Applying cognitive interviewing process to refine an instrument for measuring deprescribing experience and outcomes in older veterans.

BACKGROUND: Currently, there is no validated instrument that assesses a patient's reported deprescribing experiences and outcomes. This study reports the third phase of refining such an instrument, the Deprescribing Evaluation and Quality Improvement instrument (DEQI), using cognitive interviews. Cognitive interviews have been used in the development of many survey tools; however, there is limited evidence of their use in pharmacy research, particularly in deprescribing studies involving older adults. The aim of this phase of the DEQI instrument development is to evaluate and refine the indivi

Comparative Effectiveness of Lipid-Lowering Therapies in Increasing High-Density Lipoprotein Cholesterol Levels in Patients with Chronic Kidney Disease.

BACKGROUND: In patients with chronic kidney disease (CKD), lower and subfunctional high-density lipoprotein cholesterol (HDL-C) is associated with poor cardiovascular outcomes. Notwithstanding such poor outcomes, the primary therapeutic target in patients with CKD is low-density lipoprotein cholesterol (LDL-C), and the comparative effectiveness of commonly used lipid-lowering therapies (LLTs) in changing HDL-C levels in patients with non-dialysis-dependent CKD (NDD-CKD) remains unclear. METHODS: In this retrospective cohort study, using a target trial emulation framework, we examined a nation

Who receives psychiatry-focused pharmacogenomic testing, and is it associated with prescribing patterns and acute care utilisation in depression? Real-world evidence from a large health system.

BACKGROUND: Depression is highly prevalent, and antidepressant response and tolerability vary widely. Pharmacogenomic (PGx) testing is increasingly used to support antidepressant selection, but its real-world uptake, equity, and associations with prescribing and healthcare utilisation remain incompletely characterised. METHODS: Using electronic health record data from a large health system (2013-2023), we identified 1563 patients with depression who received psychiatry-focused PGx testing and compared them with 248,017 indexed non-PGx controls. We examined demographic and clinical factors ass

The duration of phentermine therapy in the private healthcare sector of South Africa: A retrospective analysis.

BACKGROUND: Phentermine is an amphetamine derivative and is globally the most used oral appetite suppressant for obesity. Its long-term (LT) use (a period >12 consecutive weeks) may be associated with amphetamine-like adverse effects. The US Food and Drug Administration has restricted its use to short term (ST) only (not longer than 12 consecutive weeks), specifically stated in the patient information leaflet. This serves as a prescribing guideline that is provided to assist both general and specialist practitioners in drug prescribing that is rational and safe, and to maximise patient outcome

Stability Study of Meropenem 50 mg/mL Eye Drops in Polypropylene Dropper Bottles.

Background/Objectives: Meropenem is a broad-spectrum carbapenem antibiotic with demonstrated efficacy against multidrug-resistant Gram-negative pathogens. Although its use as an ophthalmic formulation is off-label, growing clinical evidence supports its application in severe ocular infections such as keratitis and endophthalmitis. However, the intrinsic instability of meropenem in aqueous solutions and the absence of standardized ophthalmic preparations limit its routine use. Furthermore, no stability studies are currently available for meropenem 50 mg/mL eye drops stored in polypropylene (PP)

Berberine-Drug Interactions: Mechanisms, Clinical Relevance and Risk Stratification-A Narrative Review.

Background: Berberine, an isoquinoline alkaloid present in Berberis spp., Coptis chinensis and Hydrastis canadensis, is among the most widely consumed metabolic-health supplements, popularized as "nature's Ozempic". Concurrent, often undisclosed use with prescription drugs is common in older adults, yet berberine is far from inert. Objective: To synthesize the evidence on berberine as a perpetrator of supplement-drug interactions, propose a four-axis mechanistic taxonomy, with product quality treated separately as a modifier of exposure rather than as a mechanism, and derive a clinically actio

Incremental health care resource utilization, costs, and predictors of high costs in generalized myasthenia gravis.

BACKGROUND: The management of patients with generalized myasthenia gravis (gMG) is complex and often involves substantial health care expenses. As the treatment paradigm continues to evolve, understanding the economic burden of gMG must also reflect the availability and increasing use of newly approved and advanced therapies in a diverse patient population. OBJECTIVE: To estimate incremental health care resource utilization (HRU) and costs associated with gMG compared with non-MG controls and identify predictors of high health care costs in gMG in an insured US population cohort. METHODS: Ad

Pharmacists' role in recognising medication-related oral health risks in aged care: a qualitative study of pharmacists' and dentists' perspectives.

INTRODUCTION: Medication-related oral health complications, including salivary gland hypofunction, xerostomia, dental caries, candidiasis, mucosal injury, and swallowing difficulties, are common among older adults exposed to polypharmacy across aged-care and broader clinical practice contexts. Pharmacists are increasingly involved in medication management in residential aged care and may be well positioned to support the earlier recognition of medication-related oral health risks. Despite this, previous literature suggests that oral health is often insufficiently integrated into medication man

Systemic Corticosteroid Dispensing Patterns Among Patients with Sinonasal Polyps: A Population-Based Retrospective Cohort Study.

IMPORTANCE: Systemic corticosteroids (SCS) are widely prescribed for the treatment of chronic rhinosinusitis with nasal polyps (CRSwNP), yet no strict guidelines standardize their use. While data-specific to CRSwNP remains limited, growing evidence from lower respiratory disease supports a dose-response relationship between SCS use and related adverse outcomes that begin at relatively low cumulative doses. Reporting on SCS use among patients with CRSwNP is crucial to better understand prescribing patterns and inform future assessments of potential risks. OBJECTIVE: This study investigates dis

Effect of a multimodal intervention in general and digestive surgery wards on carbapenem use and antimicrobial resistance: protocol for a prospective, quasi-experimental interrupted time-series study.

INTRODUCTION: Infections are particularly common among hospitalised patients in surgical wards. Carbapenem antibiotics, major drivers of antimicrobial resistance, are frequently prescribed in this setting, often inappropriately and for unnecessarily prolonged durations. International guidelines emphasise the need for comprehensive multimodal antimicrobial stewardship programmes to optimise antibiotic use; however, evidence supporting their routine application in surgical wards remains scarce and is mostly derived from retrospective analyses or single-component interventions. This study aims to

Association of state-level PrEP coverage and new HIV diagnoses in the USA from 2012 to 2022: an ecological analysis of the population impact of PrEP.

BACKGROUND: Pre-exposure prophylaxis (PrEP) medications are highly efficacious for preventing new HIV infections, but the population impact of PrEP depends on whether prescribed PrEP reaches the people at greatest risk in a population, and adherence and persistence on PrEP over time. We sought to use ecological analysis methods to characterise the impact of PrEP on HIV infections at the population level, complementing data from efficacy trials and programmatic monitoring. METHODS: We conducted an ecological analysis of US state-level data on PrEP use, HIV viral suppression, and trends in HIV

Understanding motives for illicit medicinal cannabis use: an exploratory analysis in a medical cannabis program.

BACKGROUND: Medical Cannabis (MC) is authorized in numerous state-legislated programs to treat approved medical conditions. Notwithstanding MC access, some participants continue to use cannabis purchased outside of a state licensed MC pharmacy, otherwise known as illicit medicinal cannabis (IMC), to treat their medical conditions. Identifying barriers and contributors to MC use and motives for IMC use can promote safety, improve program design, and inform future research efforts. METHODS: This exploratory analysis utilized baseline survey data from a convenience sample-based prospective cohor
