Backward joint model and dynamic prediction of survival with multivariate longitudinal data.

An important approach to dynamic prediction of time-to-event outcomes using longitudinal data is based on modeling the joint distribution of longitudinal and time-to-event data. The widely used joint model for this purpose is the shared random effect model. Presumably, adding more longitudinal predictors improves the predictive accuracy. However, the shared random effect model can be computationally difficult or prohibitive when a large number of longitudinal variables are used. In this paper, we study an alternative way of modeling the joint distribution of longitudinal and time-to-event data

Evaluation and Calibration of In Silico Models of Thrombin Generation Using Experimental Data from Healthy and Haemophilic Subjects.

The coagulation cascade comprises numerous chemical reactions between many proteins, that finally lead to the formation of a clot to stop bleeding. Many numerical models have attempted to translate understanding of this cascade into mathematical equations that simulate the chain reactions. However, their predictions have not been validated against clinical data stemming from patients. In this paper, we propose an extensive validation of five available models, by comparing in healthy and haemophilic subjects, thrombin generation measured in vitro to thrombin generation predicted by the models i

Harnessing gene expression to identify the genetic basis of drug resistance.

The advent of cost-effective genotyping and sequencing methods have recently made it possible to ask questions that address the genetic basis of phenotypic diversity and how natural variants interact with the environment. We developed Camelot (CAusal Modelling with Expression Linkage for cOmplex Traits), a statistical method that integrates genotype, gene expression and phenotype data to automatically build models that both predict complex quantitative phenotypes and identify genes that actively influence these traits. Camelot integrates genotype and gene expression data, both generated under

Utilization of non-invasive ventilation before prehospital emergency anesthesia in trauma - a cohort analysis with machine learning.

BACKGROUND: For preoxygenation, German guidelines consider non-invasive ventilation (NIV) as a possible method in prehospital trauma care in the absence of aspiration, severe head or face injuries, unconsciousness, or patient non-compliance. As data on the utilization and characteristics of patients receiving NIV are lacking, this study aims to identify predictors of NIV usage in trauma patients using machine learning and compare these findings with the current national guideline. METHODS: A cross-regional registry of prehospital emergency services in southwestern Germany was searched for cas

Digital auscultation as a novel childhood pneumonia diagnostic tool for community clinics in Sylhet, Bangladesh: protocol for a cross-sectional study.

INTRODUCTION: The WHO's Integrated Management of Childhood Illnesses (IMCI) algorithm for diagnosis of child pneumonia relies on counting respiratory rate and observing respiratory distress to diagnose childhood pneumonia. IMCI case defination for pneumonia performs with high sensitivity but low specificity, leading to overdiagnosis of child pneumonia and unnecessary antibiotic use. Including lung auscultation in IMCI could improve specificity of pneumonia diagnosis. Our objectives are: (1) assess lung sound recording quality by primary healthcare workers (HCWs) from under-5 children with the

The Effect of Implementation of the American Heart Association Mission Lifeline PreAct Algorithm for Prehospital Cardiac Catheterization Laboratory Activation on the Rate of "False Positive" Activations.

HYPOTHESIS: Emergency Medical Services (EMS) systems have developed protocols for prehospital activation of the cardiac catheterization laboratory for patients with suspected ST-elevation myocardial infarction (STEMI) to decrease first-medical-contact-to-balloon time (FMC2B). The rate of "false positive" prehospital activations is high. In order to decrease this rate and expedite care for patients with true STEMI, the American Heart Association (AHA; Dallas, Texas USA) developed the Mission Lifeline PreAct STEMI algorithm, which was implemented in Los Angeles County (LAC; California USA) in 20

Albumin-corrected anion gap as a predictor of 28-day mortality in acute respiratory distress syndrome: A machine learning-based retrospective study.

BACKGROUND: Acute Respiratory Distress Syndrome (ARDS) remains a critical condition associated with high mortality rates, prolonged hospitalization, and reduced quality of life despite advances in critical care. The albumin-corrected anion gap (ACAG), an emerging biomarker reflecting acid-base disturbances, has been linked to poor outcomes in various critical illnesses. However, its prognostic value for mortality in ARDS patients remains unexplored. METHODS: This retrospective study analyzed data from ARDS patients admitted to intensive care units (ICUs) in the MIMIC-IV database. Patients wer

Advancing breast ultrasound diagnostics through hybrid deep learning models.

Today, doctors rely heavily on medical imaging to identify abnormalities. Proper classification of these abnormalities enables them to take informed actions, leading to early diagnosis and treatment. This paper introduces the "EfficientKNN" model, a novel hybrid deep learning approach that combines the advanced feature extraction capabilities of EfficientNetB3 with the simplicity and effectiveness of the k-Nearest Neighbors (k-NN) algorithm. Initially, EfficientNetB3, pre-trained on ImageNet, is repurposed to serve as a feature extractor. Subsequently, a GlobalAveragePooling2D layer is applied
