[Recurrent acute rhombencephalomyelitis in an adult or neuromyelitis optica? Presentation of a case].

INTRODUCTION: The lack of accepted homogeneous criteria for the definition of some demyelinating diseases makes diagnostic characterization difficult and limits data interpretation and therapeutic recommendations. Recurrent encephalomyelitis (ADE-R) along with borderline cases of neuromyelitis optica (NMO) are especially controversial. OBJECTIVE: To describe the clinical and radiological evolution of an adult-onset ADE-R versus NMO case throughout 9 years of follow-up. PATIENT AND METHODS: Our patient presented with severe symptoms of rhombencephalomyelitis and the cranial and spinal magneti

Coexistence of adult-onset Still's disease and autoimmune hyperthyroidism in a patient who responded to corticosteroids and β-blocker.

The pathogenesis of adult-onset Still's disease (AOSD), which is currently thought to be an autoimmune disorder, may share similarities with autoimmune hyperthyroidism. This report describes a middle-aged woman in whom hyperthyroidism and Still's disease developed concurrently. During the course of her illness, the hyperthyroidism was observed to be aggravated whenever her AOSD was in the active stage. After her AOSD activity was controlled, her hyperthyroidism improved clinically. The extent of activation of her hyperthyroidism was observed in parallel to the extent of activation of her AOSD.

A case of chronic progressive myelopathy.

Acute myelitis and optic neuritis are the main clinical features of patients with neuromyelitis optica (NMO), which usually appears as a relapsing-remitting course of disease that worsens over days and improves over weeks. We present a patient with chronic progressive myelitis over a 4-month period without remission as having a limited form of NMO that improved after plasmapheresis. Plasmapheresis may benefit patients with chronic progressive myelitis, which may be a manifestation of NMO, as well as those with a relapsing-remitting course of NMO.

[Autopsy case of corticosteroid-induced invasive pulmonary aspergillosis with cavity-formation].

Invasive pulmonary aspergillosis (IPA) is an important and fatal complication in the patients with neutropenic or immunosuppressed condition. In spite of intensive treatment with anti-fungal drugs, the prognosis of the patients who have been suffered from IPA is extremely poor. This case was an 85-year-old Japanese man who was diagnosed as idiopathic thrombocytopenic purpura (ITP). He underwent high-dose corticosteroids and gamma-globulin therapy. During the hospitalization, he complained respiratory symptoms, and the abnormal shadow suggesting pulmonary infiltration and cavitation was pointed

Retinal involvement in patients with juvenile dermatomyositis.

The authors describe the clinical presentations and ophthalmic findings of two patients with juvenile dermatomyositis. The results of their dilated eye examinations proved to influence the treatment of the disease process because retinal pathology was used as a factor to escalate the degree of anti-inflammatory therapy. Therefore, an initial ophthalmic examination may be considered in patients with new-onset juvenile dermatomyositis.

Timing of corticosteroid therapy is critical to prevent retinal ganglion cell loss in experimental optic neuritis.

PURPOSE: Acute vision loss from optic neuritis typically resolves; however, recovery is often not complete. Permanent vision loss from retinal ganglion cell (RGC) death occurs in 40% to 60% of patients. Current therapy (high-dose corticosteroids) speeds recovery but does not change final visual outcomes. Here the authors examined whether corticosteroids administered early in the disease course can prevent RGC loss in experimental optic neuritis. METHODS: RGCs were retrogradely labeled with fluorogold in SJL/J mice. Experimental autoimmune encephalomyelitis (EAE) was induced by immunization wi

[Interstitial pneumonia complicating amyopathic dermatomyositis: a case report].

Amyopathic dermatomyositis (ADM) is a clinical subtype of dermatomyositis, characterized by the absence of motor weakness and the presence of normal muscle enzyme levels. ADM is sometimes accompanied by neoplasm or interstitial pneumonia that shows a rapid progressive course both of them are associated with a poor prognosis. A 56-year-old woman with no medical history was referred to the department of medicine because of arthralgia with a remarkable weight loss. She also complained of rapidly progressive dyspnea, cough and photosensitivity. Physical examination on admission showed scaly erythe

Multifocal Langerhans' cell histiocytosis: a case report.

OBJECTIVES: Langerhans' cell histiocytosis (LCH) is a rare disorder of unknown aetiology, characterised by the proliferation of histiocytic cells in various tissues and organs. The role of the otolaryngologist is important in the early and accurate evaluation, staging and diagnosis of LCH, as it may mimic more common otological disorders. METHOD: We report the case of a 2-year-old child presenting with LCH and review the literature concerning this rare disease. RESULTS: Presentation in our case was with a common aural polyp, refractory to medical treatment, which when biopsied, revealed LCH.

[Two cases of acute respiratory failure due to idiopathic interstitial pneumonia successfully treated with sivelestat sodium hydrate and steroid therapy].

We encountered two patients with clinically diagnosed idiopathic interstitial pneumonia with acute respiratory distress syndrome (ARDS). Both patients required mechanical ventilation within 24 hours after admission. We managed these patients using lower tidal volume ventilation (tidal volume: 5-6 ml/kg), antibiotics, sivelestat sodium hydrate and steroid pulse therapy followed by oral prednisolone therapy. Respiratory failure was relieved within 2 weeks following treatment There was no four-fold rise in screen viral titers and screening investigations of autoantibodies were negative. Based on

[Differential diagnosis of tuberculosis of respiratory organs and pulmonary eosinophilia].

The reasons for misdiagnosis of tuberculosis in patients with pulmonary eosinophilia were analyzed, by using 6 clinical cases as an example. In most cases, the reasons for diagnostic errors are underestimation of elevated eosinophil levels and late afterexamination to verify the diagnosis. With general practitioners' increasing alertness, the inefficiency of antibiotics in patients with initially presumed pneumonia is frequently interpreted as an argument in favor of tuberculosis. Examining the cellular composition of bronchoalveolar lavage permits verification of the diagnosis of pulmonary eo

[The influence of glucocorticoids on the view of chicken's inner ear damage after exposure to wide-band-noise].

The aim of the study was to assess the influence of glucocorticoids on the view of hair cell regeneration process being in the chicken's inner ear (basilar papilla - BP) after exposure to wide-band noise at the level 120 dB (A) for 48 hours. We found that glucocorticoids given during and/or after exposure to the noise have a cytoprotective activity to the hair cells, they limitate the extensiveness and decrease the dynamics of hair cells injury. We observed that new "young" hair cells reappeared at the sensory epithelium on the 7th day after the end of exposure. Regenerated hair cells have imm
