Principles of fluid management and stewardship in septic shock: it is time to consider the four D's and the four phases of fluid therapy.

In patients with septic shock, the administration of fluids during initial hemodynamic resuscitation remains a major therapeutic challenge. We are faced with many open questions regarding the type, dose and timing of intravenous fluid administration. There are only four major indications for intravenous fluid administration: aside from resuscitation, intravenous fluids have many other uses including maintenance and replacement of total body water and electrolytes, as carriers for medications and for parenteral nutrition. In this paradigm-shifting review, we discuss different fluid management s

Multidisciplinary consensus: a practical guide for the integration of abiraterone into clinical practice.

Abiraterone improves survival, relieves pain, improves quality of life and extends time to prostate-specific antigen (PSA) progression in patients with metastatic castration-resistant prostate cancer (mCRPC). A consensus-based guide for using abiraterone in patients with mCRPC has been developed by Australian clinicians with expertise in prostate cancer, based on their experience and supported by published data. Recommendations were developed for eight key topics: abiraterone administration; steroid administration and duration of use; concomitant medications and drug interactions; timing of te

Clinical review: Functioning gonadotroph adenomas.

CONTEXT: Functioning gonadotroph adenomas (FGAs) are pituitary tumors secreting biologically active gonadotropins. The published literature includes only small case series or individual case reports. This review summarizes the published data on this rare entity and, based on them, suggests guidance on the follow-up of these patients. EVIDENCE ACQUISITION: A review of articles in English retrieved from the PubMed up to December 2013 was conducted. The following terms were used for the search: "functioning gonadotroph adenomas," "FSH secreting adenomas," "LH secreting adenomas," "gonadotroph ad

Intracranial hemorrhage surgery on patients on mechanical circulatory support: a case series.

BACKGROUND: Cardiac disease is the leading cause of death in the United States and late-stage heart failure is associated with a high level of morbidity and mortality. The ventricular assist devices and extracorporeal membrane oxygenators are the mainstay of mechanical circulatory support devices for the patients with extremely low cardiac output. However, they come at a price of significant risk factors, including intracranial hemorrhage. The incidence of intracranial hemorrhage on extracorporeal membrane oxygenators and on ventricular assist devices is 37% and 13% to 14%, respectively. METH

Current epidemiology and management of acute heart failure: the multicenter prospective OFICA2 cohort.

AIMS: Acute heart failure (AHF) represents a major cause of morbi-mortality. In the last decade, the management of HF has changed, but up-to-date real-life data are scarce. The aim of our study was to describe contemporary data about AHF patients' medical pathway, management and to identify predictors of in-hospital outcomes. METHODS AND RESULTS: OFICA2 is a French prospective multicenter cohort led from March to April 2021 in 80 participating centres. In-hospital outcomes were defined as a composite of all-cause death, cardiogenic shock or transfer to intensive care unit. A total of 1513 con

Integrating social and neurobehavioral understandings of epilepsy stigma into personalized care strategies.

There is a bidirectional relationship between epilepsy-related stigma (ERS) and medical outcomes. ERS negatively influences treatment adherence and disease management, whereas insufficient medical improvement can, in turn, exacerbate ERS. ERS remains a significant challenge, often leading to delays in diagnosis, limiting appropriate therapies, and creating difficulties in social integration for patients with epilepsy. Even when treatment successfully achieves seizure cessation, failure to address ERS can limit patient social integration, reduce quality of life, and hamper long-term psychosocia

Clinical practice patterns among older multimorbid adults presenting with suspected ischemic symptoms: A multi-center survey.

BACKGROUND: When patients with suspected or known coronary artery disease (CAD) present with new or worsening ischemic symptoms, initial referral to imaging or optimization of guideline directed medical therapy (GDMT) with deferral of testing are both acceptable management approaches. METHODS: In this 12-center study, a 19-item survey exploring preferred management strategy for symptomatic older adults (≥75 years) with or without known CAD, and major patient and clinical factors driving this decision making was administered to clinicians. RESULTS: There were 96 respondents (70.8 % cardiologi

Acute severe ulcerative colitis: using JAK-STAT inhibitors for improved clinical outcomes.

Acute Severe Ulcerative Colitis (ASUC) is a well-known and potentially fatal disease state, characterized by symptoms of systemic toxicity including fever, severe anemia, elevated inflammatory markers, and autonomic instability. The life-threatening nature of this condition requires clinicians to make prompt diagnoses and take rapid action, either directing patients towards surgical interventions or medical management. Failure to treat ASUC may lead to toxic dilation of the colon, hemorrhage, or sepsis. Current algorithms suggest the use of intravenous (IV) corticosteroids upon diagnosis, with

American College of Rheumatology White Paper on Antimalarial Cardiac Toxicity.

Hydroxychloroquine (HCQ) and chloroquine (CQ) are well-established medications used in treating systemic lupus erythematosus and rheumatoid arthritis, as well as skin conditions such as cutaneous lupus erythematosus. In rare cases, arrhythmias and conduction system abnormalities, as well as cardiomyopathy, have been reported in association with HCQ/CQ use. Recently, however, the corrected QT interval (QTc)-prolonging potential of these medications, and risk of torsade de pointes (TdP) in particular, have been highlighted in the setting of their experimental use for COVID-19 infection. [Recent advances in the management of iron deficiency and iron deficiency anemia during pregnancy].

Iron deficiency (ID) and iron deficiency anemia (IDA) are common obstetric complications associated with various adverse pregnancy outcomes. According to the latest evidence-based medical evidence, this article reviews the management strategies for ID and IDA during pregnancy. Intravenous iron therapy provides substantial advantages for patients who cannot tolerate or do not respond to oral iron, as well as for those with severe IDA requiring rapid correction. The application of new-generation intravenous iron agents has improved treatment efficiency; however, their safety in the first trimest

Advances in the Treatment of Ulcerative Colitis-From Conventional Therapies to Targeted Biologics and Small Molecules.

The goals of ulcerative colitis (UC) treatment are focused on improving quality of life, achieving steroid-free remission, and minimizing the risk of cancer. In UC traditional management, a step-up strategy involves introducing increasingly more immunosuppressive medications, thus avoiding unnecessary overexposure to more potent drugs. However, in cases of severe, acute UC, priority is rapid and effective treatment to minimize the risk of complications such as bleeding, intestinal perforation, toxic megacolon or the need for colectomy. Modern approach to UC management shifts to an "accelerated

Biologics in Chronic Rhinosinusitis with Nasal Polyps: The Otolaryngologist's Perspective.

PURPOSE OF REVIEW: Monoclonal antibodies ("biologics") have increasingly been used to manage chronic sinusitis with nasal polyps (CRSwNP). The aim of this manuscript was to compare biologic outcomes to traditional management of CRSwNP with endoscopic sinus surgery (ESS). RECENT FINDINGS: There are now four approved biologics for the treatment of CRSwNP in the United States: dupilumab, mepolizumab, omalizumab, and tezepelumab. Cost analysis and comparison between phase 3 clinical trial results of biologics and ESS show ESS is at least as effective as biologics in controlling symptoms with sign

International Clinical Practice Guideline Recommendations for Acute Pulmonary Embolism: Harmony, Dissonance, and Silence.

Despite abundant clinical innovation and burgeoning scientific investigation, pulmonary embolism (PE) has continued to pose a diagnostic and management challenge worldwide. Aging populations, patients living with a mounting number of chronic medical conditions, particularly cancer, and increasingly prevalent health care disparities herald a growing burden of PE. In the meantime, navigating expanding strategies for immediate and long-term anticoagulation, as well as advanced therapies, including catheter-based interventions for patients with more severe PE, has become progressively daunting.
