Preventing renal complications in diabetic patients: the Diabetics Exposed to Telmisartan And enalaprIL (DETAIL) study.

Diabetic nephropathy is characterised by hypertension and persistent proteinuria. If ineffectively controlled, a progressive decline in renal function can result in end-stage renal disease. Patients with diabetic nephropathy are also at greatly increased risk of cardiovascular disease. Angiotensin-converting enzyme (ACE) inhibitors display additional renoprotective effects beyond systemic blood pressure lowering, perhaps due to reduction in intraglomerular pressure by inhibition of angiotensin II activity. In type 2 diabetics, ACE inhibitors have variable effects, with some studies showing a r

Reporting health-related quality of life in randomised clinical trials evaluating adjuvant systemic therapy: recommendations of Common-Sense Oncology and the European Organisation for Research and Treatment of Cancer.

The goal of adjuvant systemic therapy after locoregional treatment of cancer is to increase the probability of cure by eradicating micro-metastases. Adjuvant treatment adds side-effects and can impair health-related quality of life (HRQOL); this must be balanced against a reduction in the probability of recurrence, which has a substantial negative effect on HRQOL. Since many adjuvant therapies lead to small gains in relapse-free or overall survival, the severity and duration of treatment-related side-effects and their impact on patient functioning and overall HRQOL are relevant to clinical dec

Standardized Artemisia argyi Extract Alleviates Scopolamine-Induced Memory Impairment in Mice.

Cognitive dysfunction has become a major health issue in the global aging society. For treating cognitive dysfunction or dementia, several small molecules or innovative drugs, including a monoclonal antibody against the amyloid β protein, have been prescribed. However, their adverse effects require the exploration of alternative treatments. Artemisia argyi (Asteraceae) has been used as traditional medicine for gastrointestinal disorders and inflammation. Recent reports suggest it has potential neuroprotective effects; however, evidence regarding its cognitive benefits and underlying mechanisms

Research on vibration control and nonlinear dynamic behavior of railway vehicle systems based on nonlinear energy sinks.

During long-term service, railway vehicles are prone to abnormal vibrations of the car body and bogie frames. To suppress vibrations in both the car body and bogie frames simultaneously, this paper proposes the distributed installation of nonlinear energy sinks (NES) on railway vehicles, with multiple NES units installed on the car body and bogie frames. First, a nine-degree-of-freedom coupled dynamic model of the railway vehicle and NES is established. An approximate analytical solution of the model is derived using the harmonic balance method and verified by numerical simulations. The effect

Impaired Bone Healing and Fracture Complications during Limb Lengthening in a Neurofibromatosis Type 1 Patient Receiving Selumetinib: Case Report and Literature Review.

INTRODUCTION: Neurofibromatosis Type 1 (NF1) is a genetic disorder that may lead to the development of plexiform neurofibromas (Pns). Selumetinib, a selective mitogen-activated protein kinase (MEK1/2) inhibitor, has shown clinical benefit in shrinking inoperable PNs. However, its long-term effects on the skeleton remain unclear. This case describes impaired bone healing and pathological fractures following limb lengthening in a child with NF1 receiving prolonged selumetinib therapy. CASE REPORT: A 5-year-old boy with genetically confirmed NF1 and left lower limb PN-induced overgrowth presente

PM289, a synthetic CB2 in vitro receptor agonist, modulates morphine-induced antinociceptive effect and withdrawal syndrome in an animal model of osteoarthritic pain.

Osteoarthritis is a common condition, and the associated pain is a leading cause of disability. One main challenge in its management stems from the complexity of the disease and the limited efficacy of current analgesic treatments. Although evidence-based clinical guidelines provide valuable recommendations, they often prove inadequate. Opioids, while effective, are not considered first-line therapies due to adverse effects, particularly with long-term use. A promising strategy for chronic pain involves the coadministration of opioid and cannabinoid receptor agonists. This study investigates t

Paternal scrotal heat stress epigenetically impairs spatial memory in F1 male offspring in mice.

Increasing evidence indicates that the maternal preconception environment and lifestyle affect offspring health; however, the effects of paternal preconception environment and lifestyle have rarely been examined. Therefore, we aimed to examine whether paternal infertility-related factors affect offspring produced via assisted reproductive technology in mice. Scrotal heat impairment, which can result from factors such as varicoceles, sauna use, physical inactivity, or occupations such as welding, is a common risk factor associated with reduced male fertility. To model these effects, we exposed

The Rising Use of E-Cigarettes: Unveiling the Health Risks and Controversies.

The use of e-cigarettes has tremendously increased in recent times due to the widespread availability of e-cigarettes in diverse flavors, reduced cost compared to regular cigarettes, and misconception of being comparatively safe, which have led to around 2.55 million US middle and high school students smoking e-cigarettes. These devices use a nicotine-rich liquid, which is aerosolized electronically, producing vapors that may also include hazardous chemicals and heavy metals. E-cigarettes are associated with e-cigarette or vaping-associated lung injury, which presents as an acute respiratory a

New insights into the pathogenesis of SARS-CoV-2 during and after the COVID-19 pandemic.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is responsible for the respiratory distress condition known as COVID-19. This disease broadly affects several physiological systems, including the gastrointestinal, renal, and central nervous (CNS) systems, significantly influencing the patient's overall quality of life. Additionally, numerous risk factors have been suggested, including gender, body weight, age, metabolic status, renal health, preexisting cardiomyopathies, and inflammatory conditions. Despite advances in understanding the genome and pathophysiological ramifications o

Recent advances in Alzheimer's disease: Mechanisms, clinical trials and new drug development strategies.

Alzheimer's disease (AD) stands as the predominant form of dementia, presenting significant and escalating global challenges. Its etiology is intricate and diverse, stemming from a combination of factors such as aging, genetics, and environment. Our current understanding of AD pathologies involves various hypotheses, such as the cholinergic, amyloid, tau protein, inflammatory, oxidative stress, metal ion, glutamate excitotoxicity, microbiota-gut-brain axis, and abnormal autophagy. Nonetheless, unraveling the interplay among these pathological aspects and pinpointing the primary initiators of A

Antioxidative and Anti-Inflammatory Activities of Rosebud Extracts of Newly Crossbred Roses.

Oxidative stress and inflammation are basic pathogenic factors involved in tissue injury and pain, as well as acute and chronic diseases. Since long-term uses of synthetic steroids and non-steroidal anti-inflammatory drugs (NSAIDs) cause severe adverse effects, novel effective materials with minimal side effects are required. In this study, polyphenol content and antioxidative activity of rosebud extracts from 24 newly crossbred Korean roses were analyzed. Among them, Pretty Velvet rosebud extract (PVRE) was found to contain high polyphenols and to show in vitro antioxidative and anti-inflamma

Systemic iron deficiency does not affect the cardiac iron content and progression of heart failure.

Chronic heart failure (HF) is often accompanied by systemic iron deficiency (ID). However, effects of ID on cardiac iron status and progression of HF are unknown. To investigate these effects rats underwent LAD ligation to induce post-myocardial infarction HF or sham operation. After 3 weeks the animals from both groups were randomized into three subgroups: control, moderate ID and severe ID+anemia (IDA) by a combination of phlebotomy and low iron diet for 5 weeks. Serum and hepatic iron content were reduced by 55% and 70% (ID) and by 80% and 77% (IDA), respectively, while cardiac iron content
