[Rational lymph node dissection for lung cancer according to the occurrence lobe and histological type].

Between January 1989 and December 1998, 134 cases of squamous cell carcinoma and 244 cases of adenocarcinoma underwent surgical resection of the lung with systematic lymph node dissection in our hospital. The cN diagnosis by CT scan and pN diagnosis were compared. In squamous cell carcinoma pN 2-3 cases were only one patient (2%) out of 60 patients with cN 0, 5 patients (18%) out of 28 patients with cN 1, and 21 patients (46%) out of 46 patients with cN 2-3. On the other hand in adenocarcinoma pN 2-3 cases were 27 patients (14%) out of 193 patients with cN 0, 3 patients (25%) out of 12 patient

The application of 67Ga scanning in determining the operability of bronchogenic carcinoma.

In a prospective study of 34 patients with histologically proven bronchogenic carcinoma, 67Ga scanning of the chest was used to screen for mediastinal involvement. Mediastinal scans and pathologic findings were in agreement in 27 patients. Two patients had false-negative scans and 5 had positive scans but no evidence of mediastinal tumor. These results indicate a sensitivity of 89%, a specificity of 66.6%, an accuracy of 77% for positive scans, and an accuracy of 83% for negative scans. Overall correlation between 67Ga scanning of the chest and mediastinal-tumor involvement is statistically si

Button battery ingestion in children: Update and practical approach.

The ingestion of button batteries is an increasingly recognized medical emergency in pediatrics, particularly among children under the age of 6. These batteries, which are widely used in everyday electronic devices, can cause serious and potentially life-threatening injuries, especially when they become lodged in the esophagus. Timely diagnosis and immediate intervention are essential to prevent serious complications, including perforation, mediastinitis, and fistula formation. This review summarizes the most recent clinical practice guidelines, with an emphasis on pathophysiology, clinical im

Management of boerhaave's syndrome in the intensive care unit.

Boerhaave's syndrome (BS), or spontaneous esophageal perforation, is a rare and life-threatening condition. Despite advancements in medical and surgical management, BS carries a high mortality rate, up to 50%, and presents significant diagnostic and therapeutic challenges. Management options include both surgical and nonoperative approaches, with the Pittsburgh score providing a useful tool for predicting outcomes and guiding treatment. Complications such as mediastinitis and sepsis are common. This review explores the demographics, risk factors, clinical presentation, diagnostic modalities, a

[Diagnosis and management of peritonsillar abscess].

A peritonsillar abscess is a common infection in young adults. Clinical signs often include unilateral pharyngodynia, a "hot potato" voice, unilateral swelling with uvular deviation, asymmetric palatal contraction, and trismus. Complications include infections of the deep neck spaces and mediastinitis, which require prompt recognition due to their severity. Rapid diagnosis is essential. While not consistently used, CT scanning provides excellent specificity. The management of a peritonsillar abscess is based on an individualized approach that combines antibiotic therapy with interventional pro

Cervicofacial Subcutaneous Emphysema and Pneumomediastinum Following Class V Resin Restoration: A Case Report.

Air can become trapped within the subcutaneous layer of the cervicofacial area, a condition known as subcutaneous emphysema (SE), unexpectedly after routine dental procedures. SE can worsen by extending superiorly to the peri-orbital region and inferiorly to the thorax and abdomen along the fascial planes. Upward progression can result in cavernous sinus thrombosis. Conversely, downward progression can cause a variety of complications such as pneumothorax, mediastinitis, and compression of the venous trunks. The presence of these conditions highlights the significance of prompt identification,

A peculiar foreign body ingestion in 2-year-old girl complicated by esophageal perforation: case report and review of the literature.

Foreign body ingestion is a common pediatric gastrointestinal emergency, which should be suspected in all patients who present with signs of airway obstruction or upper GI bleeding, especially if it developed after the child was left unwitnessed for a while. The most common foreign bodies identified in the literature are button batteries or coins. Early identification and management of suspected foreign body ingestion is crucial as it can lead to devastating complications including bleeding, fistula formation, perforation, mediastinitis, or abscess. Here we report a case of a peculiar foreign

[Beating heart surgical treatment of atrio-esophageal fistula after radiofrequency ablation of atrial fibrillation].

Atrio-esophageal fistula (AEF) is a rare (0.02-0.1%) complication of radiofrequency ablation for atrial fibrillation and is associated with high mortality. It typically presents between 2 and 6 weeks after catheter ablation. AEF was reported to be the second complication as cause of death after radiofrequency ablation with a mortality rate of 71%. Common clinical features of AEF include dysphagia, nausea, heartburn, hematemesis or melena, high fever, sepsis, pericardial or pleural effusions, mediastinitis, seizures, and stroke. Once the diagnosis of AEF is made, early surgical repair is mandat

Redo aortic root and arch replacement with 3 different homografts.

A 74-year-old male entered the hospital with a medical history of an aortic arch and a descending thoracic aneurysm. To prevent arm ischaemia after the frozen elephant trunk procedure, a left subclavian artery to left common carotid artery anastomosis was performed. The postoperative period was complicated by sepsissternal infection and mediastinitis. We decided to perform a combined total aortic root and aortic arch replacement with 3 cryopreserved homografts. In addition, the "reverse arch technique" was applied to adapt the distal anastomosis. The operation is associated with a high risk of

Acute type A aortic dissection surgery in retrosternal gastric tube reconstruction patient.

BACKGROUND: Retrosternal gastric tube reconstruction is a common surgical technique for managing esophageal cancer, but it complicates acute type A aortic dissection repair and raises concerns about gastric tube damage. CASE PRESENTATION: A 73-year-old female, who underwent esophagectomy with retrosternal gastric tube reconstruction 6 months ago for esophageal cancer, presented with severe chest pain. Acute type A aortic dissection was confirmed by contrast-enhanced computed tomography, and emergency hemiarch replacement through a median sternotomy was performed, preserving the gastric tube w

A swollen face and neck after dental surgery: Think of subcutaneous emphysema and pneumomediastinum.

Pneumomediastinum and subcutaneous emphysema are defined as the pathological presence of free air in the mediastinum or subcutaneous tissue, respectively. In the majority of cases, pneumomediastinum is secondary to an iatrogenic cause, but has rarely been described after a routine dental extraction. This condition is generally self-limiting, but major complications can occur, such as mediastinitis, which is more frequently associated with iatrogenic pneumomediastinum. To highlight the importance of including this presumably underdiagnosed complication in the differential diagnosis, we present

Mycotic aneurysm of the distal thoracic aorta after botulinum toxin injection for esophageal dysmotility.

Endoscopic injection of botulinum toxin is a common method to treat esophageal dysmotility and achalasia. Patients undergoing this procedure who subsequently present with abdominal or back pain and constitutional symptoms should be evaluated for possible complications of the procedure, including occult esophageal perforation, mediastinitis, and mycotic aneurysm of the thoracic aorta. The case described herein illustrates the importance of serial imaging in a patient with persistent symptoms after botulinum toxin injection to identify and to treat occult aortic inoculation leading to mycotic an

A Technique for 360-Degree Rigid Sternal Fixation with Sternal Plates and Wire Cerclage.

Sternal instability after cardiac surgery can lead to poor bony healing, as well as deep sternal wound infections and mediastinitis. Rigid plate fixation is associated with greater stability and fewer complications compared with wire cerclage, however, rigid plate fixation alone lacks posterior stability of the sternum and may be less effective in morbidly obese or osteoporotic patients. This article describes a surgical technique of combined rigid plate fixation and wire cerclage that provides 360-degree stabilization for sternotomies in high-risk patients. We employed this technique in 40 pa

Tracheostomy Exchange Resulting in Rare Combination of Pneumomediastinum, Pneumothorax, Massive Pneumoperitoneum, and Subcutaneous Emphysema.

Tracheostomy tube change is a relatively common and simple procedure once a tract is formed between the skin and the trachea. Regular tracheostomy tube changes decrease the risk of postoperative pulmonary infection and granulation tissue formation. However, serious complications, such as loss of airway, subcutaneous emphysema, and mediastinitis, can occur if the tube exchange is performed inappropriately. We present a rare association of subcutaneous emphysema, tension pneumothorax, pneumomediastinum, and pneumoperitoneum following a tracheostomy tube exchange in a 56-year-old patient who had

Pulmonary patch repair of tracheobronchial necrosis with perforation secondary to caustic ingestion.

Corrosive aerodigestive injury complicated by tracheobronchial necrosis is a rare complication after caustic ingestion that carries a high risk of mortality and morbidity. If left untreated, tracheobronchial necrosis results in perforation, mediastinitis, and death. Pulmonary patch repair is a safe, effective, and easily performed surgical technique for the treatment of tracheobronchial necrosis with perforation. Frequent endoscopic surveillance leads to early diagnosis and prompt operative management to prevent significant morbidity. We describe a pulmonary patch repair of a patient who had s

Aortic pseudoaneurysm due to graft infection after invasive thymoma resection.

A 69-year-old woman with invasive thymoma underwent chemoradiotherapy followed by surgical resection, including extended thymectomy, right upper lobectomy, and innominate vein and superior vena cava reconstruction with a polytetrafluoroethylene graft. Computed tomography on the 27th postoperative day revealed mediastinitis and right hemothorax; open debridement and evacuation were performed. Vacuum-assisted closure was initiated to heal the median incision. The wound gradually cleared, and the vacuum was terminated on the 150th postoperative day. On the 155th postoperative day, the patient suf

Application of radiographic images in diagnosis and treatment of deep neck infections with necrotizing fasciitis: a case report.

The advent and wide use of antibiotics have decreased the incidence of deep neck infection. When a deep neck infection does occur, however, it can be the cause of significant morbidity and death, resulting in airway obstruction, mediastinitis, pericarditis, epidural abscesses, and major vessel erosion. In our clinic, a patient with diffuse chronic osteomyelitis of mandible and fascial space abscess and necrotic fasciitis due to odontogenic infection at the time of first visit came. We successfully treated the patient by early diagnosis using contrast-enhanced CT and follow up dressing through

Esophageal rupture due to Sengstaken-Blakemore tube misplacement.

The author presents three cases of esophageal rupture during the treatment of massive esophageal variceal bleeding with Sengstaken-Blakemore (SB) tube. In each case, simple auscultation was used to guide SB tube insertion, with chest radiograph obtained only after complete inflation of the gastric balloon. Two patients died of hemorrhagic shock and one died of mediastinitis. The author suggests that confirmation of SB tube placement by auscultation alone may not be adequate. Routine chest radiographs should be obtained before and after full inflation of the gastric balloon to confirm tube posi

[Pectoralis major muscle island flaps. Personal experiences].

Fifty-six pectoralis major island flap procedures were carried out in 52 patients for the immediate reconstruction of defects after resection of head, neck or thoracic tumors. In 28 we used only a part of the muscle covered with meshgraft, in another 28 as a myocutaneous island flap. The complication rate was low: One partial flap necrosis, two bleedings postoperatively on the donor site, four fistulas (one required a second operation). One necrosis of ribs required the resection and covering with a latissimus dorsi flap. One patient died with mediastinitis. The functional aspects of the muscl

Thoracoscopic resection of a cystic seminoma of the mediastinum.

Mediastinal seminoma is an uncommon tumor that accounts for 25% of primary mediastinal germ cell tumors, which in turn comprise fewer than 5% of all germ cell tumors. Although CT normally shows a solid, lobulated tumor, mediastinal cystic seminoma has rarely been described. Here, we report a 24-year-old man who presented with a mediastinal cystic tumor that was resected after an 18-month delay via video-assisted thoracoscopic surgery while in the supine position; the procedure involved lifting the chest wall with a subcutaneous Kirschner wire. Pathological examination revealed a mediastinal cy
