Identifying Obstructive Sleep Apnea in At-Risk Youth: An Exploratory Cross-Sectional Study in Adolescents Living With Obesity.

Obstructive sleep apnea (OSA) is one of the most common sleep disorders, affecting 1-10 % of children. A key risk factor is elevated body mass index (BMI). This exploratory study aimed to assess OSA prevalence and severity in adolescents living with obesity and explore associations with clinical and metabolic parameters. Adolescents with obesity aged 10-15 years hospitalized for weight management were enrolled. Participants underwent examination including anthropometry, blood pressure, and lipid profile. BMI was evaluated using WHO BMI-for-age z-scores and an internal standard deviation score

Severe bradycardia in patients with obstructive sleep apnoea and good early response to CPAP.

BACKGROUND: Obstructive sleep apnoea (OSA) may lead to heart rhythm abnormalities including bradycardia. Our aim was to ascertain clinical and echocardiographic parameters in patients with OSA in whom severe bradycardia was detected in an outpatient setting, as well as to evaluate the efficacy of CPAP therapy on heart rate normalization at the early stages of treatment. METHODS: Fifteen patients mild, moderate or severe OSA and concomitant bradycardia were enrolled. A standard non-invasive diagnostic assessment protocol was followed, including blood tests and echocardiography. The 24-hour Hol

Effect of chronic benzodiazepine and benzodiazepine receptor agonist use on sleep architecture and brain oscillations in older adults with chronic insomnia.

STUDY OBJECTIVES: Insomnia in older adults is associated with widespread benzodiazepine (BZD) and benzodiazepine receptor agonist (BZRA) use despite evidence that chronic use disrupts sleep regulation and cognition. Little is known about BZD/BZRA effects on Non-rapid eye movement (NREM) slow oscillations (SO), spindles and their coupling, which is crucial for memory in older adults. Our objective was to investigate the effects of chronic BZD/BZRA use on sleep macro-architecture, electroencephalogram (EEG) relative power, SO and spindle characteristics and coupling. METHODS: After habituation

Symptoms and Physiological Parameters of Obstructive Sleep Apnoea Patients Diagnosed with a Portable Sleep Monitor: Implications and Challenges.

BACKGROUND: Obstructive sleep apnoea is a chronic, debilitating condition that if left undiagnosed and untreated is associated with adverse clinical events. The WatchPAT® is a portable wrist-worn sleep study device that uses peripheral arterial tonometry with pulse oximetry and actigraphy to assess respiratory disturbances. Unlike other level 111 sleep monitors, it measures actual sleep time, distinguishes between REM and Non-REM sleep stages, and thus generates actual apnoea-hypopnoea index. OBJECTIVES: The study was done to assess symptoms of obstructive sleep apnoea, evaluate physiological

PTSD Symptom Severity Associated With Sleep Disturbances in Military Personnel: Evidence From a Prospective Controlled Study With Ecological Recordings.

Sleep disturbances, including insomnia and trauma-related nightmares (TRNs), are the core symptoms of post-traumatic stress disorder (PTSD) in military personnel. Furthermore these nocturnal manifestations are directly related to the persistence of daytime PTSD symptoms and are known to exacerbate comorbid conditions such as depression, suicidality, and daytime impairments. This prospective study examined the variability of PTSD-related sleep disruptions and its relationship to symptom severity using ecological recordings over several nights. One hundred thirty PTSD-diagnosed service members a

Temporal link between cardiac arrhythmias and postoperative episodes of hypoxemia during nocturnal sleep in patients with obstructive sleep apnea syndrome.

OBJECTIVE: Obstructive Sleep Apnea (OSA) is known to impact morbidity in the perioperative period through a postoperative exacerbation of respiratory events after general anesthesia. Cardiac arrhythmias may be triggered by respiratory and/or hypoxic events, therefore we searched for a temporal link between cardiac arrhythmias and episodes of hypoxemia following surgery under general anesthesia during the nocturnal sleep phase. METHODS: We included patients with a preoperative STOP-BANG questionnaire score between 3 and 8, planned for an elective surgery with general anesthesia. Patients had a

The role of physical activity on obstructive sleep apnea severity and hypoxic load, and the mismatch between subjective and objective physical activity assessments.

Obesity is the primary risk factor for the development of obstructive sleep apnea, and physical inactivity plays an important role. However, most studies have either only evaluated physical activity subjectively or objectively in obstructive sleep apnea. The objectives of this study were: (i) to assess the relationship between obstructive sleep apnea severity (both apnea-hypopnea index and desaturation parameters) and both objectively and subjectively measured physical activity after adjustment for anthropometry and body composition parameters; and (ii) to assess the relationship between objec

Sleep patterns and risk of chronic disease as measured by long-term monitoring with commercial wearable devices in the All of Us Research Program.

Poor sleep health is associated with increased all-cause mortality and incidence of many chronic conditions. Previous studies have relied on cross-sectional and self-reported survey data or polysomnograms, which have limitations with respect to data granularity, sample size and longitudinal information. Here, using objectively measured, longitudinal sleep data from commercial wearable devices linked to electronic health record data from the All of Us Research Program, we show that sleep patterns, including sleep stages, duration and regularity, are associated with chronic disease incidence. Definition of diaphragmatic sleep disordered breathing and clinical meaning in Duchenne muscular dystrophy.

BACKGROUND: Diaphragmatic sleep disordered breathing (dSDB) has been recently identified as sleep dysfunction secondary to diaphragmatic weakness in Duchenne muscular dystrophy (DMD). However, scoring criteria for the identification of dSDB are missing. This study aimed to define and validate dSDB scoring criteria and to evaluate whether dSDB severity correlates with respiratory progression in DMD. METHODS: Scoring criteria for diaphragmatic apnoea (dA) and hypopnoeas (dH) have been defined by the authors considering the pattern observed on cardiorespiratory polygraphy (CR) and the dSDB pathop

Longitudinal effects of early exposure to intermittent hypoxia on autonomic cardiovascular control in very preterm infants.

INTRODUCTION: Cardiorespiratory control is immature in infants born preterm compared to those born at term. Animal studies have shown that repetitive hypoxia associated with periodic breathing can alter autonomic control. We aimed to elucidate if the amount of time spent with apnoea and periodic breathing in the neonatal unit was associated with longitudinal changes in autonomic control assessed using heart rate variability. METHODS: Twenty-nine very preterm infants (10 M 19F) were studied during supine daytime sleep on 4 occasions. Study 1: 32-36 weeks post menstrual age (PMA) (n = 29), Stud

Cone beam computed tomography changes upon oral appliance therapy for adult patients with obstructive sleep apnea: A non-randomized clinical trial.

BACKGROUND: Obstructive sleep apnea (OSA) is caused by narrowing or obstruction of the airway lumen at single or multiple levels of the airway, starting from the nasal cavity up to the larynx. Oral appliance therapy for the management of OSA is prescribed as an alternative treatment option for patients with mild to moderate OSA who fail to adhere to Continuous Positive Airway Pressure (CPAP) therapy. Treatment with oral appliances addresses the craniofacial deficiencies that cause OSA by providing means to mandibular advancement and palatal expansion, thus opening the airways and potentially p

The effect of pharyngeal structures on the severity of obstructive sleep apnea.

BACKGROUND/OBJECTIVE: Obstructive sleep apnea (OSA) is characterized by complete or partial cessation of breathing during sleep. The tongue is suggested as a possible anatomical site causing airway obstruction. However, the role of other pharyngeal structures in the development of OSA remains unclear. We designed a study using both the apnea-hypopnea index (AHI) and the oxygen saturation measurements to assess the severity of OSA. We aimed to identify critical anatomical structures of the upper airway that correlate with the severity of OSA and to evaluate the utility of magnetic resonance ima

Machine learning and geometric morphometrics to predict obstructive sleep apnea from 3D craniofacial scans.

BACKGROUND: Obstructive sleep apnea (OSA) remains massively underdiagnosed, due to limited access to polysomnography (PSG), the highly complex gold standard for diagnosis. Performance scores in predicting OSA are evaluated for machine learning (ML) analysis applied to 3D maxillofacial shapes. METHODS: The 3D maxillofacial shapes were scanned on 280 Caucasian men with suspected OSA. All participants underwent single night in-home or in-laboratory sleep testing with PSG (Nox A1, Resmed, Australia), with concomitant 3D scanning (Sense v2, 3D systems corporation, USA). Anthropometric data, comorb
