Managing hand-foot-mouth disease in children: More of counseling, less of medicines.

Hand, foot, and mouth disease (HFMD) is a clinical syndrome characterized by a febrile illness, followed by an oral exanthema and a macular, maculopapular, or vesicular rash of the hands, also involving palms, feet, and buttocks caused by enteroviruses of the picornaviridae family. It is a benign self-limiting disease which spreads by fecal-oral, oral-oral, and respiratory droplet contact. This case series describes cases of HFMD, presenting with typical clinical features in the North Indian region where early diagnosis and management of the condition can be carried out to halt the disease pro

Self-Managed Medication Abortion: Implications for Clinical Practice.

UNLABELLED: Medication abortion represents more than 50 percent of abortions in the United States (US). Since its approval in the US in 2000, the Food and Drug Administration (FDA) has progressively relaxed the prescribing requirements such that currently, no office visit, in-person dispensing, or ultrasound is required. Obtaining medication for abortion online without medical supervision or evaluation is also possible. This article reviews the complications of medication abortion by examining major studies and delineates the risks specific to self-managed abortion to inform clinicians in cari

Current Status of Primary, Secondary, and Tertiary Prevention of Coronary Artery Disease.

Fifty percent of all death from cardiovascular diseases is due to coronary artery disease (CAD). This is avoidable if early identification is made. Preventive health care has a major role in the fight against CAD. Atherosclerosis and atherosclerotic plaque rupture are involved in the development of CAD. Modifiable risk factors for CAD are dyslipidemia, diabetes, hypertension, cigarette smoking, obesity, chronic renal disease, chronic infection, high C-reactive protein, and hyperhomocysteinemia. CAD can be prevented by modification of risk factors. This paper defines the primary, secondary, and

Methods for assessing the severity of perinatal asphyxia and early prognostic tools in neonates with hypoxic-ischemic encephalopathy treated with therapeutic hypothermia.

Despite the progress in perinatal care, perinatal asphyxia (PA) remains a significant problem in neonatology. The development of therapeutic hypothermia (TH) has improved the prognosis, but it still remains uncertain in hypoxic neonates. The evaluation of the severity of ischemia/hypoxia after birth is crucial to the choice of treatment, and with accurate long-term prognosis, appropriate further patient care can be planned. This article presents various methods for the preliminary assessment of brain damage and prognosis in newborns with PA treated with TH. The importance of assessing the neur

Review of Cystic Fibrosis.

Cystic fibrosis (CF) is an autosomal recessive disease characterized by pancreatic insufficiency and chronic endobronchial airway infection. This latter feature results in progressive bronchiectasis and ultimately respiratory failure, which is the leading cause of death in patients with CF. Other complications include sinusitis, diabetes mellitus, bowel obstruction, hepatobiliary disease, hyponatremic dehydration, and infertility. Diagnosis of CF is confirmed by demonstration of elevated sweat chloride. Most cases of CF are identified through newborn screening (NBS). There are also infants wit

[Pyoderma gangrenosum: a report of 3 cases].

OBJECTIVE: To improve the surgeon's understanding of pyoderma gangrenosum (PG) and avoid misdiagnosis and incorrect treatment. METHODS: Three cases of PG managed in the department of general surgery during the past 10 years were retrospectively reviewed. RESULTS: All of the 3 cases mainly presented with necrosis of skin and soft tissues, followed by formation of painful and extensive creeping ulcer. They all had fever and were initially diagnosed and managed as "infection", but repeated wound exudates culture showed negative results and antimicrobial therapy was not effective. The ulcers wer

Taking care of patients with recessive dystrophic epidermolysis bullosa from birth to adulthood: a multidisciplinary Italian Delphi consensus.

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is a rare and severe mucocutaneous fragility disorder due to mutations in the COL7A1 gene encoding collagen VII, the major constituent of anchoring fibrils essential for epithelial adhesion. RDEB is characterized by unremitting blistering, chronic painful wounds and fibrotic scarring that results in hand and foot pseudosyndactyly, microstomia, and esophageal strictures. RDEB complications include nutritional compromise, chronic anemia, failure to thrive, delayed puberty, osteoporosis, and renal involvement. In addition, early onset

Outcomes in KEYNOTE-355 after chemotherapy discontinuation before pembrolizumab discontinuation and with immune-mediated adverse events.

Pembrolizumab plus chemotherapy significantly improved progression-free survival (PFS) and overall survival (OS) versus chemotherapy in the phase 3 randomized KEYNOTE-355 study in participants with previously untreated, advanced triple-negative breast cancer (TNBC) with PD-L1 combined positive score (CPS) ≥ 10. Among participants with clinical benefit from pembrolizumab plus chemotherapy, discontinuation of chemotherapy before pembrolizumab for any reason was associated with similar median PFS and OS to that in the overall population. Metoclopramide-Induced Akathisia Mimicking Seizure With Transient Lactic Acidosis.

Metoclopramide is a commonly used antiemetic associated with extrapyramidal side effects, including akathisia. Although typically mild, severe presentations may mimic neurologic emergencies. We present an 81-year-old male with chronic obstructive pulmonary disease (COPD) admitted for respiratory syncytial virus (RSV) pneumonia who developed acute severe akathisia after a single dose of metoclopramide. The episode began with acute back pain followed by marked restlessness and nonrhythmic movements with preserved awareness, favoring a toxic-metabolic process over seizure. Laboratory evaluation r

Symptoms, Causes, and Treatment of Sarcoidosis.

Sarcoidosis is a heterogeneous, multisystem granulomatous disorder of unknown etiology that predominantly affects adults aged 20-60 years. Clinical presentation ranges from asymptomatic disease detected incidentally to progressive organ dysfunction. The lymphatic system, lungs, and skin are most commonly involved, although virtually any organ may be affected. Diagnosis remains challenging due to the absence of a pathognomonic test and significant overlap with infectious, autoimmune, and neoplastic conditions, necessitating careful evaluation to exclude alternative etiologies. Accordingly, sarc

Colombian protocol for the hospital management of non-critically ill patients with suspected or confirmed yellow fever.

Yellow fever (YF) continues to cause high lethality in Colombia; timely, standardized care is critical. This guideline consolidates current evidence and frontline experience to support hospital management of non-critically ill patients with YF. It introduces an operational five-group classification (A, B1, B2, C1, C2) to guide level of care, monitoring, and escalation, complemented by NEWS2 and West Haven scores to help judgment. Recommendations cover initial assessment; laboratory tests and imaging; restriction of fluids; bleeding risk mitigation with transfusion thresholds; avoidance of hepa

Therapeutic Considerations in Preventing Chronic Kidney Disease.

Chronic kidney disease (CKD) affects 35.5 million US adults, but most patients are unaware of their diagnosis. Screening for CKD at-risk individuals is required, as symptoms do not appear until advanced stages. The combination of urine albumin-to-creatinine ratio and estimated glomerular filtration rate permits the classification of CKD stages and the determination of risk of CKD progression and cardiovascular disease, which is the most common cause of death in CKD. Cardiovascular-kidney-metabolic syndrome highlights the complex interplay between the heart, kidney, and metabolic disorders, suc

A Combat Journey With Pulmonary Rehabilitation and Palliative Care in a Patient With Pleural Effusion, Secondary to Metaplastic Breast Carcinoma.

Metaplastic breast cancer is an uncommon and fatal condition. It is described histologically as a tumor having epithelial differentiation into squamous and mesenchymal components, with numerous elements frequently co-existing in the same tumor. This case study sought to provide analgesic treatment in a case with malignant pleural effusion related to breast cancer based on evidence. A 67-year-old female with a known history of metaplastic breast carcinoma came to the tertiary care hospital with complaints of breathlessness which progressed to grade II on the Modified Medical Research Council (m

Spinal muscular atrophy: A changing phenotype beyond the clinical trials.

Spinal muscular atrophy is a monogenic, progressive motor neuron disorder caused by deletion or mutation in the SMN1 gene. A broad range of phenotypic severity, from very weak infants (Type 1) to ambulant children (type 3), is modified mainly by the number of copies of the "backup" SMN2 gene. Since the discovery of the role of both genes, basic research into the pathobiology of SMA, with in vitro and animal model studies, has identified therapeutic targets. Development of clinical outcome measures, natural history studies and standard of care guidelines have contributed to the development of p

Rationale, design and clinical performance of the Superion® Interspinous Spacer: a minimally invasive implant for treatment of lumbar spinal stenosis.

Lumbar spinal stenosis is a progressive degenerative condition that manifests as low back pain with neurogenic claudication as a cardinal clinical feature. Although mild radicular symptoms can often be successfully treated with conservative care, management of lumbar spinal stenosis grows increasingly difficult as symptoms worsen. No satisfactory nonsurgical treatments exist to manage moderate radicular symptoms and, therefore, these patients are faced with the decision of continuing ineffective conservative options or opting to undergo invasive decompressive spine surgery.
