Quantifying mast cells in bladder pain syndrome by immunohistochemical analysis.

OBJECTIVES: To evaluate a simple method for counting mast cells, thought to have a role in the pathophysiology of bladder pain syndrome (BPS, formerly interstitial cystitis, a syndrome of pelvic pain perceived to be related to the urinary bladder and accompanied by other urinary symptoms, e.g. frequency and nocturia), as >28 mast cells/mm(2) is defined as mastocytosis and correlated with clinical outcome. PATIENTS AND METHODS: The current enzymatic staining method (naphtolesterase) on 10 microm sections for quantifying mast cells is complicated. In the present study, 61 patients had detrusor

Congenital insensitivity to pain with Anhidrosis (NTRK1 mutation) and early onset renal disease: clinical report on three sibs with a 25-year follow-up in one of them.

Congenital insensitivity to pain with anhidrosis (CIPA) is an autosomal recessive disorder caused by mutations in the neurotrophic tyrosine receptor kinase 1 (NTRK1) gene which encodes the receptor for nerve growth factor (NGF). We report the clinical course in three sibs with CIPA and proven NTRK1 gene mutations with a follow-up over a 25-year period in one of them. They had the characteristic clinical features of an abnormally high pain threshold, and mental retardation; in addition their clinical course was marked by the occurrence of early onset renal disease with recurrent microhematuria

Evaluation of the Effectiveness of Electrotherapy in Voice Treatment: A Controlled Randomized Clinical Trial.

INTRODUCTION: Transcutaneous electrical nerve stimulation (TENS) is widely used as a therapeutic tool for pain relief, muscle relaxation, and fatigue reduction. Vocal production, in turn, is a complex process involving the interaction of anatomical, physiological, neurological, and emotional factors. In some cases, dysphonia is related to muscular changes, manifesting as excessive contraction of the intrinsic and extrinsic laryngeal muscles and changes in glottal closure. These conditions can lead to continuous vocal effort, favoring the development of nodules, edema, and other impairments, ne

Effect of opioid-free anesthesia on postoperative recovery in elderly patients undergoing laparoscopic abdominal surgery: a randomized controlled trial.

BACKGROUND: Opioid-free anesthesia (OFA) has been proposed to reduce opioid-related adverse effects, although its impact on overall postoperative recovery remains uncertain. We evaluated whether OFA improves postoperative quality of recovery compared with opioid-based anesthesia (OA) in elderly patients undergoing laparoscopic abdominal surgery. METHODS: In this single-center randomized controlled trial, 200 patients aged ≥60 years undergoing elective laparoscopic abdominal surgery were randomized to receive either OFA (dexmedetomidine, esketamine, and lidocaine) or conventional OA. The prima

Promoter demethylation of cerebellin 2 by ten-eleven translocation 3 contributes to peripheral sensitization in trigeminal neuropathic pain of mice.

BACKGROUND: Chronic neuropathic pain involves complex molecular adaptations, and emerging evidence indicates that cerebellins (CBLNs) play a role in sensory processing. This study investigates the role of CBLN2 in trigeminal neuropathic pain (TNP) and examines its regulation through epigenetic mechanisms. METHODS: Bioinformatics analyses of CBLNs were performed using publicly available expression data from the trigeminal ganglion (TG). A mouse model of TNP was established through partial infraorbital nerve transection (pIONT). Facial allodynia in mice was assessed using the von Frey test. Qua

Amoxicillin vs. placebo to reduce symptoms in children with group A streptococcal pharyngitis: a randomized, multicenter, double-blind, non-inferiority trial.

UNLABELLED: The efficacy of antibiotic therapy for group A streptococcus (GAS) pharyngitis is debated. The role of antibiotics in preventing complications seems limited, with the main potential benefit being symptom duration reduction. Our study aimed to evaluate whether a placebo is non-inferior to amoxicillin in reducing fever duration. We randomized 88 children between 3 and 15 years of age presenting with acute symptoms of pharyngitis and a positive rapid antigen detection test for GAS to receive 6-day treatment with either placebo (n = 46) or amoxicillin (n = 42). The primary outcome was

Association between descending pain modulatory system and cognitive impairment in fibromyalgia: A cross-sectional exploratory study.

BACKGROUND: The successful regulation of sensory input to the central nervous system depends on the descending pain modulatory system (DPMS). For the effective regulation of sensory input to the central nervous system and behavioral responses to pain, the DPMS is required. Its connection to fibromyalgia (FM)-related cognitive dysfunction has not yet been investigated. Therefore, this study tested whether measures of verbal fluency, sustained attention, and short-term and working memory could distinguish FM patients from healthy controls (HC). Additionally, it investigated, using a standardized

Validation of Motor Outcome Measures in Myotonic Dystrophy Type 2.

Introduction: Myotonic dystrophy type 2 (DM2) lacks disease-specific, validated, motor outcome measures (OMs), and patients' reported outcomes (PROs). This represents a limit for the monitoring of disease progression and treatment response. Our aim was to identify the most appropriate OMs to be translated in clinical practice and clinical trials on DM2. This study has been registered on clinicaltrials.gov NCT03603171 (https://clinicaltrials.gov/ct2/show/NCT03603171). Methods: Sixty-six patients with genetically confirmed DM2 underwent a baseline and a follow-up visit after 1 year. The tested O

Inhibition effect of choline and parecoxib sodium on chronic constriction nerve injury-induced neuropathic pain in rats.

PURPOSE: The simultaneous use of drugs with different mechanisms of analgesic action is a strategy for achieving effective pain control while minimizing dose-related side effects. Choline was described to potentiate the analgesic action of parecoxib sodium at small doses in several inflammatory pain models. However, these findings are still very limited, and more associated data are required to confirm the effectiveness of the combined choline and parecoxib sodium therapy against inflammatory pain. METHODS: Adult rats were randomly divided into 9 groups (N = 6/group). The sham surgery group r

Individualized regulation of inflatable laryngeal mask airway cuff pressure reduces postoperative pharyngolaryngeal pain in elderly patients: a randomized controlled trial.

BACKGROUND: Inflatable laryngeal mask airway (ILMA) cuff pressure lacks routine monitoring and standardized safe thresholds, with clinical practice often referencing manufacturer specifications. Empirical inflation in clinical practice often leads to overinflation and increased pharyngolaryngeal complications. This study established an individualized ILMA cuff pressure regulation strategy based on physiological characteristics and sealing principles, aiming to compare its effect on postoperative pharyngolaryngeal pain in elderly patients with empirical inflation. METHODS: This single-center,

Phenotyping strategies for chronic overlapping pain conditions and internalizing disorders in Veterans: Prevalence, comorbidity, and latent structure as evidence for construct validity.

Chronic overlapping pain conditions (COPCs), internalizing (INT) disorders, and opioid use disorder (OUD) are common, comorbid, and difficult to phenotype at scale. Electronic health record (EHR) studies commonly define cases using Any Code (AC; ≥1 ICD-9/10 code) and Multiple Code (MC; ≥1 inpatient or ≥2 outpatient codes) phenotyping strategies, but it is unclear whether these thresholds change only case numbers or also the clinical relationships among conditions. This cross-sectional study included approximately 950,000 Million Veteran Program participants with ≥2 visits. AC and MC phenotypes
