Exploring the relationships between multisite musculoskeletal pain, pain characteristics, and physical function: a cross-sectional analysis.

OBJECTIVE: Multisite musculoskeletal pain is highly prevalent and negatively impacts physical function through several potential mechanisms. However, it remains unclear how the number of affected pain regions relates to function. This study explored associations between different aspects of pain (global pain intensity, catastrophizing, kinesiophobia, pain interference) and physical function in individuals with multisite pain. METHODS: This cross-sectional analysis included 101 participants aged 18-85 years. Multisite pain was assessed across eight anatomic regions, using binary questions, wit

Influence of Refraction on Widefield Retinal and Choroidal Vascular Metrics in Optical Coherence Tomography Angiography in a Caucasian Population.

PURPOSE: Refraction-related differences in retinal and choroidal vascular metrics have been reported using optical coherence tomography angiography (OCTA), primarily within the central fundus. This study aimed to establish normative vascular reference values for the retina and choroid using widefield swept-source OCTA (WF-SS-OCTA), stratified by spherical equivalent (SE), to characterize associations between refractive status and OCTA-derived vascular measurements extending beyond the central fundus. METHODS: In this prospective single-center study, healthy volunteers underwent WF-SS-OCTA at

Etiologic heterogeneity across oral cavity cancers.

BACKGROUND: Oral cavity cancers are generally considered etiologically similar, with a majority (>75%) attributable to smoking/alcohol. Yet, incidence trends for oral cavity cancers have recently diverged in the United States and several European countries (with declines for most oral cavity cancers but increases for oral tongue cancers), suggesting etiologic differences. This study quantified etiologic heterogeneity across oral cavity cancers using data from the NIH-AARP Diet and Health Study. METHODS: Cox regression models were used to quantify associations (hazard ratios [HRs] and populati

[18F]AlF-NOTA-PCP2 PET/CT to Assess PD-L1 Status in Head and Neck Cancer: Head-to-Head Comparison With [18F]FDG PET/CT.

PURPOSE: While PD-L1 expression heterogeneity limits the predictive value of localized biopsies, molecular imaging offers a dynamic alternative. We developed [18F]AlF-NOTA-PCP2, a hydrophilic PD-L1-targeted peptide tracer, and first evaluated its clinical utility in head and neck (HNC) cancer through a head-to-head comparison with [18F]FDG PET/CT. METHODS: Preclinical validation included competitive binding assays and micro-PET/CT in PD-L1-positive HNC xenografts. A prospective trial (NCT04304066) enrolled 24 HNC patients for [18F]AlF-NOTA-PCP2 PET/CT, with paired [18F]FDG scans in 17 patient

The Genetics of Cerebellar Structure and Associations With Cognitive Performance: A Twin Magnetic Resonance Imaging Study.

The cerebellum, traditionally associated with motor control, is increasingly recognized for its involvement in higher-order cognitive functions. However, the role of cerebellar subregions in cognition remains underexplored, as are the roles of genetic factors on cerebellar structure and brain-behavioral associations. The primary goal of this study was to investigate the relationship between cerebellar subregion volumes and cognitive performance. A secondary aim was to quantify the genetic contributions to cerebellar structure and determine the degree to which any brain-behavior associations we

Higher postoperative gabapentin doses are associated with pulmonary complications after total shoulder arthroplasty.

BACKGROUND: Gabapentinoids are increasingly being used as adjunctive analgesic medications after total shoulder arthroplasty (TSA). However, data suggest that when used in conjunction with opioids, gabapentinoids may induce sedation and possibly potentiate respiratory depression. The aim of this study was to evaluate the impact of gabapentin postoperative pulmonary complications and total opioid utilization after primary TSA. METHODS: Patients who underwent primary, elective anatomic and reverse TSA from January 1, 2016, to December 31, 2020, were identified using the Premier Healthcare Datab

Patient-related prognostic factors in older adults with head and neck cancer: The EGeSOR clinical trial.

BACKGROUND: Due to its anatomic site, the treatment of head and neck cancer (HNC) significantly affects activities of daily living. Clinical heterogeneity and accelerated aging in older adults make it crucial to identify patient-related, prognostic factors. The objective of the present study was to assess the prognostic value of patient-related factors for 2-year overall survival (OS) in older adults with HNC, after adjustment for tumor-related factors. METHODS: This was a secondary analysis of the EGeSOR randomized, controlled trial conducted in France between 2013 and 2018. Patients with HN

Health care system factors associated with receipt of treatment and treatment intent in stage III non-small cell lung cancer: A population-based study in Ontario.

PURPOSE: Stage III non-small cell lung cancer (NSCLC) is a heterogeneous disease, with a spectrum of anatomic extent, health status, and treatment approaches. Receipt of treatment and its intent should be independent of health system factors where care quality is optimal. We investigated the degree that modifiable health system factors are associated with receipt of treatment and treatment intent in stage III NSCLC in a large, universal health system. METHODS: This was a population-based, retrospective cohort study with health administrative data from Ontario, Canada, 2010-2018 for those aged

Successful Cardiac Resynchronization Therapy Reduces Negative Septal Work in Patient-Specific Models of Dyssynchronous Heart Failure.

In patients with dyssynchronous heart failure (DHF), cardiac conduction abnormalities cause the regional distribution of myocardial work to be non-homogeneous. Cardiac resynchronization therapy (CRT) using an implantable, programmed biventricular pacemaker/defibrillator, can improve the synchrony of contraction between the right and left ventricles in DHF, resulting in reduced morbidity and mortality and increased quality of life. Since regional work depends on wall stress, which cannot be measured in patients, we used computational methods to investigate regional work distributions and their

Therapeutic delivery of CCL2 modulates immune response and restores host-microbe homeostasis.

Many chronic inflammatory diseases are attributed to disturbances in host-microbe interactions, which drive immune-mediated tissue damage. Depending on the anatomic setting, a chronic inflammatory disease can exert unique local and systemic influences, which provide an exceptional opportunity for understanding disease mechanism and testing therapeutic interventions. The oral cavity is an easily accessible environment that allows for protective interventions aiming at modulating the immune response to control disease processes driven by a breakdown of host-microbe homeostasis. Tailoring left ventricular assist device cannula implantation using coupled multi-scale multi-objective optimization.

BACKGROUND: The frequent occurrence of thromboembolic cerebral events continues to limit the widespread implementation of Ventricular Assist Devices (VAD) despite continued advancements in VAD design and anti-coagulation treatments. Recent studies point to the optimal positioning of the outflow graft (OG) as a potential mitigator of post implantation thromboembolism. OBJECTIVE: This study aims to examine the tailoring of the OG implantation orientation with the goal of minimizing the number of thrombi reaching the cerebral vessels by means of a formal shape optimization scheme incorporated in

Thrombosis in the pathogenesis of abdominal aortic aneurysm.

BACKGROUND: Abdominal aortic aneurysms (AAAs) are a relatively common vascular pathology of the elderly with high morbidity potential. Irreversible degeneration of the aortic wall leads to lethal rupture if left untreated. Nearly all AAAs contain intraluminal thrombus (ILT) to a varying degree, yet the mechanisms explaining how thrombosis is disturbed in AAA are relatively unknown. This review examined the thrombotic complications associated with AAA, the impact of thrombosis on AAA surgical outcomes and AAA pathogenesis, and the use of antithrombotic therapy in the management of this disease.

A Reusable, Low-Cost Model to Improve Resident Confidence in Performance of Postpartum Tubal Ligation.

Proficiency in postpartum abdominal sterilization is a core objective that residents must acquire during residency as outlined by the Council on Resident Education in Obstetrics and Gynecology. Surgical sterilization remains a popular and effective form of contraception in the United States; however, many women requesting immediate postpartum tubal ligation are unable to have the procedure given institutional barriers and variations in practice. Additionally, increasing use of long-term reversible contraceptives has contributed to declining immediate postpartum sterilization rates, which may u

A self calibrating, magnetic sensor approach accurately positions an aortic damage control stent in a porcine model.

OBJECTIVES: Non-compressible torso hemorrhage remains a high mortality injury, with difficulty mobilizing resources before exsanguination. Previous studies reported on a retrievable stent graft for damage control and morphometric algorithms for rapid placement, yet fluoroscopy is impractical for the austere environment. We hypothesized that magnetic sensors could be used to position stents relative to an external magnet placed on an anatomic landmark, whereas an electromagnet would allow self-calibration to account for environmental noise. METHODS: A magnetic sensor alone (MSA) and with integ

Objective documentation of hypospadias anatomy with three-dimensional scanning.

INTRODUCTION: The absence of a standardized classification of hypospadias hinders understanding of the anatomic differences among patients and the evaluation of outcomes following surgical repair. In working towards a standardized, objective method of recording patients' hypospadias anatomy, we describe our initial experience using a non-invasive three-dimensional scanner. MATERIAL AND METHODS: An Artec3D Space Spider scanner was used to obtain 3D scans in 29 patients undergoing hypospadias repair. Variability in the Arterial Supply of Intracranial Meningiomas: An Anatomic Study.

BACKGROUND AND OBJECTIVES: Intracranial meningiomas are a diverse group of tumors, which vary by grade, genetic composition, location, and vasculature. Expanding the understanding of the supply of skull base (SBMs) and non-skull base meningiomas (NSBMs) will serve to further inform resection strategies. We sought to delineate the vascular supply of a series of intracranial meningiomas by tumor location. METHODS: A retrospective study of intracranial meningiomas that were studied using preoperative digital subtraction angiograms before surgical resection at a tertiary referral center was perfo
