BACKGROUND: Overdose deaths involving opioid pain relievers (OPR), also known as opioid analgesics, have increased and now exceed deaths involving heroin and cocaine combined. This report describes the use and abuse of OPR by state.
METHODS: CDC analyzed rates of fatal OPR overdoses, nonmedical use, sales, and treatment admissions.
RESULTS: In 2008, drug overdoses in the United States caused 36,450 deaths. OPR were involved in 14,800 deaths (73.8%) of the 20,044 prescription drug overdose deaths. Death rates varied fivefold by state. States with lower death rates had lower rates of nonmedica

BACKGROUND: The impact of opioid dependence on employers, managed care, and society is significant. Inappropriate use of narcotic analgesics leads to uncontrolled pain management, dependence, and may lead to patient deaths, creating a tremendous cost burden to the health care system.
OBJECTIVE: To provide an overview of the clinical and economic impact of treating opioid dependence on managed care, employers, and society.
SUMMARY: An estimated 6% to 15% of people in the United States abuse drugs, and approximately 20% of Americans report using prescription opioids for nonmedical use. This is

INTRODUCTION AND AIMS: To investigate the prevalence and nature of injecting behaviour among patients on Methadone Maintenance Treatment (MMT) programs.
DESIGN AND METHODS: A self-reported questionnaire was handed to 423 patients enrolled in MMT across six clinics in the lower North Island of New Zealand.
RESULTS: A total of 151 patients responded, giving a 35.6% response rate. One hundred and twenty (79.5%) respondents reported they had injected methadone while enrolled in MMT, 84 (55.6%) had injected methadone in the last year and of those 43 (35.8%) had injected methadone in the last week

BACKGROUND: Neonatal abstinence syndrome (NAS) is a constellation of symptoms resulting from in utero exposure to opioids that appears in 30-80% of opioid exposed infants. Variability in NAS symtomatology is not well understood, and recently it has been suggested that the sex of the infant may play a role in predicting NAS severity. The current study examines the relationship of sex to need for NAS treatment, length of NAS treatment, and peak dose of medication required to treat NAS symptoms.
METHODS: Retrospective chart review of 308 infants was conducted to determine whether significant dif

This study examined individual and system characteristics associated with access to methadone maintenance treatment (MMT) among Medicaid-eligible adults entering treatment for opiate use in Oregon and Washington. Logistic regression was used to examine the relative contributions of predisposing, need, and enabling characteristics on access to MMT. Although the number of methadone admissions increased in both states, access rates (the percentage of opiate-using adults presenting for treatment who were placed on methadone) declined after 1995. Adults in remote counties were one fifth to one tent

The relationship between pupil size and subjective symptoms of opiate withdrawal during gradual opiate agonist detoxification has not yet been studied. In the current study, the authors sought to determine the relationship between pupil size and intensity of opiate withdrawal symptoms. To accomplish this, they examined 19 subjects meeting DSM-IV criteria for opiate dependence (304.00) on agonist therapy. All subjects were undergoing opiate detoxification with either methadone or the longer-acting 1-alpha acetylmethadol (LAMM). During two separate visits, subjects' pupil sizes were assessed in

1. In the last ten years basic research on the mechanism of action of opiates has led to the clearcut demonstration of the existence of opiate receptors--possibly several slightly different kinds--in the nervous system. 2. A number of endogenous ligands also called endorphins or enkephalins for these receptors have been discovered that proved to be peptides with opiate-like pharmacological activity and were shown to be localized in strategic neuronal pathways in the brain, spinal cord and pituitary gland. 3. Clinical researchers are beginning to explore the possible role of these opiate-like p

OBJECTIVES: Hepatitis C virus (HCV) and HIV transmission in the United States may increase as a result of increasing rates of opioid use disorder (OUD) and associated injection drug use (IDU). Epidemiologic trends among American Indian/Alaska Native (AI/AN) persons are not well known.
METHODS: We analyzed 2010-2014 Indian Health Service data on health care encounters to assess regional and temporal trends in IDU indicators among adults aged ≥18 years. IDU indicators included acute or chronic HCV infection (only among adults aged 18-35 years), arm cellulitis and abscess, OUD, and opioid-relate

Medication-assisted treatment (MAT) with methadone or buprenorphine has been shown to be more effective at reducing the use of illicit opioids, the risk of drug-related overdose, and overall healthcare costs, on average, compared to abstinence-based addiction treatments for individuals with an opioid use disorder (OUD). Individuals who are adherent to MAT are more likely to experience positive outcomes. We used physical and behavioral Medicaid claims data of individuals newly treated with methadone (n = 212) and buprenorphine (n = 972) to examine the overall predictors of adherence, difference

Individuals seeking substance use treatment who have one or more co-occurring mental health problems tend to have lower treatment engagement, higher rates of attrition, and poorer treatment outcomes. Readiness to change (RTC) is an integral construct in the recovery process, with higher RTC associated with improved treatment outcomes. However, the impact of psychiatric symptoms on RTC is not fully understood, especially among specialty subpopulations, such as military Veterans. Therefore, the aim of the present study was to examine the associations of mental health problems with RTC in a sampl

The origin of the psychiatric illnesses observed in drug abusers is often unclear. This study examines the causal relation between drug abuse and specific psychiatric disorders. Fifty-one male veterans first seen in 1972, who were admitted at least once per year for six consecutive years for inpatient drug-abuse treatment, underwent psychiatric assessments at each admission. Eleven men mainly used stimulants, 14 depressants, and 26 opiates. Initial psychiatric examinations showed low symptom levels in all groups but no statistically significant differences among them. By the end of six years,

BACKGROUND: Opioids are frequently prescribed for chronic low back pain (CLBP), but there are little prospective data on which patient subgroups may benefit. Psychiatric comorbidity, such as high levels of depression and anxiety symptoms (termed comorbid negative affect [NA]), is a common presentation and may predict diminished opioid analgesia and/or increased opioid misuse.
METHODS: The authors conducted a 6½-month prospective cohort study of oral opioid therapy, with an active drug/placebo run-in period, in 81 CLBP patients with low, moderate, and high levels of NA. Treatment included an o

OBJECTIVE: The ability to predict risk for violating opioid medication policies, known as aberrant drug-related behavior, is critical for providing optimal treatment. Many pain management centers measure risk using one of several partially validated measures: the Screener and Opioid Assessment for Patients with Pain (SOAPP), the Diagnosis, Intractability, Risk, and Efficacy inventory (DIRE), and/or the Opioid Risk Tool (ORT). However, little is known about how these measures compare with each other in predicting aberrant drug-related behavior and discontinuance of opioid pain medications. The

BACKGROUND AND AIMS: Treatment for opioid use disorders (OUD) reduces the risk of mortality and infectious disease transmission; however, opportunities to quantify the potential economic benefits of associated decreases in drug-related crime are scarce. This paper aimed to estimate the costs of crime during and after periods of engagement in publicly funded treatment for OUD to compare total costs of crime during a hypothetical 6-month period following initiation of opioid agonist treatment (OAT) versus detoxification.
DESIGN: Retrospective, administrative data-based cohort study with compreh

Opioid Use Disorder (OUD) is a chronic relapsing clinical condition with tremendous morbidity and mortality that frequently persists, despite treatment, due to an individual's underlying psychological, neurobiological, and genetic vulnerabilities. Evidence suggests that these vulnerabilities may have neurochemical, cellular, and molecular bases. Key neuroplastic events within the mesocorticolimbic system that emerge through chronic exposure to opioids may have a determinative influence on behavioral symptoms associated with OUD. In particular, structural and functional alterations in the dendr

BACKGROUND: Sexually transmitted infections (STIs) and opioid misuse are co-occurring concerns among U.S. young adults. We assessed the association between lifetime opioid misuse and past-year chlamydia/gonorrhea diagnosis among U.S. college students and described condom use and sexual partner number in relation to opioid misuse and STI diagnosis. METHODS: We analyzed American College Health Association-National College Health Assessment (ACHA-NCHA III) survey data from fall 2019 through fall 2022. The analytic sample included 138,757 students aged 18–24 years with data on lifetime opioid misu

OBJECTIVE: To describe the route of drug use among overdose decedents in North Carolina.
METHODS: We use data from the North Carolina State Unintentional Drug Overdose Reporting System from 2018 to 2023. We include variables on the characteristics of the decedents and context of the overdose, including the reported route of drug use and the drugs involved.
RESULTS: There were 19,520 overdose deaths, largely involving opioids (87.7%), particularly fentanyl and its analogues (75.5%), stimulants (61.3%), and benzodiazepines (20.9%). Route of drug use was not documented for most (53.9%) deaths.

BACKGROUND: Injection drug-use (IDU) is related to significant morbidity and mortality. Recent studies of IDU-associated infections have primarily focused on regions outside of the southern US. However, this area presents greater barriers to healthcare funding and lower availability of harm reduction services such as syringe service programs. In this study, we sought to describe infectious disease burden, uptake of medication for opioid use disorder (MOUD), and healthcare utilization in patients with opioid use disorder (OUD) who received inpatient addiction psychiatry consultation at a large

INTRODUCTION: Xylazine-adulterated fentanyl use is associated with manifold negative health outcomes, including skin and soft tissue infections, intensified opioid withdrawal, and can complicate medication for opioid use disorder (MOUD) induction. This qualitative research aimed to explore how healthcare providers test for and treat xylazine-related health harms among people who use drugs (PWUD).
METHODS: From February-April 2025, twelve healthcare providers with expertise in managing the health harms of xylazine from three Northeastern states completed an approximately 45-min semi-structured
