Vestibular abnormalities in patients with Bell's palsy.

In order to assess the extent of the underlying pathology in Bell's palsy, the vestibular function of 24 patients was evaluated within the first week from the onset of facial paralysis. Seven patients had pathological findings by ENG and an additional three had subjective vestibular signs; four of these ten patients also showed abnormal auditory brain stem evoked potentials. There was no correlation between the severity of the paralysis or the prognosis for recovery and the presence of vestibular abnormalities.

[Fuchs's heterochromic cyclitis in Moebius' syndrome (author's transl)].

In a 16-year-old boy with congenital bilateral paralysis of the sixth cranial nerve and unilateral paralysis of the seventh nerve (on the right) heterochromia of the right eye was observed, characterized by a lighter color of the iris, precipitates on Descemet's membrane and absence of synechiae. The development of Fuch's heterochromic cyclitis in combination with Moebius' syndrome is discussed in connection with possible pathogenic factors, in particular immunopathologic processes which may contribute to an understanding of the origin and progression of the disease.

Recurrent Laryngeal Nerve Reinnervation.

To date, there is no standardized surgical therapy for pediatric unilateral vocal fold paralysis in the pediatric population. Treatment options include observation with voice therapy, vocal fold injection, laryngeal framework surgery, and recurrent laryngeal nerve reinnervation. The latter has been shown to be highly successful in the treatment of dysphonia, aspiration and dyspnea/stridor due to persistent unilateral vocal fold paralysis, and it provides a durable and safe option for this population.

Acute Laryngeal Dyspnoea Revealing Anterior Cervical Hyperostosis (Forestier's Disease).

UNLABELLED: Forestier's disease, or diffuse idiopathic spinal hyperostosis, can exceptionally lead to acute obstruction of the upper airways when the hyperostosis is located in the cervical region. We report the case of an 87-year-old man admitted for acute inspiratory dyspnoea and progressive dysphonia. Endoscopic examination confirmed left laryngeal paralysis and pharyngolaryngeal oedema. Imaging revealed a large anterior hyperostosis compressing the airway. This case illustrates the interaction between mechanical compression, local inflammation, and chronic laryngeal paralysis, which can le

Leukemia inhibitory factor promotes recovery of locomotor function following spinal cord injury in the mouse.

We describe an easy, minimal, rapid, and reproducible model of mouse spinal cord injury (SCI) that results in permanent paralysis involving one hind limb. We used this model to evaluate whether the paralysis can be prevented using two known neuroprotective drugs, namely leukemia inhibitory factor (LIF) and minocycline (MIN). Mice in the control vehicle (VEH) and MIN groups with SCI had negligible recovery of locomotor behavior. In contrast, the LIF groups showed a statistically significant improvement in locomotor behavior. Maximal recovery was observed when LIF was administered 2, 8, and 24 h

Residual poliomyelitis of lower limb-pattern and deformities.

The pattern of muscle paralysis and paresis in the lower limbs has been studied in 1356 children with 1800 poliomyelitic limbs, revealed that more than two-third (68 = 55%) muscles were affected, with the ratio of paresis and paralysed muscles were 1.89:1.00. The muscles most frequently affected were tibialis anterior (1516), quadriceps (1465) and tibialis posterior (1435) while tibialis anterior (1070), tibialis posterior (820) and quadriceps (766) were most commonly paralysed in descending order. The tibialis anterior which leads in paralysis group, has come last in order of frequency of par

Laryngeal synkinesis: its significance to the laryngologist.

Basic research and surgical cases have shown that the injured recurrent laryngeal nerve (RLN) may regenerate axons to the larynx that inappropriately innervate both vocal cord adductors and abductors. Innervation of vocal cord adductor muscles by those axons that depolarize during inspiration is particularly devastating to laryngeal function, since it produces medial vocal cord movement during inspiration. Many patients thought to have clinical bilateral vocal cord paralysis can be found to have synkinesis on at least one side. This will make the glottic airway smaller, particularly during ins

Vertebral Artery Dissecting Aneurysm Causing Central Tapia's Syndrome: A Case Report.

The central type of Tapia's syndrome is an extremely rare presentation, characterized by unilateral paralysis of the vagal and hypoglossal nerves, contralateral hemiparesis, or hemihypesthesia. This report describes a case of a middle-aged patient who developed central Tapia's syndrome due to a right vertebral artery dissecting aneurysm (VADA). The patient complained about swallowing difficulty and odynophagia. Right vocal cord paralysis, mild tongue deviation to the right side, left hypesthesia, and decreased temperature sensation with left hemiparesis were observed in neurologic exams. A rig

Clinical and neurophysiological variability in Andersen-Tawil syndrome.

INTRODUCTION: Andersen-Tawil syndrome (ATS) is characterized by a triad of periodic paralysis, ventricular arrhythmias, and dysmorphism. However, patients often lack one or more of these features. METHODS: Clinical and neurophysiological features were reviewed of five members in two families with heterozygous mutations in KCNJ2 (R218Q and R67W). RESULTS: Only one patient had all features of the triad of ATS. One patient had low-set ears, and the others had minor anomalies. Bidirectional ventricular tachycardias were seen in two patients. Two patients (R67W) never had episodes of paralysis. T

Antecedents, development, adoption, and application of Duchenne's trocar for histopathologic studies of neuromuscular disorders in the nineteenth century.

Duchenne de Boulogne was one of the founders of clinical neurology. His name has been eponymically linked to the most common form of muscular dystrophy, originally described by him as pseudo-hypertrophic muscular paralysis or myo-sclerotic paralysis. Obtaining muscle biopsy specimens was essential to gain insight about the etiopathogenensis of the disease. Duchenne invented a novel instrument: l'emporte-pièce histologique, also known as "Duchenne's trocar," to perform muscle biopsies. Following Duchenne's design and instructions, a Parisian company, Charrière, constructed the first instrument

Facial nerve palsy in a child: Bell's palsy? Think again!

INTRODUCTION: Half of facial paralysis in children is idiopathic at origin. However, dismissing facial paralysis as being idiopathic without a thorough history and meticulous examination could be disastrous as illustrated by this case. CASE REPORT: We report a case of sphenoid wing meningioma in a 4-year-old girl. She first presented with only facial asymmetry that was noticed by her mother. Examination suggested a left upper motor neuron facial nerve palsy. A sphenoid wing meningioma was found on magnetic resonance imaging (MRI) of her brain. She underwent craniotomy and total tumour excisio

Hilger facial nerve stimulator: a 25-year update.

Percutaneous nerve excitability testing using the Hilger facial nerve stimulator was introduced about 25 years ago. The test is reliable, easy to use, and inexpensive; it continues to be the most frequently used method for predicting prognosis of facial nerve disorders. Between 1966 and 1974, we recorded 10,243 nerve excitability tests on 865 patients with a mean of 3.29 tests for each peripheral branch and 3.43 for the trunk. Using a multiple regression model, we determined the effect on nerve stimulation values of age, sex, race, diabetes, hypertension, partial or complete clinical paralysis

Assessment of pre-extubating recurrent laryngeal nerve palsy using ultrasound in postoperative patients with esophageal cancer: a prospective observational study.

PURPOSE: Ultrasound performed after extubation has been suggested to be useful for the diagnosis of recurrent laryngeal nerve (RLN) paralysis. However, the use of ultrasound for this purpose before extubation has not been examined. The aim of this study was to examine the versatility (interrater reliability) and usefulness of ultrasound for evaluating the movement of vocal cords before extubation. METHODS: The subjects were 30 patients who underwent radical surgery for esophageal cancer from August 2020 to December 2021. An experienced examiner performed an ultrasound examination before and a

Dichlorphenamide: A Review in Primary Periodic Paralyses.

Oral dichlorphenamide (Keveyis™) is a carbonic anhydrase inhibitor that is approved in the USA for the treatment of primary hyperkalaemic and hypokalaemic periodic paralyses and related variants. The efficacy and safety of dichlorphenamide in patients with primary periodic paralyses have been evaluated in four 9-week, randomized, double-blind, placebo-controlled, phase III trials [two parallel-group trials (HOP and HYP) and two crossover trials]. In two trials in patients with hypokalaemic periodic paralysis, dichlorphenamide was associated with a significantly (eightfold) lower paralytic atta

Residual neuromuscular block: lessons unlearned. Part II: methods to reduce the risk of residual weakness.

The aim of the second part of this review is to examine optimal neuromuscular management strategies that can be used by clinicians to reduce the risk of residual paralysis in the early postoperative period. Current evidence has demonstrated that frequently used clinical tests of neuromuscular function (such as head lift or hand grip) cannot reliably exclude the presence of residual paralysis. When qualitative (visual or tactile) neuromuscular monitoring is used (train-of-four [TOF], double-burst, or tetanic stimulation patterns), clinicians often are unable to detect fade when TOF ratios are b

Paralytic syndromes in children: epidemiology and relationship to vaccination.

Acute flaccid paralysis is a standard outcome for detection of poliomyelitis globally and an ongoing potential vaccine-associated adverse event concern for polio, influenza, and meningococcal vaccines. No systematic population-based data on the epidemiologic and clinical features of this condition, or its potential association with immunization, have been reported from the United States. The present retrospective cohort study of acute flaccid paralysis in the Southern and Northern California Kaiser Permanente Health Care Plans was conducted using computerized diagnosis data and medical record

[The one and a half vertical syndrome as the only evidence of bilateral thalamo-subthalamic infarction].

INTRODUCTION: The one and a half vertical syndrome consists of paralysis of upward vertical conjugated gaze and monocular paralysis of downward gaze or vice-versa. It occurs as a consequence of a mesencephalodiencephalic lesion, either unilaterally or bilaterally, due to effects on structures such as the interstitial nucleus of Cajal, posterior commissure and rostral interstitial nucleus of the medial longitudinal bundle. The arterial supply of the structures involved in the supranuclear control of vertical gaze depends on the paramedian thalamic arteries (or posterior thalamo-subthalamic arte
