Acute Lymphoblastic Leukemia (ALL) is highly curative but accompanied by a severe toxicity profile. Evaluating the genetic susceptibility to these adverse events could inform hypothesis-generating risk stratification. In this exploratory retrospective cohort study (N = 178), we investigated the influence of 14 candidate pharmacogenetic variants on the development of prevalent Adverse Drug Reactions (ADRs, ≥ 10 events). Multiple logistic regression, adjusted for basic demographic and anthropometric covariates (age and body surface area), was utilized across

Association of miR-499, miR-27a, and miR-34a Gene Polymorphisms With Susceptibility to Type 2 Diabetes Mellitus in an Iranian Population: A Case-Control Study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is a multifactorial metabolic disorder influenced by both environmental and genetic factors. MicroRNAs (miRNAs) are involved in the regulation of insulin signaling, inflammation, and pancreatic β-cell function. Genetic polymorphisms in miRNA-related regions may contribute to interindividual differences in T2DM susceptibility, although their functional effects remain uncertain. OBJECTIVE: This study is aimed at evaluating the association of five miRNA polymorphisms, miR-499 rs3746444, miR-27a rs895819/rs895519, miR-34a rs6577555, miR-124a rs531564, a

Individual Participant Data Meta-Analysis Identifies Risk Factors for Acute and Persistent Posttraumatic Stress Disorder and Depression Symptoms Following Trauma.

BACKGROUND: Posttraumatic stress disorder (PTSD) and major depressive disorder (MDD) are common sequelae to trauma. Identifying individuals at risk for persistent symptomology allows for targeted interventions. METHODS: We conducted an individual participant data meta-analysis (IPD-MA) by pooling data from five prospective emergency room and critical care unit studies (N participants = 2571) to examine risk factors for PTSD and MDD. We derived harmonized measures of PTSD and MDD in the acute period (0-60 days) following a traumatic event and in the follow-up persistent period (122-456 days).
