Placental adaptations to micronutrient dysregulation in the programming of chronic disease.

Poor nutrition during pregnancy is known to impair foetal development and increase the risk of chronic disease in offspring. Both macronutrients and micronutrients are required for a healthy pregnancy although significantly less is understood about the role of micronutrients in the programming of chronic disease. This is despite the fact that modern calorie rich diets are often also deficient in key micronutrients. The importance of micronutrients in gestational disorders is clearly understood but how they impact long term disease in humans requires further investigation. In contrast, animal s

[Pulmonary embolism in pregnancy].

Pulmonary embolism (PE) is very rarely observed during pregnancy and postpartum period. The frequency of PE in pregnant women is relatively difficult to assess because the existing data estimate all venous-thromboembolic (VTE) complications including deep venous thrombosis (DVT) and PE. The incidence of PE itself has been evaluated on 3 cases per 10000 deliveries. Diagnostics of PE during pregnancy is very difficult and requires unequivocal confirmation or exclusion because of exposure of fetus to potential complications of antithrombic therapy. Very important is to assess the clinical probabi

Factors Influencing the Incidence of Pre-term Birth in Calabar, Nigeria.

Women who had pre-term birth in the University of Calabar Teaching Hospital, Calabar, over a two and a half year period were studied. The aim was to establish the factors influencing the incidence of pre-term birth in Calabar. Factors which significantly increase the incidence of pre-term delivery included: previous induced abortion [P < 0.0001], nulliparity [P < 0.001], out of wedlock birth [P < 0.05] and lack of antenatal care [P < 0.01]. Women with multiple pregnancy [P < 0.001] or previous pre-term delivery [P < 0.01], have a significantly high risk for pre-term birth. Antenatal complicati

General strategy for the management of bronchial asthma in pregnancy.

Epidemiological studies showed that bronchial asthma is one of the most common diseases which can complicate pregnancy (1-7%). In about 0.05-2% of the cases, asthma occurs as a life-threatening event. In the common medical practice a waiting strategy or, even, the complete refusal for drug therapies are frequently observed. This is justified by the fear of the possible adverse effects of drugs on developing fetus. On the contrary, several studies have demonstrated that severe and uncontrolled asthma may produce serious maternal and fetal complications, such as gestational hypertension and ecla

Maternal plasma TIMP-4 levels combined with clinical risk factors for the early prediction of pregnancy-induced hypertension.

OBJECTIVE: The objective of this study was to create a model for early predicting pregnancy-induced hypertension (PIH) using plasma markers and clinical risk factors. METHODS: A nested case-control study was performed at the Laboratory Department of Guangzhou Women and Children's Medical Center. From a prospective cohort of tens of thousands of unselected women with singleton pregnancies at 8-20 weeks gestation, maternal plasma samples were obtained from 73 women who subsequently developed PIH (PIH group) and 146 gestational age- and maternal age-matched women with normotensive pregnancies (c

Maternal snuff use and smoking and the risk of oral cleft malformations--a population-based cohort study.

OBJECTIVE: To determine if maternal use of snuff (containing high levels of nicotine, low levels of nitrosamines and no combustion products) is associated with an increased risk of oral cleft malformations in the infant and whether cessation of snuff use or smoking before the antenatal booking influences the risk. METHOD: A population-based cohort study was conducted on all live born infants, recorded in the Swedish Medical Birth Register from 1999 through 2009 (n = 1 086 213). Risks of oral clefts were evaluated by multivariate logistic regression analyses (using adjusted odds ratios, with 9

Prevalence of depression and associated factors among critically ill pregnant women in Palestine.

BACKGROUND: Depression during pregnancy poses significant challenges for both the mother and fetus, especially in cases where pregnancy complications become life-threatening. Recognizing the prevalence and risk factors associated with prenatal depression in critically ill pregnant women is essential, particularly in resource-limited settings like Palestine. METHODS: A cross-sectional study was conducted among 304 critically ill pregnant women in eight Palestinian hospitals between January and March 2024. Data were collected through the Clinically Useful Depression Outcome Scale (CUDOS) and an

Pregnancy and lupus nephritis in developing countries: A systematic review.

Systemic lupus erythematosus (SLE) and lupus nephritis (LN) have a significant impact on the course of pregnancy, as well as on maternal and fetal outcomes. LN in pregnancy can increase the maternal risks of SLE flare, acute kidney injury, preeclampsia, and even death. It also affects fetal outcomes by the increased risk of intrauterine growth retardation, premature delivery, and fetal loss. Successful pregnancy outcomes have been well documented in the developed world, but less is known about patients in developing nations. [Sexual violence in Congo-Kinshasa: necessity of decriminalizing abortion].

The sexual violence's committed in the Democratic Republic of Congo (DRC) are from their scales and consequences on women, real public health, politico-legal, and socio-economical challenges. More than a million of women have been victims of sexual violence on a period of less than fifteen years. Systematic rapes of women were used as war weapon by different groups involved in the Congolese war. Sexual violence against women has impacted public health by spreading sexually transmissible diseases including HIV/AIDS, causing unwanted pregnancies, leading to the gynaecological complications of ra

[Pregnancy, contraception and HRT and venous thromboembolism].

Endogeneous and exogeneous hormonal factors may favor venous thromboembolism (VTE) in the woman. Pregnancy and post-partum, combined contraception and hormonal replacement therapy (HRT) of menopause are associated with an increased risk of VTE. Even when the relative risk is statistically significant, the absolute risk remains generally low unless additional factors are present. The occurrence of a thrombosis depends on the conditions associated to these risk factors: multiparity, caesarean section, type of contraception or HRT, and also on risk factors associated to the woman such as age, obe

[Groin hernia in female patients: sex-specific management].

Groin hernia in women is an entity whose clinical manifestations can be quite subtle, and which therefore risks to go unnoticed. Imaging studies by ultrasound or dynamic CT/MRI is strongly recommended in case of a clinical doubt. Optimal treatment consists of laparoscopic surgery, even in oligosymptomatic patients, because the risk of incarceration, and subsequent morbidity and mortality is high especially in cases of femoral hernia, which are frequent and often overlooked in women. During pregnancy, the risk of developing a groin hernia is very low (around 0,1%) and a wait-and-see attitude is

Prenatal co-exposure to phthalate metabolites and bisphenols among non-syndromic cleft lip and/or palate in offspring.

Phthalate metabolites and bisphenols can cause adverse pregnancy outcomes. However, there is no study to evaluate the associations of prenatal exposure to phthalate metabolites and bisphenols with non-syndromic cleft lip and/or palate (NSCL/P) risk in offspring. A population-based case-control study was conducted in a multicenter setting from 2005 to 2021, enrolling 448 pregnant women. Seven phthalate metabolites and six bisphenols were quantified in placenta using liquid chromatography-tandem mass spectrometry. In the logistic regression analysis, high levels of mono-ethyl phthalate, mono-cyc

Reducing the burden of surveillance in pregnant women with no history of fetal atrioventricular block using the negative predictive value of anti-Ro/SSA antibody titers.

BACKGROUND: The risk of fetal atrioventricular block in anti-Ro/SSA antibody-exposed pregnancies with no previous affected offspring is approximately 2%. A high antibody titer is necessary but not sufficient for atrioventricular block, and specific antibody titers do not predict risk. However, there are no data on the negative predictive value of antibody titer to identify pregnancies at low risk of fetal atrioventricular block, and may not require surveillance. OBJECTIVE: This study aimed to define anti-Ro52 and anti-Ro60 antibody thresholds for the identification of fetuses unlikely to deve

Pregnancy & cardiovascular disease: the PREG-CVD-HH registry.

BACKGROUND: Cardiovascular disease (CVD) remains the leading cause of death in pregnant and peripartal women in western countries. Physiological changes during pregnancy can lead to cardiovascular complications in the mother; women with pre-existing heart disease may not tolerate these changes well, increasing their susceptibility to adverse cardiovascular outcomes during pregnancy. The aim of this study is to characterize pregnancy-induced changes in cardiac function, biomarker concentrations and cardiovascular outcomes in women with CVD during pregnancy at a tertiary care hospital in Germany

Mode-of-Delivery-Specific Nomograms for Predicting Stress Urinary Incontinence One Year After Childbirth.

INTRODUCTION AND HYPOTHESIS: Postpartum stress urinary incontinence (SUI) is a common pelvic floor disorder that impairs quality of life. Early identification of high-risk women is important for targeted interventions; however, prediction tools specific to delivery mode remain limited. METHODS: This retrospective cohort study enrolled women who underwent vaginal delivery (VD) or cesarean section (CS) at a single center. Two delivery-mode-specific prediction models for SUI at 1 year postpartum were developed using LASSO regression followed by multivariable logistic regression. Independent pred
