[How I explore…optic neuritis].

Optic neuritis is an inflammation of the optic nerve most often caused by autoimmune or systemic disease. The diagnostic management is transdisciplinary. The clinical approach should be global first and then system-oriented. It determines the choice of additional investigations whose purpose is the characterization of the type and the cause of the optic neuritis. The exploration initially involves neuroimaging techniques, visual electrophysiology and biological analyses. Optical coherence tomography is of considerable interest in specifying the differential diagnosis. Diagnostic accuracy influ

Protein Misfolding in the Pathogenesis and Diagnosis of Neurodegenerative Diseases (Review).

This review systematizes existing data on protein misfolding in the pathogenesis of neurodegenerative diseases (with a focus on α-synuclein, β-amyloid, and tau protein). Modern laboratory and neuroimaging methods used for clinical diagnosis and scientific research of proteinopathies are discussed. The paper describes promising protein amplification techniques that enable the detection of ultra-low concentrations of aberrant protein forms in biological fluids. The challenges and prospects of early diagnosis of neurodegenerative diseases through protein misfolding detection are also shown.

Posterior Reversible Encephalopathy Syndrome in a Pediatric Patient with End-Stage Renal Disease.

Posterior reversible encephalopathy syndrome (PRES) is a clinical and neuroimaging syndrome that can affect both children and adults and has variable etiology. It is clinically defined by headaches, consciousness disorders, seizures and visual disturbances. Early recognition (clinical and imaging) can lead to appropriate general measures to correct the underlying cause of PRES. In this paper, we report a case of PRES in an eight-year-old boy with bilateral renal hypoplasia and end-stage renal disease (ESRD).

Macrocephaly in the Primary Care Provider's Office.

Macrocephaly is commonly encountered in the primary care provider's office. It is defined as an occipitofrontal circumference that is greater than 2 standard deviations above the mean for the child's given age. Macrocephaly is a nonspecific clinical finding that may be benign or require further evaluation. An algorithmic approach is useful for aiding in the clinical decision-making process to determine if further evaluation with neuroimaging is warranted. Abnormal findings may signify a harmful underlying cause, requiring referral to a genetic specialist or neurosurgeon.

[Hemiconvulsion-hemiplegia-epilepsy syndrome].

Hemiconvulsion-hemiplegia-epilepsy syndrome is a rare consequence of a status epilepticus in the course of a febrile illness in children under the age of four years. Various degrees of hemiplegia and within a variable interval, subsequent epilepsia follows. Neuroimaging show unilateral cytotoxic oedema at the initial convulsive state, followed by severe chronic atrophy of the affected hemisphere. The aetiology remains unclear. Several mechanisms may contribute to this condition. To improve the outcome, further studies are needed and early diagnosis is essential.

[Congenital nonpulsatile midline nasal masses].

These tumours are rare, benign abnormalities including dermoids, gliomas and encephaloceles that result from aberrant embryologic development. They can cause severe deformity of the midface and nasal structures and may have an intracranial extension that requires neurosurgical consultation. Thus preoperative manipulations, i.e. biopsies, are contraindicated as it can lead to cerebrospinal fluid leak and meningitis. The treatment is surgical excision and should be performed early. Neuroimaging is essential in the evaluation of specific type, presence of intracranial extension and presurgical pl

IgG4-related inflammatory pseudotumors mimicking multiple meningiomas.

IgG4-related disease is an emerging clinicopathologic entity. Hypophysitis, diffuse thickening of dura, and enlargement of the trigeminal nerve are well-known intracranial involvements of IgG4-related disease. This report of a case of systemic IgG4-related disease is the first to present neuroimaging of apparent supratentorial meningioma-like lesions and thickening and contrast enhancement of the walls of the intracranial internal carotid arteries. It is important to recognize IgG4-related intracranial pseudotumors so that patients do not undergo unnecessary surgical procedures.

Cerebral microbleeds: a new dilemma in stroke medicine.

Cerebral microbleeds (CMBs) are an increasingly common neuroimaging finding in the context of ageing, cerebrovascular disease and dementia, with potentially important clinical relevance. Perhaps the most pressing clinical question is whether CMBs are associated with a clinically important increase in the risk of intracerebral haemorrhage (ICH), the most feared complication in patients treated with thrombolytic or antithrombotic (antiplatelet and anticoagulant) drugs. This review will summarize the evidence available regarding CMBs as an indicator of future ICH risk in stroke medicine clinical

Abducens nerve palsy associated with a clival epidural hematoma.

A clival epidural hematoma is a rare lesion that usually develops after a hyperflexion or hyperextension injury of the neck, often in a child. A 5-year-old girl presented after a motor vehicle accident with multiple cranial neuropathies, including bilateral abducens nerve pareses and right facial, glossopharyngeal, and hypoglossal cranial nerve palsies. Neuroimaging identified a clival epidural hematoma. The child was observed and the hematoma resolved. The abducens nerve palsies resolved during the ensuing 14 months.

Unrelated umbilical cord blood transplant for juvenile metachromatic leukodystrophy: a 5-year follow-up in three affected siblings.

Unrelated umbilical cord blood transplantation (UCBT) was used to treat three siblings with juvenile metachromatic leukodystrophy (jMLD). The efficacy of this therapy was measured over a 5-year period with serial neurological examinations, neuroimaging, nerve conduction studies (NCS), and neuropsychological evaluations (NPE). Outcomes were a function of disease stage at time of UCBT with alteration of disease course occurring in the first 2 years after UCBT and then subsequent halting of progression and stabilization of symptoms and disease.

Sensitivity of the brain to loss aversion during risky gambles.

Little is known about the neural systems that subserve human loss aversion. A recent neuroimaging study by Tom, Poldrack and colleagues reports that this pattern of behaviour is directly tied to the greater sensitivity of the brain to potential losses compared with potential gains and uncovers a brain network whose activity increases with potential gains and decreases with potential losses. These results challenge the common view that loss aversion engages a distinct emotion-related brain network (e.g. amygdala and insula).

Extramedullary plasmacytoma extensively affecting the sella turcica and paranasal sinuses.

We report a 62-year-old Japanese male who complained of double vision and showed clear boundary mass extending to the clivus, intrasella, suprasella, ethmoidal sinus and sphenoid sinus on neuroimaging. The tumor mass was partially resected via transsphenoidal approach and was diagnosed as the extramedullary plasmacytoma by IgA immunostaining and electron microscopy. Making diagnosis from the imaging findings was difficult in this rare case, but immunohistological and electron microscopic examinations were useful for pathological diagnosis.

Primary congenital upper eyelid retraction in infants and children.

Four infants with unilateral or bilateral eyelid retraction were examined. A full clinical evaluation revealed no ocular or systemic pathology. In all cases, clinical examination, thyroid function tests, and neuroimaging of the brain and orbits revealed no underlying disease process. The eyelid retraction did not change over the follow-up period of 2 to 6 years. Unilateral or bilateral eyelid retraction in infants and children may be an isolated clinical entity without an associated underlying disease.

Dystonia: lessons from brain mapping.

Functional neuroimaging with positron emission tomography, single photon emission computer tomography, magnetic resonance imaging, and magnetoencephalography have provided powerful tools to elucidate anatomo-functional impairment underlying movement disorders such as dystonic movements. They have revealed that presymptomatic cerebral abnormalities may be a common feature in dystonia whatever the clinical status. Techniques using specific markers have recently focused on the type of receptors that may be dysactivated and on the kind of neurotransmitter that may be dysregulated in dystonia.

Encephalocraniocutaneous lipomatosis: complete neuroradiologic evaluation and follow-up of two cases.

Encephalocraniocutaneous lipomatosis (ECCL) is a rare neurocutaneous syndrome characterized by unilateral scalp, facial, and ocular lesions and ipsilateral cerebral malformations. To define the neuroimaging features of this disorder we studied two patients affected by ECCL and compared our data with those reported in the literature. Sonographic, CT, and MR imaging examinations showed quite specific CNS findings that are highly suggestive of the diagnosis of ECCL. To our knowledge this is the first report of a complete neuroradiologic evaluation and follow-up of this disorder.

Visual loss with Langerhans cell histiocytosis: multifocal central nervous system involvement.

A 42-year-old woman with a 6-year history of diabetes insipidus and progressive hypersomnolence presented with visual loss. Neuroimaging showed infiltration in the hypothalamus, the optic nerve, and the chiasm, as well as multiple lesions in other areas of the brain parenchyma. Biopsy showed Langerhans cell histiocytosis. This is an unusual presentation of Langerhans cell histiocytosis, involving the visual pathways without manifestations outside of the central nervous system. The differential diagnosis and the magnetic resonance imaging findings will be discussed.

Periventricular nodular heterotopia and childhood absence epilepsy.

A young female presented with an epileptic syndrome resembling childhood absence epilepsy, a normal neurologic examination, generalized 3-Hz spike-and-wave discharges, and clinical absences. Her seizures responded to treatment with valproic acid. Other abnormalities in her electroencephalogram prompted neuroimaging studies, which demonstrated periventricular nodular heterotopia. Review of published reports confirmed this presentation to be atypical of this developmental lesion. The authors describe their patient and discuss this unexpected association and the relevant reports briefly.

Dopamine, the prefrontal cortex and schizophrenia.

Dysfunction of the prefrontal cortex (PFC) in schizophrenia has been suspected based on observations from clinical, neuropsychological and neuroimaging studies. Since the PFC receives a dense dopaminergic innervation, abnormalities of the mesocortical dopamine system have been proposed to contribute to the pathophysiology of schizophrenia. In this review, aspects of the anatomy, physiology and pharmacology of the mesencephalic-frontal cortical dopamine system as they may relate to schizophrenia are described, and evidence for altered dopaminergic neurotransmission in the frontal cortex of schi

Mesenchymal chondrosarcoma of the cauda equina.

Primary central nervous system mesenchymal chondrosarcomas are extremely rare tumors composed of primitive mesenchymal cells punctuated by islands of cartilage in various stages of differentiation. Although there are isolated reports of tumors arising in the spinal dura, to our knowledge extraskeletal mesenchymal chondrosarcoma localized to the cauda equina has not been reported in the literature. We present an unusual case of a mesenchymal chondrosarcoma involving the nerve roots of the cauda equina. The clinical, pathological and neuroimaging features are reviewed and discussed.
