The Role of Interventional Endoscopic Ultrasound in Liver Diseases: What Have We Learnt?

Chronic liver disease (CLD) is still a major problem, where the disease progression will lead to liver cirrhosis (LC) or hepatocellular carcinoma (HCC). Portal hypertension (PH) management and loco-regional therapy for HCC have become the cornerstones in advanced liver disease management. Recently, there are studies looking at the potential role of interventional endoscopic ultrasound (EUS) in liver diseases. EUS may be useful in vascular changes of the digestive wall evaluation, performing dynamic assessment of hemodynamic changes, predicting variceal bleeding and rebleeding risk, and assessi

Colonic metastasis of hepatocellular carcinoma with repeated retroperitoneal bleeding: a case report.

BACKGROUND: Colonic metastasis is uncommon in patients with hepatocellular carcinoma (HCC). In the past, extrahepatic metastasis of HCC was not treated aggressively because of its poor prognosis. Herein, we describe the case of a patient with HCC who survived for 30 months following resection of a metastatic tumor in the ascending colon. CASE PRESENTATION: An 80-year-old man presented at our hospital with symptoms of abdominal pain on the right side and fever. He had undergone transcatheter arterial chemoembolization and posterior segment resection of the liver because of HCC, followed by rad

Transient ventricular arrhythmia as a rare cause of dizziness during exercise: A case report.

BACKGROUND: Dizziness is a common symptom in adults and usually due to peripheral causes affecting semicircular canal function or central causes affecting the pons, medulla, or cerebellum. Arrhythmia is a recognized cause of dizziness in people with structural or ischemic heart disease. We report a case of exercise-induced transient ventricular tachycardia and dizziness in a man with no evidence of organic heart disease. CASE SUMMARY: A 42-year-old man presented with a 6 mo history of transient exercise-induced dizziness and prodromal palpitations. The patient was otherwise asymptomatic. Phys

Multidisciplinary Approach to the Treatment of Advanced Hepatocellular Carcinoma in the Era of New Biologic Agents.

With recent advances in systemic therapy, an increasing number of patients with advanced hepatocellular carcinoma (HCC) are expected to benefit from surgery. However, given the complex background of the disease and frequent presence of underlying liver injury, treatment of advanced HCC is complex and the treatment principle applied to colorectal liver metastases, for which conversion surgery has been actively performed, is often not applicable to patients with HCC. To maximize the survival outcomes of patients with HCC, optimization of each step of treatment through a multidisciplinary approac

Association of apelin and AF in patients with implanted loop recorders undergoing catheter ablation.

BACKGROUND: Previous studies showed an association between apelin and atrial fibrillation (AF). The aim of this study was to analyse the effect of pulmonary vein isolation (PVI) in patients with paroxysmal AF on plasmatic apelin concentrations. METHODS: Nine consecutive patients (aged from 43 to 69 years, 3 females and 6 males) with documented paroxysmal atrial fibrillation and implanted loop recorders (ILR) for continuous ECG monitoring were included in this study. All the patients underwent a radiofrequency catheter ablation with PVI. RESULTS: The plasmatic concentration of apelin increase

A novel gain-of-function mutation in SCN5A responsible for multifocal ectopic Purkinje-related premature contractions.

Recently, four SCN5A mutations have been associated with Multifocal Ectopic Purkinje-related Premature Contractions (MEPPC), a rare cardiac syndrome combining polymorphic ventricular arrhythmia with dilated cardiomyopathy (DCM). Here, we identified a novel heterozygous mutation in SCN5A (c.611C>A, pAla204Glu) in a young woman presenting with polymorphic premature ventricular contractions (PVCs) and DCM. After failure of antiarrhythmic drugs and an attempt of radiofrequency catheter ablation showing three exit-sites of PVCs, all with presystolic Purkinje potentials, a treatment by hydroquinidin

[RIGHT VENTRICULAR ARRHYTMOGENIC CARDIOMYOPATHY – HAVE WE AVOIDED A FAMILY TRAGEDY BY APPLYING CONTEMPORARY DIAGNOSTIC AND TREATMENT APPROACH?].

Arrhytmogenic right ventricular cardiomyopathy (ARVC) is an inherited cardiomyopathy characterized by fibrofatty replacement, dominantly in the right, but often also in the left ventricle. It is a significant cause of sudden death in children and adolescents. A thorough family history and contemporary diagnostic and treatment approach are prerequisites for the prevention of the sudden death syndrome. The diagnosis is more often established in adults than in children. Patients: We present a family with four members affected and diagnosed with ARVC already in childhood/adolescence. The average a

Inhibition of Angiofibromas in a Tuberous Sclerosis Patient Using Topical Timolol 0.5% Gel.

Angiofibromas are the most recognized cutaneous manifestations of tuberous sclerosis complex. Angiofibromas can be associated with disfigurement, bleeding, pruritus, and erythema and may lead to significant psychosocial consequences for affected patients. Histopathologically, angiofibromas consist of a mixture of vascular and interstitial cells. Traditional treatment options include cryotherapy, electrocoagulation, radiofrequency ablation, dermabrasion, laser treatment, and topical agents such as podophyllotoxin. However, complications such as pain, postinflammatory hyperpigmentation, scarring

Hepatectomy for hepatocellular carcinoma in patients with hemophilia.

BACKGROUND: Hepatitis C virus infection due to contamination in large-pool clotting factor concentrate is a major co-morbidity in adult patients with hemophilia. Although the development of hepatocellular carcinoma following infection with hepatitis virus is a crucial health problem, there are few reports of hemophilia patients undergoing liver resection for hepatocellular carcinoma. METHODS: Six patients with hemophilia underwent liver resection for hepatocellular carcinoma in our department from January 2003 to December 2012. We compared these hemophilic patients (H group, n = 6) to control

Gastric ulcer after prophylactic balloon-occluded retrograde transvenous obliteration.

A 74-year-old man with compensated hepatitis C virus-related liver cirrhosis was admitted for the treatment of small hepatocellular carcinoma (HCC) by radiofrequency ablation therapy (RFA). As a routine pretreatment examination, gastrointestinal endoscopy was performed, and large nodular varices were observed in the gastric fornix, with telangiectasia on top of the varices. As soon as the RFA was completed, prophylactic balloon-occluded retrograde transvenous obliteration (B-RTO) was performed. Seven days after the B-RTO, the patient complicated of upper abdominal pain. Gastrointestinal endosc

Atrial fibrillation: insights from clinical trials and novel treatment options.

Atrial fibrillation (AF) is the most common encountered sustained arrhythmia in clinical practice. The last decade the result of large 'rate' versus 'rhythm' control trials have been published that have changed the current day practise of AF treatment. It has become clear that rate control is at least equally effective as a rhythm control strategy in ameliorating morbidity as well as mortality. Moreover, in each individual patient the risk of thromboembolic events should be assessed and antithrombotic treatment be initiated. There have also been great advances in understanding the mechanisms o

Arrhythmogenic right ventricular dysplasia/cardiomyopathy: screening, diagnosis, and treatment.

Arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) is a heart muscle disorder characterized pathologically by fatty or fibrofatty replacement and electrical instability of the right ventricular myocardium. Clinical manifestations include structural and functional malformations (fatty infiltration, dilatation, aneurysms) of the right ventricle, ECG abnormalities, and presentation with ventricular tachycardias with left bundle branch block pattern or sudden death. The disease often is familial with an autosomal inheritance. The typical hallmarks of ARVD/C are distributed in the s

Thyroid metastases from colorectal cancer: the Institut Gustave Roussy experience.

The prevalence of thyroid metastases in colorectal cancer (CRC) patients is unknown. We retrieved the records of all patients with CRC and pathologically proved thyroid metastasis for the period 1993-2004. Among 5,862 consecutive patients with CRC, 6 (0.1%) were diagnosed with thyroid metastases, a median of 61 months after the diagnosis of primary tumour, and a median of 19 months after the last surgical resection or radiofrequency ablation of other metastases (which were present in all cases). Signs and symptoms, when present (n=3), consisted of cervical pain, cervical adenopathy, goitre, dy

Excision of rhinophyma with high-frequency electrosurgery.

BACKGROUND: Rhinophyma is an uncommon progressive disfiguring process of the lower nasal region that most commonly affects middle-aged white men. It is characterized by painless hyperplasia of the sebaceous and subcutaneous tissues, in the end stage, resulting in a bulbous nodular telangiectatic nose. Surgical modalities of treatment include electrosurgery, laser ablation, dermabrasion, cryosurgery, scalpel excision, and others. OBJECTIVE: We present two patients with rhinophyma who were treated with the Ellman Surgitron using high-frequency current. METHODS: Two patients with significant rh

Supraventricular tachycardia in the neonate and infant.

Supraventricular tachycardia (SVT) is the most common sustained arrhythmia to present in the neonatal and infancy age group. Predisposing factors (congenital heart disease, drug administration, illness and fever) occur only in 15% of infants. The presentation of SVT in the neonate is frequently subtle, and may include pallor, cyanosis, restlessness, irritability, feeding difficulty, tachypnea, diaphoresis and grunting. Congestive heart failure is more common in infants under 4 months of age (35% incidence). Age-related differences in the distribution of SVT mechanisms occur in different age gr

Nonoperative Management of Hepatic Adenomas: A Review.

Hepatic adenomas (HAs) are relatively common benign liver tumors that carry small risks of hemorrhage and malignant transformation, both of which increase with tumor size. While traditionally linked to estrogen-containing oral contraceptives (OCPs), other risk factors including obesity have been associated with HA formation and growth. Treatment strategies depend on the risk of malignant transformation or hemorrhage, tumor size, histological subtype, and patient-specific factors. While traditionally, surgical resection has been the standard recommendation for large, symptomatic, or high-risk t

Case report: CMR work-up for a case of long QT syndrome, dilated left ventricle, and hypertension-when incidental findings unmasked the fundamental issue.

BACKGROUND: Cardiac magnetic resonance imaging (CMR) is increasingly used for detailed evaluation of cardiac structures and function. Its wide field of view also allows identification of extracardiac abnormalities, some of which may significantly impact clinical management. CASE SUMMARY: We present a case of a 44-year-old woman referred for evaluation of prolonged QT interval and hypertension. Investigations revealed mild left ventricular dilatation, and suspicion of long QT syndrome (LQTS). CMR clarified the underlying cardiac abnormalities, identifying a patent ductus arteriosus (PDA) as th

Evolving techniques in the endoscopic evaluation and management of pancreas cystic lesions.

PURPOSE OF REVIEW: Accurate diagnosis of pancreatic cystic lesions (PCLs) is essential to guide appropriate management and reduce unnecessary surgeries. Despite multiple guidelines in PCL management, a substantial proportion of patients still undergo major resections for benign cysts, and a majority of resected intraductal papillary mucinous neoplasms (IPMNs) show only low-grade dysplasia, leading to significant clinical, financial, and psychological burdens. This review highlights emerging endoscopic approaches that enhance diagnostic accuracy and support organ-sparing, minimally invasive man

Fire or ice - emerging therapies for unresectable pulmonary nodules.

IMPORTANCE: Lung cancer and pulmonary metastases remain the leading cause of cancer-related mortality. While surgery is the gold standard, local tissue-ablative therapies are increasingly used in unresectable disease or inoperable patients. Minimally invasive local control presents a safe, repeatable approach to slow disease progression and prolong survival. OBSERVATIONS: Ablative techniques (radiofrequency, microwave, cryoablation, pulsed electric field) utilize extreme heat, cold, or energy to induce cancer cell death. Traditionally, ablation occurred percutaneously with image guidance, but
