One-stage reconstruction of composite extremity defects with a sural neurocutaneous flap and a vascularized fibular graft: a novel chimeric flap based on the peroneal artery.

BACKGROUND: The fibula flap has been widely used for reconstruction of composite bone and soft-tissue defects. The skin paddle of the fibula flap has played a critical role in providing a cutaneous component and good monitor for the bone component. In this report, the authors designed a perforator-based sural neurocutaneous flap instead of a peroneal perforator flap to enlarge a skin paddle for wound coverage. METHODS: A novel chimeric flap was harvested based on the peroneal artery, and consisted of a sural neurocutaneous flap and a fibular graft. The sural neurocutaneous flap was elevated b

In vivo kinematics of the scaphoid, lunate, capitate, and third metacarpal in extreme wrist flexion and extension.

PURPOSE: Insights into the complexity of active in vivo carpal motion have recently been gained using 3-dimensional imaging; however, kinematics during extremes of motion has not been elucidated. The purpose of this study was to determine motion of the carpus during extremes of wrist flexion and extension. METHODS: We obtained computed tomography scans of 12 healthy wrists in neutral grip, extreme loaded flexion, and extreme loaded extension. We obtained 3-dimensional bone surfaces and 6-degree-of-freedom kinematics for the radius and carpals. The flexion and extension rotation from neutral g

The benefit of magnetic resonance imaging for patients with posttraumatic radial wrist tenderness.

PURPOSE: To describe fractures revealed by magnetic resonance imaging (MRI) in a series of skeletally mature patients with radial wrist pain after an acute injury and clinically suspected to have a scaphoid fracture. Additionally, we attempted to assess the diagnostic value of radiographs and computed tomography (CT) in patients with scaphoid and other carpal fractures verified by MRI. METHODS: We conducted the study prospectively over a 3-year period on skeletally mature patients presenting at our emergency department with tenderness on the radial side of the wrist after an injury. A total o

Radiographic analysis of polyethylene glenoid components using modern cementing techniques.

The purpose of this study was to examine the initial radiographic appearance and changes occurring over time in patients who have undergone total shoulder arthroplasty by modern methods of bone preparation and current cement techniques. Sixty-five patients underwent seventy total shoulder arthroplasties by use of a cemented all-polyethylene, keeled glenoid component. The surface radius was equal to the radius of a one-piece humeral component. The mean clinical follow-up was 4.18 years (range, 2-8.6 years); radiographic follow-up averaged 3.9 years (range, 2-8.6 years). Three observers evaluate

Field-specific visual-evoked potentials: identifying field defects in vigabatrin-treated children.

OBJECTIVE: To derive a visual-evoked potential (VEP) technique for identifying visual field defects in children with epilepsy treated with vigabatrin and unable to perform perimetry. BACKGROUND: Studies have linked vigabatrin to a specific pattern of visual field loss. Few studies have included the pediatric population because of difficulties in assessing the visual field by perimetry below a developmental age of 9 years. METHODS: A field-specific VEP was developed with a central (0 degrees to 5 degrees radius) and peripheral stimulus (30 degrees to 60 degrees radius). Stimuli consisted of b

Changes in patterns of left ventricular hypertrophy after aortic valve replacement for aortic stenosis and regurgitation with St. Jude Medical cardiac valves.

In this study, we analyzed the extent and pattern of regression of left ventricular (LV) hypertrophy after aortic valve replacement in patients with aortic stenosis (AS) and compared the results with those of another group of patients with aortic regurgitation (AR). Seventy patients who underwent isolated aortic valve replacement were divided into 2 groups. Group 1 was comprised of 29 patients who underwent aortic valve replacement for aortic stenosis, and Group 2 of 41 patients who underwent aortic valve replacement for aortic regurgitation. A third group of 10 healthy subjects served as a he

Weight-bearing exercise and ground reaction forces: a 12-month randomized controlled trial of effects on bone mineral density in healthy postmenopausal women.

The effects of brief daily exercise on bone mineral density (BMD) were assessed in a randomized controlled trial in 44 healthy postmenopausal women using weight-bearing exercise in a regimen adapted from osteogenic protocols reported in animal studies. BMD was assessed masked using dual energy X-ray absorptiometry at 0, 6, and 12 months. The sites assessed were the proximal femur (neck, Ward's triangle, and trochanter) and the lateral spine (L2-3) to assess the effects of the exercise, and the radius (ultradistal and 33% distal) as a marker for systemic effects. The test group was required to

Pattern evoked potentials in human albinism. Evidence of two different topographical asymmetries reflecting abnormal retino-cortical projections.

In fifteen subjects with oculo-cutaneous or ocular albinism pattern-reversal visual evoked potentials (VEP) were recorded to monocular whole-field (16 degrees radius) and right and left vertical half-field (16 degrees radius) stimulation from an array of occipital electrodes. In all 15 subjects the VEP was of low amplitude and the monocular response to whole-field stimulation showed abnormal asymmetry in scalp topography. This asymmetry was similar to that produced by stimulation of the temporal half-field in the same eye. Two distinct types of VEP asymmetry were readily identified in differen

Restoring rotational balance of the paediatric forearm: lessons from post-traumatic malunion.

INTRODUCTION: Post-traumatic malunion of paediatric forearm fractures may lead to impairment of pronation and supination. Treatment decisions are traditionally based on radiographic alignment, although functional limitations do not always correlate with deformity on plain radiographs. Understanding the interaction between bony deformity, soft tissue contracture and growth related remodelling is essential for restoring rotational balance of the forearm.Current concepts:Impaired forearm rotation may result from angular or rotational malalignment, loss of the radial bow or soft tissue contracture

Structural balance restoration in complex forearm problems using 3D technology.

INTRODUCTION: The human forearm is a finely tuned system in which the radius and ulna act as an interdependent pair to enable stability, mobility and efficient load transfer. Disruption through trauma, malunion, congenital deformity or failed surgical intervention produces imbalance, presenting with restricted motion, instability, pain, and functional loss. Advances in three-dimensional (3D) imaging, planning, and printing now allow for more accurate analysis and more precise correction of forearm deformities.Reconstruction:Restoration of bone morphology alone does not ensure functional recove

[Management of forearm shaft fractures : Challenges and solution approaches].

Forearm shaft fractures are the most common fractures of the upper extremity in young adults. By definition, these fractures are diaphyseal fractures; however, due to the complex functional unity formed by the forearm shaft during motion both bone forearm fractures are treated as intra-articular fractures [1, 3]. This is why the gold standard of treatment in adults is osteosynthesis. The aim of the surgical intervention is the anatomical reduction with exact reconstruction of length, shaft axis and rotation. This particularly concerns Monteggia and Galeazzi fractures where the adjacent joint n

[Translated article] Induced osteogenesis by chitosan (3-glycidoxypropyl)trimethoxysilane and silica-based biomaterial in rabbits.

INTRODUCTION: Synthetic biomaterials obtained through tissue engineering offer an alternative to the use of autologous and heterologous grafts for the repair of bone defects of various etiologies, although the ideal material for this purpose has not yet been developed. A gel based on chitosan (CS) (3-glycidoxypropyl)trimethoxysilane (GPTMS), and silica have shown efficacy in experimental studies from a biomechanical and in vitro osseointegration perspective. OBJECTIVE: To demonstrate that the hybrid aerogel based on CS-GPTMS-silica is effective and safe for treating bone defects in load-beari

The osteocutaneous radial forearm free flap: A multidisciplinary review of the evidence.

The osteocutaneous radial forearm (OCRFF) is a versatile free flap option for bony defects of the head and neck, given the thinness and pliability of the forearm cutaneous paddle, pedicle length, reliability, lack of atherosclerosis, and functional concerns common to other osseous donor sites. The OCRFF was once associated with a high risk of radial fracture, in addition to concerns about the quality and durability of bone stock for osseous reconstruction, particularly for the mandible. Following the introduction of prophylactic plating of the radius, the incidence of symptomatic radial fractu

Addressing the Central Band in Acute and Chronic Essex-Lopresti Injury: A Case Report and Technique Description.

CASE: A 34-year-old man was acutely treated with radial head arthroplasty and central band repair following Essex-Lopresti injury. A 38-year-old man presented with chronic longitudinal instability following failed radial head arthroplasty, which was performed for failed fixation. Treatment with revision radial head arthroplasty and central band reconstruction restored longitudinal stability. CONCLUSION: We have a low threshold to repair the central band in acute Essex-Lopresti injury with sufficient evidence of disruption. Nearly all chronic cases require central band reconstruction to restor

Bilateral anterior shoulder dislocation: A case report.

Bilateral anterior shoulder dislocation (BASD) is a rare condition typically associated with a history of trauma. This case report presents a patient with BASD resulting from trauma and discusses treatment options. A 51-year-old male patient was admitted to the emergency department following a fall from a height. Radiographic examination revealed BASD and a comminuted intra-articular fracture of the distal end of the left radius. The patient did not have any other additional pathologies. After a preoperative evaluation, the patient was urgently taken to the operating room, where both shoulders

Design and Performance Analysis of a Bioelectronic Controlled Hybrid Serial-Parallel Wrist Exoskeleton.

Wrist exoskeletons are increasingly being used in the rehabilitation of stroke and hand dysfunction because of its ability to assist patients in high intensity, repetitive, targeted and interactive rehabilitation training. However, the existing wrist exoskeletons cannot effectively replace the work of therapist and improve hand function, mainly because the existing exoskeletons cannot assist patients to perform natural hand movement covering the entire physiological motor space (PMS). Here, we present a bioelectronic controlled hybrid serial-parallel wrist exoskeleton HrWr-ExoSkeleton (HrWE) w

Arthrex Mini Tightrope Fixation for Chronic Distal Radioulnar Joint Instability.

Chronic distal radioulnar joint (DRUJ) instability is a complex clinical condition that is difficult to treat. Currently, there is no gold standard treatment. We present a novel technique using Arthrex Mini Tightrope for DRUJ stabilization. In this case series, a 1.6 mm K-wire was passed transversely through the distal ulna and radius. The Mini Tightrope was inserted into the end of the K-wire and pulled through the bone tunnels. Appropriate tension was achieved to stabilize the joint according to individual laxity comparable to the contralateral side. Five patients (3 males and 2 females) com
