Chlorimipramine therapy for obsessive-compulsive neurosis.

Twenty patients with a history of treatment-resistant obsessional neurosis underwent a 4-week clinical trial of chlorimipramine. Scores on the Psychiatric Questionnaire for Obsessive Compulsive Neurosis, the Leyton Obsessional Inventory, and 3 self-assessments indicated that although there was no significant change in obsessive ruminations, obsessive rituals, horrific temptations, or pervading doubt, there was a substantial significant improvement in the obsessive symptom item and severity of obsessions, as well as anxiety, depression, and phobia. Side effects were mild. The authors believe th

Dynamic and structural features of a case of compulsive neurosis as revealed in dreams.

Several examples of the manifest dreams reported by a patient with a compulsive neurosis are presented. An outstanding feature of the dreams was the sudden shift of the dreamer's self-representation from the role of criminal to the role of policeman. This shift is thought to parallel the oscillation between unconscious instinctual gratification and conscious attempts at reparation which is the main dynamic feature of the compulsive neurosis in waking life. The shift of orientation of the self reflects the operation of early defense mechanisms against the sadistic impulse. It also portrays grap

Validation of MMPI-2-RF Personality Disorder Spectra scales in a psychiatric sample.

The diagnostic operationalization of personality disorders is at the crossroads. The Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5), includes 2 separate systems for the diagnosis of personality disorders (PDs)-1 that is purely categorical (Section II) and 1 that is primarily dimensional (Section III). The International Classification of Diseases (11th ed.) will use a dimensional approach for the diagnosis of PDs. The Minnesota Multiphasic Personality Inventory 2-Restructured Form (MMPI-2-RF) PD Spectra scales were developed as measures of both the traditional and the al

Tourette Syndrome and Chronic Tic Disorders: The Clinical Spectrum Beyond Tics.

The clinical surveillance and active management of Tourette syndrome (TS) and other primary chronic tic disorders cannot be limited to tics, as these patients manifest a spectrum of sensory-, behavioral-, cognitive-, and sleep-related problems that have a major impact on their functioning and quality of life, influencing enormously clinical decision making on a routine basis. The sensory phenomena of primary tic disorders consist of premonitory urges and heightened sensitivity to external somatosensory and interoceptive stimuli. Recent evidence suggests that raised interoceptive awareness may

Scientific Evidence for the Evaluation of Neurological Soft Signs as Atypical Neurodevelopment Markers in Childhood Neuropsychiatric Disorders.

Motor dysfunction is commonly present in children with neurodevelopmental disorders. Developmental changes in voluntary control of motor skills include improvements in speed and motor coordination as well as reduced frequency of neurological soft signs (NSS) that are commonly observed in typically developing younger children. NSS are motor and sensory conditions that cannot be linked to specific cerebral lesions. The persistence of NSS into later childhood and adolescence is linked with an increased risk of psychiatric disorders. This finding gives support to the neurodevelopmental model of NS

Epidemiology and Comorbidities of Excoriation Disorder: A Retrospective Case-Control Study.

Excoriation disorder is a psychocutaneous disorder characterized by repetitive skin-picking and associated with significant morbidity. Currently, epidemiological data in patients with excoriation disorder are lacking so we sought to characterize common patient demographics and comorbidities. We conducted a retrospective case-control study comparing 250 patients with excoriation disorder with 250 age-, race- and sex-matched controls identified between 2007 and 2019 at a single tertiary care center. We found that the majority of excoriation disorder patients were female (76%), Caucasian (82%) an

Quetiapine: novel uses in the treatment of depressive and anxiety disorders.

IMPORTANCE OF THE FIELD: Quetiapine, an atypical antipsychotic, has been approved for the treatment of schizophrenia, acute mania, bipolar depression and unipolar major depression. However, it is often used (off-label) to treat other depressive disorders and anxiety disorders in children and adults. AREAS COVERED IN THIS REVIEW: This article reviews the evidence for the safety and efficacy of quetiapine in these populations, as both monotherapy and augmentation to other psychotropics. WHAT THE READER WILL GAIN: This article provides an in-depth review of the published literature on the topic

[Aatypical antipsychotics in child and adolescent psychiatry--indications apart from schizophrenia].

OBJECTIVES: Given their special receptor profile, atypical antipsychotics are effective in the treatment of both positive and negative symptoms. Especially the serotonergic affinity suggests their potential utility for the treatment of depressive, anxious, and obsessive-compulsive symptoms as well. Reviewed here are the clinical experience with, studies of, and published reports on the use of atypical antipsychotics in the treatment of different psychiatric disorders other than schizophrenia in children and adolescents. METHODS: The literature from 1998-2004 was reviewed by means of PubMed an

Group schema therapy for cluster-C personality disorders: A multicentre open pilot study.

BACKGROUND: Group schema therapy (GST) is increasingly popular as a treatment for personality disorders (PDs), including Cluster-C PDs. Individual ST has proven to be effective for Cluster-C PD patients, while the evidence for GST is limited. This study aimed to investigate the effectiveness of GST for Cluster-C PD. Moreover, differences between the specific Cluster-C PDs (avoidant PD, dependent PD and obsessive-compulsive PD) were explored. METHODS: A multicentre open trial was conducted, including 137 patients with a Cluster-C PD (avoidant PD: n = 107, dependent PD: n = 11 and obsessive-com

FAST CBT for pediatric OCD: A multiple-baseline controlled pilot trial of parent training in exposure and response prevention delivered via telehealth.

OBJECTIVE: The current study utilized a single case series, non-concurrent multiple baseline design to examine the efficacy of training parents via telehealth videoconferencing in exposure and response prevention (ERP) for home delivery of the treatment for their children and adolescents with obsessive compulsive disorder (OCD). METHOD: There were nine participants aged 8 to 14  years who had received a primary diagnosis of OCD. The design involved a series of AB replications, whereby following pre-treatment assessments participants were randomly assigned to either a 2-week (n = 4) or 3-week

EEG correlates of induced anxiety in obsessive-compulsive patients: comparison of autobiographical and general anxiety scenarios.

BACKGROUND: The underlying symptomatology of obsessive-compulsive disorder (OCD) can be viewed as an impairment in both cognitive and behavioral inhibition, regarding difficult inhibition of obsessions and behavioral compulsions. Converging results from neuroimaging and electroencephalographic (EEG) studies have identified changes in activities throughout the medial frontal and orbital cortex and subcortical structures supporting the cortico-striato-thalamo-cortical circuit model of OCD. This study aimed to elucidate the electrophysiological changes induced by autobiographical and general anxi

Effects of mood stabilizers on marble-burying behavior in mice: involvement of GABAergic system.

RATIONALE: Obsessive-compulsive disorder (OCD) is characterized by recurrent unwanted thoughts (obsessions), usually accompanied by repetitive behaviors (compulsions) intended to alleviate anxiety. Marble-burying behavior is a pharmacological model for study of OCD. OBJECTIVES: In the present study, we examined the effects of mood stabilizers on marble-burying behavior in mice, as well as the role of GABA receptors in this behavior. METHODS: The effects of treatment with valproate, carbamazepine, lithium carbonate, lamotrigine, muscimol and baclofen on marble-burying behavior in mice were ev

Challenging anxiety: a focus on the specificity of respiratory symptoms.

Physiological symptoms are characteristic features of anxiety states. Presumably, specific psychophysiological profiles differentiate between anxiety disorders, which would offer potential for diagnostic purposes. Abundant evidence points to a causal relationship between panic disorder and instability of respiratory regulation. However, the specificity of most measures that indicate aberrant functioning of the respiratory system in PD can be questioned. Possibly, the traditional measures of respiratory functioning are too restricted. The underlying respiratory vulnerability in PD seems to cons

Long-term potentiation and memory processes in the psychological works of Sigmund Freud and in the formation of neuropsychiatric symptoms.

Far from disproving the model of mind functioning proposed by psychoanalysis, the recent advances in neuropsychiatrical research confirmed the crucial ideas of Sigmund Freud. The hypothesis that the origin of mental illnesses lies in the impossibility for a subject to erase the long-term effects of a remote adverse event is in tune with the view that several psychiatric disturbances reflect the activation of aberrant unconscious memory processes. Freud's insights did not stop here, but went on to describe in an extremely precise manner the neural mechanisms of memory formation almost a century

Functional Heterogeneity within the Primate Ventral Striatum for Motivational Regulation.

The ventral striatum (VS) is a key brain region for reward processing and motivation, and its dysfunctions have been implicated in psychiatric disorders such as apathy and obsessive-compulsive disorder. Although functional heterogeneity within the VS has been well established in rodents, its relevance and mechanisms in primates remain unclear. To address this issue, we performed bilateral pharmacological inactivation of the VS in two male macaque monkeys using muscimol, a GABAA receptor agonist. Precise targeting was achieved through computed tomography and magnetic resonance imaging. Behavior

The Treatment Acceptability/Adherence Scale: Moving Beyond the Assessment of Treatment Effectiveness.

It is becoming more broadly recognized that beyond effectiveness, the acceptability of interventions for anxiety disorders is an important consideration for evidence-based practice. Although advances in treatments for anxious psychopathologies have demonstrated that cognitive-behavioural interventions are more desirable than other types of psychotherapy or pharmacotherapy, there continue to be problems with adherence and dropout. It has been suggested that low treatment acceptability may be partially responsible for high dropout rates. Although a number of preliminary investigations in this do
