Chronic meningococcaemia is a very rare clinical manifestation of invasive infection by Neisseria meningitidis. A 9-year-old girl was admitted to our clinic with complaints of fever, headache, arthralgia, and maculopapular rash. The diagnosis was made by the growth of Neisseria meningitidis in the blood cultures. Four days after admission, liver function tests were increased and were compatible with cholestatic hepatitis. Thereafter, the patient was successfully treated and symptoms were completely resolved. To our knowledge, there have been no previous reports of Neisseria meningitidis causin

A 37-year-old woman suffered from middle cerebral artery infarction secondary to dissection of the left internal carotid artery. Nine days before, a cesarean section had been performed on her after 20 h of unsuccessful labor. Cerebral angiography at admission revealed no further vascular abnormalities. A few days later, however, the patient developed additional dissections of the right internal carotid artery and both vertebral arteries. Pregnancy, childbirth, and a history of rheumatoid arthritis in this patient may have contributed to the dissections; however, due to the unknown etiology of

A 55-year-old woman presented with fever and a stiff neck due to an intracranial poorly differentiated carcinoma at the right cerebellopontine angle. The patient suffered from typical trigeminal pain and had undergone a removal of the right cerebellopontine angle epidermoid 13 years before at another hospital. On admission, MRI imaging showed a lesion at the right cerebellopontine angle with marked contrast enhancement. Partial removal of the tumor was achieved. A histological examination of the tumor showed a poorly differentiated carcinoma accompanied by typical desquamated tissue of the epi

Epistaxis, being the commonest ear, nose and throat (ENT) emergency requiring hospital admission, is clearly an important condition--not only to the specialist but also to any general practitioner. This short review looks at the aetiology, associated clinical considerations (especially those of hypertension and hypoxaemia) and evolution of its management. Herein is also suggested a methodical and systematic approach to treating the disorder, both at the level of the accident and emergency department and at that of in-patient care. The patient with epistaxis of a refractory nature is included,

We report a case of hypoglycaemic shock which occurred in a patient 18 h after injury. The patient was involved in a road traffic accident and sustained multiple rib fractures and bruising in the (R) lumbar region. Eleven hours after his last meal he developed sudden weakness, profuse sweating, air hunger, thirst, disorientation and suddenly lapsed into coma 18 h after admission. Administration of 50 ml of 50% dextrose resulted in immediate restoration of consciousness. This case shows that this complication can occur in trauma patients even though the metabolic response to trauma is usually a

A 65-year-old man with Behçet's disease was admitted to our hospital because of an acute left occipital headache and truncal ataxia. He had been treated with prednisolone since the age of 40. MRI showed a hyperintense lesion on T2-weighted images on the left of the medulla oblongata. Left vertebral angiogram showed findings consistent with a dissection at the V4 segment of the artery. After admission, the patient showed marked improvement and was discharged 1 month later without neurological deficits. This is a very rare case of medullary infarction due to vertebral dissecting aneurysm in a pa

We report a case of 47-year-old patient with von Willebrand's disease (VWD) caused by polycythemia vera (PV) who underwent CABG surgery. The patient has been suffering of PV for 10 years and was admitted because of post infarction angina. On admission, she was found to have a decreased von Willebrand factor which was suggested by prolonged APTT. CABG was safely performed without undue bleeding with the use of Factor VIII concentrates. The appropriate control of polycythemic state before surgery and perioperative use of Factor VIII concentrates was considered to be important for successful open

We experienced an adult patient with EDTA-dependent pseudothrombocytopenia due to rubella virus infection. A 23-year-old male complaining of eruption and fever was admitted to our hospital on June 21, 1992. Laboratory findings on admission showed the platelet count 1.5 x 10(4)/microliters with EDTA, but 11.5 x 10(4)/microliters with heparin. Platelet agglutination was absorbed in the peripheral blood smear samples with EDTA. The hemagglutination inhibition titer for rubella virus raised from 1:32 to 1:52 in paired sera. We diagnosed this patient as having EDTA-dependent pseudothrombocytopenia

We encountered a patient who was diagnosed as rheumatoid arthritis (RA) at 15 years old and developed malignant RA (MRA) within one year. He suffered from mononeuritis multiplex and cutaneous infarction. Despite of treatment including steroid pulse therapy, neuritis progressed. Lung infiltration, pancreatitis and intestinal bleeding were accompanied. He died of disseminated intravascular coagulation on 153 days after admission. Autopsy revealed systemic rheumatoid vasculitis in coronary artery, pancreas, liver, small and large intestine, kidney and lung. These severe vasculitis occurred in you

Two patients with systemic lupus erythematosus (SLE) and T-zone lymphoma are described. On admission, both showed arthralgia, generalized lymphadenopathy, hypergammaglobulinemia and positive antinuclear antibody. Lymph node biopsies revealed diffuse infiltration of atypical T-lymphocytes in the expanded interfollicular area (T-zone), a finding characteristic for the T-zone lymphoma. Renal biopsy showed lupus nephritis and neoplastic lymphoid cell infiltration in the glomeruli of one patient, but only diffuse infiltration of neoplastic lymphoid cells and mature plasma cells were observed in the

A case of severe lithium carbonate self-poisoning is described, presenting with a very high serum lithium level (14.6 mmol/L) on admission. Lengthy and repeated hemodialyses were required to lower lithemia to nontoxic ranges. As is usually reported, our patient had prolonged neurologic manifestations (coma, hyperreflexia, fluctuating focal signs) and developed hypotension, cardiovascular collapse, nephrogenic diabetes insipidus, and diarrhea. Other less common features were the occurrence of acute myocardial infarction without coronary artery lesions and thrombocytopenia. The possible pathogen

The authors describe two cases of siderosis of the bulbus which developed as a consequence of late diagnosis of intraocular foreign bodies. The character of siderosis differed, however, in the two patients. In the first patient the picture of generalized siderosis confirmed also by electroretinographic examination permitted the assumption of an adverse course of the disease already on admission, as was confirmed by the progression of the disease even after removal of the intraocular foreign body. In the second patient probable encapsulation of the foreign body led to focal siderosis and a favo

We measured the concentrations of serum nitrates/nitrites and plasma cyclic guanosine monophosphate as markers of nitric oxide synthesis in patients with or without septic shock for 5 days following admission to intensive care. We found that nitrate/nitrite concentrations, when corrected for the effect of renal failure, were significantly higher in patients with septic shock, both on admission and in the final samples drawn. In a logistic regression analysis, the rate of change of nitrate/nitrite concentration was associated with survival to day 28 (falling in survivors). The concentration of

OBJECTIVE: Monitoring cytomegalovirus (CMV) plasma DNAemia is not used in routine clinical practice for immunocompetent patients. However, immunocompetent patients in the intensive care unit (ICU) may develop transient immunosuppression due to severe illness and its treatment, potentially leading to CMV reactivation. This study aimed to investigate the incidence, risk factors, and clinical outcomes of CMV reactivation in non-immunocompromised patients in the intensive care unit (ICU).
MATERIALS AND METHODS: Patients admitted to the Internal Medicine and Anesthesia ICUs were included. CMV-sero

BACKGROUND: Aboriginal Australians are reported to have a high burden of chronic airway diseases. However, prescribing patterns and related outcomes of airway directed inhaled pharmacotherapy, (short-acting beta agonists (SABA), short-acting muscarinic antagonists (SAMA), long-acting β-agonists (LABA), long-acting muscarinic antagonists (LAMA) and inhaled corticosteroids (ICS)) among Aboriginal Australian patients with chronic airway disease have been sparsely reported in the past.
METHODS: A retrospective cohort study was conducted, using clinical, spirometry data, chest radiology, primary h

BACKGROUND: The quick Sepsis-related Organ Failure Assessment (qSOFA) is a new screening system for sepsis that has prognostic performance equal to the full SOFA for patients with suspected infection outside the intensive care unit (ICU). The predictive value of qSOFA for mortality and site of care in patients with pneumonia is not clear. The present study was designed to investigate the predictive performance of qSOFA, CRB-65 (confusion, respiratory rate ≥30/minute, systolic blood pressure <90 mmHg or diastolic blood pressure ≤60 mmHg, age ≥65 years) and CRB (confusion, respiratory rate ≥30/m

BACKGROUND: Burn patients who require CPR before admission to a burn center are anecdotally known to suffer higher mortality than those who do not require pre-hospital CPR.
STUDY DESIGN: A retrospective chart review identified adult patients admitted to our burn center between 2013 and 2015. Included patients met 1 or both of the following criteria: 20% or more total body surface area burned and need for intubation before admission to our facility. We sought to identify predictors of early death, late death, and survival among burn patients who underwent CPR before admission.
RESULTS: Of the

Eosinophilic granulomatosis with polyangiitis is usually characterised by asthma, allergic rhinitis and peripheral eosinophilia. Presentations can vary greatly especially when there is cardiac involvement as demonstrated in these two case reports. Patient A initially presented to casualty with severe sinus pain and was diagnosed with severe sinonasal polyposis. After routine nasal polypectomy he had a cardiac arrest and was transferred to intensive care. Patient B presented to his general practitioner with a 4-week history of breathlessness, joint pain and a rash resulting in admission to hosp

BACKGROUND: Older adults admitted to acute geriatric units frequently have multidimensional geriatric vulnerability, including frailty, functional and cognitive impairment, multimorbidity, and nutritional risk. The Modelo de Asignación Diagnóstica y Terapéutica (MADiT; Diagnostic and Therapeutic Assignment Model) is a pragmatic clinical-care classification framework used in routine practice to organise pre-admission multidimensional vulnerability into four ordered categories.
OBJECTIVES: To examine the association between MADiT and 1-year all-cause mortality in older adults admitted to an acu

AIM: Endodontic conditions are associated with severe odontogenic infections. However, no studies have explicitly explored their role in the infectious process. This study aimed to assess associations between tooth-related and patient-related factors resulting in severe odontogenic infections and clinical care needs.
METHODOLOGY: A retrospective audit was conducted of all records of patients who presented with severe odontogenic infections requiring admission between 2018 and 2023, at the largest public hospital in South Australia. Pericoronitis, postoperative, and non-odontogenic infections
