Lutetium-177 Radiopharmeceutical Therapy Extravasation Lessons Learned.

Lutetium-177 ( 177 Lu) dotatate has been offered at Fox Chase Cancer Center since 2017 as part of a clinical trial and then in 2018 as a commercially available cancer therapy, and we thought we were prepared for most 177 Lu issues by the fall of 2020. A single phone call identified that the Radiation Safety Department had not been prepared to address extravasations. Fortunately for the patient and Radiation Safety, the 177 Lu therapies are administered by an infusion nurse and Fox Chase Cancer Center has a robust infusion center. The expertise of our infusion center team helped to quickly iden

Cannabis Use and Bleomycin: An Overview and Case Study of Pulmonary Toxicity.

BACKGROUND: Legalization efforts in many states have heightened awareness of the medicinal uses of cannabis, and oncology nurses are more frequently caring for patients who have used or are using cannabis. Significant epidemiologic data on the prevalence of cannabis use in patients with cancer are not yet available, and not much is known about the effects of cannabis on cancer treatment. OBJECTIVES: This article describes the effects cannabis may have on the lungs, reviews indications for cannabis use in patients with cancer, and explores an atypical case of progressive pulmonary toxicity in

An Eye on Vision: Seven Questions About Vision Screening and Eye Health-Part 4.

Current evidence-based and best-practice vision screening and eye health approaches, tools, and procedures are the result of revised national guidelines in the past 3 years and advances in research during the past 18 years. To help the busy school nurse with little time to keep up with changes in children's vision practices and a growing body of literature, the National Center for Children's Vision and Eye Health at Prevent Blindness is providing answers to seven questions that are often received from the field. Topical areas are (1) instrument-based screening and stereopsis, (2) optotype-base

[Clinical Application of Basic Research on Mohs' Paste to Meet Medical Needs].

Mohs' paste (MP), which is widely used in medical services as a specific hospital preparation, has been considered to have demerits, such as increased hardness after preparation and marked adhesiveness. However, factors associated with variations in its physical properties have not yet been clarified. Therefore, we conducted studies to clarify the physicochemical phenomena influencing such variations, and also examined prescription drug designs of MP preparations that are difficult to use clinically due to the above-mentioned demerits, with a view to improving their usability. Furthermore, wit

Community-Based Care of the Fibromyalgia Patient: Strategies to Promote Self-Management.

Along with a variety of distressing symptoms, a diagnosis of fibromyalgia (FM) brings with it substantial physical, psychosocial, and financial costs. Research shows that self-management is an effective means to manage FM and can lead to fewer healthcare visits. Yet due to the complexity of this disorder, healthcare providers need to be able to tailor treatment to individual patients by understanding effective treatment interventions. Home healthcare nurses (HHNs) are in a unique position to assess and implement effective treatment recommendations in the home setting and as such could consider

Jackfruit anaphylaxis in a latex allergic patient.

Several fruits have been reported to crossreact with latex antigen in latex allergy patients but little is known regarding tropical fruits in particular. Here we report the case of a 34-year old nurse who developed anaphylaxis following the ingestion of dried jackfruit (Artocarpus heterophyllus). The patient had a history of chronic eczema on both hands resulting from a regular wear of latex gloves. She and her family also had a history of atopy (allergic rhinitis and/or atopic dermatitis). The results of skin prick tests were positive for jackfruit, latex glove, kiwi and papaya, but the test

Towards a more comprehensive definition of shift work tolerance.

The aim of the present study was to define the nature of individual differences in shift work tolerance (SWT). This was investigated by an exploratory factor analysis of scores from a wide range of established instruments designed to measure sleep, sleepiness, fatigue, social functioning, as well as physical and mental health. Data were collected from a representative sample of 1,529 Norwegian nurses engaged in rotating shift work. The analyses yielded two factors that seemed to be especially relevant for SWT, namely "Well-being" and "Physical health". Both factors were related to several demo

Think before you flush! A sustainable aquatic eco-system's relation to human health.

What we do every day at work and in our home lives can make a difference in the quality of our environment. Consider, for example, the flushing of pharmaceuticals into the sewer system can lead to water pollution resulting in a threat to aquatic and human life. In contrast, keeping aquatic life healthy may contribute to human health. Some aquatic-based medications are currently on the market. Others are in various stages of development. In this article the authors argue that, for the benefit of both human and marine life, it is time to implement safer disposal methods for unwanted medications.

The inaccuracy of automatic devices taking postural measurements in the emergency department.

Automatic devices are used to take postural blood pressures in the emergency department despite research proving their inaccuracy in taking single blood pressures. This study assessed the accuracy of an automatic device compared with a manual aneroid reference standard for determining orthostatic hypotension and postural drops at triage. Supine and standing blood pressures were taken with an automatic and a manual device in a sequential and random order, and postural drops were calculated. The manual device indicated 10/150 emergency department patients had orthostatic hypotension (7%) and the

Urban sprawl and you: how sprawl adversely affects worker health.

Urban sprawl, once thought of as just an environmental issue, is currently gaining momentum as an emerging public health issue worthy of research and political attention. Characteristics seen in sprawling communities include increasing traffic volumes; inadequate public transportation; pedestrian unfriendly streets; and the division of businesses, shops, and homes. These characteristics can affect health in many ways. Greater air pollution contributes to higher asthma and other lung disorder rates. An increased dependence on the automobile encourages a more sedentary lifestyle and can potentia

Systemic Candida infections in patients with leukemia: an overview of drug therapy.

Systemic fungal infections are becoming increasingly common in patients with hematologic malignancies receiving antineoplastic therapy. The presence of acute myeloid or acute lymphoid leukemia, plus the use of chemotherapy to totally ablate malignant bone marrow cells, puts patients in a protracted neutropenic state. During this profound and prolonged neutropenic phase, patients receive antibiotic therapy for suspected or identified bacterial infections. However, when fever or other signs of infection continue despite antibiotic therapy, patients frequently need to be treated for suspected or

Physical growth, infant nutrition, breastfeeding, and general nutrition.

A constant in the practice of general pediatrics is an emphasis on infant nutrition and monitoring of growth. These topics are the focus of this issue's "Office pediatrics" section. Much of the new research and publication concerning nutrition in childhood in the past year continues to focus on breastfeeding-its short-term and long-term benefits and strategies to increase the number of women who successfully nurse their infants. The documentation of breastfeeding's benefits is becoming increasingly strong, even in developed countries such as the United States. At the same time, the many weakne

The findings of the Agency for Toxic Substances and Disease Registry Medical Waste Tracking Act report.

The Agency for Toxic Substances and Disease Registry (ATSDR) report "The Public Health Implications of Medical Waste: A Report to Congress" has been finalized and submitted to Congress. The report is a comprehensive review of all available data and information on the subject. Based on the data developed in the report, ATSDR concludes that the general public is not likely to be adversely affected by medical waste generated in the traditional health setting. However, the increase of in-home health care and other sources of nonregulated medical waste (e.g., intravenous drug users) provides opport
