Factors associated with one-year outcome after distal radial fracture treatment.

PURPOSE: To identify factors that affect functional outcome one year after distal radial fracture treatment. METHODS: 521 women and 121 men aged 16 to 92 (mean, 59) years with distal radial fractures underwent external fixation (n=123), open reduction and internal fixation (n=63), a combination of both (n=10), distal radial osteotomy for symptomatic malunion (n=90), or casting with (n=194) or without (n=162) prior closed manipulation under regional anaesthesia. The indication for surgery (rather than casting) was metaphyseal instability. Radiographic evaluation (dorsal angulation, radial shor

Ultrasound-guided pediatric forearm fracture reductions in a resource-limited ED.

BACKGROUND: Pediatric forearm fractures are a common presentation in emergency departments in Papua New Guinea. Often these children undergo "blind" closed reduction with reduction adequacy assessed by standard radiographs. This study aims to demonstrate the safety and efficacy of ultrasound (US) in guiding closed reduction of pediatric forearm fractures in a resource-limited setting. METHODS: We recruited consecutive children with closed forearm fractures requiring reduction. A US scanner was used to visualize and aid fracture reductions. The outcome measures were the rate of successful redu

Successful reconstruction of a gunshot segmental defect of the radius with a free vascularised fibular osteocutaneous flap.

The use of the vascularised fibular flap for bony reconstruction was first described for the mandible. It has since been used for upper limb bony defects of various aetiologies (congenital, infective, traumatic and neoplastic). Specifically gunshot segmental defects of the upper limb have been reconstructed with vascularised fibular in the humerus and radius, the common indication in these previous cases was delayed reconstruction. We describe a challenging case of a 40-year-old male with a 6.5 cm segmental radial defect caused by gunshot wounds, which was primarily reconstructed using this mi

Use of elastic stable intramedullary nailing for treating unstable forearm fractures in children.

BACKGROUND: The majority of the pediatric diaphyseal forearm fractures are treated with closed reduction and the application of an above elbow cast for a varying period, depending on the age of the child. Some forearm diaphyseal fractures require osteosynthesis mainly because of the presence of an unacceptable angular deformity, fracture instability, presence of an open injury or failure of conservative management in the form of redisplacement in the cast after manipulation. METHODS: We present our experience with elastic stable intramedullary nailing (ESIN) for displaced and unstable pediatr

Gradual reduction of distal radial malunion using distraction osteogenesis.

PURPOSE: To evaluate gradual distraction lengthening or distraction osteogenesis as a technique for treating malunions of the distal radius. METHODS: Twenty patients with clinical and radiographic evidence of distal radius malunion were treated with osteotomy of the distal radius using distraction osteogenesis. At the follow-up evaluation each patient was evaluated for healing rates, complications, resolution of pain, and radiographic alignment. Surgical treatment consisted of an application of a nonbridging external fixator that could be distracted to correct the deformity. A loosely set scr

[Elastic stable intramedullary nailing (ESIN) in pediatric forearm shaft fractures: intraoperative image intensifier times].

AIM: Elastic stable intramedullary nailing (ESIN)is the therapy of choice in pediatric forearm shaft fractures. It requires increased intraoperative image intensifier times and radiation load. METHOD: We performed a retrospective analysis of 78 operative procedures from a five-year period. In 16 cases the image intensifier times of the distinct steps of the operation were investigated prospectively. RESULTS: Average duration of the surgical procedure was 36.9 (18-144) minutes. Average radiation time was 59.5 (8-222) seconds. In educational operations, the duration of surgery was significantl

Standard surgical protocol to treat elbow dislocations with radial head and coronoid fractures.

BACKGROUND: The results of elbow dislocations with associated radial head and coronoid fractures are often poor because of recurrent instability and stiffness from prolonged immobilization. We managed these injuries with a standard surgical protocol, postulating that early intervention, stable fixation, and repair would provide sufficient stability to allow motion at seven to ten days postoperatively and enhance functional outcome. METHODS: We retrospectively reviewed the results of this treatment performed, at two university-affiliated teaching hospitals, in thirty-six consecutive patients (

Functional recovery after fractures of the distal forearm. Analysis of radiographic and other factors affecting the outcome.

Functional recovery of the wrist and hand after a fracture of the distal forearm in 207 consecutive patients was analysed. A good or excellent result was achieved in 77 percent, fair in 22.5 percent and poor in 0.5 percent of cases. The result in 14 unstable fractures treated with external fixation was as good as that of the whole series. Good functional results were associated with extra-articular fractures, good anatomical results, male sex and low age. Comminuted intra-articular fractures of the radiocarpal joint, fracture line into the distal radio-ulnar joint, fracture of the ulnar styloi

Closed Reduction of Pediatric Distal Radial Fractures and Epiphyseal Separations.

BACKGROUND: Sedated, closed reduction of a displaced distal radial fracture followed by cast immobilization is indicated in cases of unacceptable alignment on post-splint imaging. The aim of this procedure is to obtain acceptable reduction and cast immobilization for fracture-healing. DESCRIPTION: The patient is positioned supine with the injured arm on the image intensifier. Adequate sedation is achieved with conscious sedation, general anesthesia, or regional anesthesia (hematoma block). The radial or ulnar translation is corrected with in-line traction. The wrist is typically hyperdorsifle

Patient-specific plate for navigation and fixation of the distal radius: a case series.

PURPOSE: Corrective osteotomy of a malunited distal radius conventionally relies on 2D imaging techniques for alignment planning and evaluation. However, this approach results in suboptimal bone repositioning, which is associated with poor patient outcomes. In this case series, we evaluate the use of novel patient-specific plates (PSPs), which feature navigation and fixation of bone segments as preoperatively planned in 3D. METHODS: Ten participants with distal radius malunion underwent CT scans for preoperative alignment planning. Patient-specific guides and plates were designed, 3D-printed,

Transverse coronoid fracture: when does it have to be fixed?

BACKGROUND: After elbow fracture-dislocation, surgeons confront numerous treatment options in pursuing a stable joint for early motion. The relative contributions of the radial head and coronoid, in combination, to elbow stability have not been defined fully. QUESTIONS/PURPOSES: The purpose of this study was to evaluate the effect of an approximately 50% transverse coronoid fracture and fixation in the setting of an intact or resected radial head on coronal (varus/valgus) and axial (internal and external rotational) laxity in (1) gravity varus stress; and (2) gravity valgus stress models. Denervation of the radiocarpal joint. A follow-up study in 22 patients.

Denervation surgery has been a mainstay of our management of chronic pain in the wrist. If there is useful movement at the wrist we prefer denervation to arthrodesis. We have reviewed 22 patients at a mean of 50 months after such denervation surgery at the wrist. This was the only treatment in 16 patients; the other six also had other treatments. Pain was reduced in 16 patients, and 17 were satisfied or improved. None of the patients wished to have a supplementary arthrodesis. We stress the importance of preoperative blockade tests and of a very detailed knowledge of the local anatomy.

Outcome of corrective osteotomies of the distal radius using three-dimensional-printed patient-specific implants.

Corrective osteotomy for a malunited distal radial fracture (DRF) can be challenging. Three-dimensional printing techniques can be used to produce patient specific implants (PSIs). Theoretically, this technique improves accuracy of reconstruction and therefore patient-related outcomes. The aim of this retrospective cohort study was to compare pre- and postoperative patient-rated functional and radiological outcomes following corrective osteotomy of 47 malunited DRFs with PSIs. The primary outcome was the difference in Patient-Rated Wrist Hand Evaluation score (PRWHE). The median PRWHE improved

Does endoscopic carpal tunnel release during distal radial fracture fixation improve outcomes? A randomized controlled trial.

This study investigated the functional outcome of prophylactic carpal tunnel release during distal radial fracture fixation. A total of 60 patients undergoing anterior locking plate fixation for a distal radial fracture were randomized into two groups: one having prophylactic endoscopic carpal tunnel release and the other serving as a control. Functional outcomes were assessed using the Patient-Reported Wrist Evaluation, Quick Disabilities of the Arm, Shoulder, and Hand, EuroQol 5-Dimension 5-Level questionnaire, Boston Carpal Tunnel Questionnaire, Pain Numerical Rating Scale, grip strength an

Post-traumatic Non-haemophilic Haemosiderotic Synovitis of the Elbow: A Case Report.

Haemosiderotic synovitis (HS) is a rare synovial proliferative disease secondary to haemarthrosis, often with articular cartilage destruction. It is most frequently reported in patients with haemophiliacs, and the knee joint is most frequently affected. However, there are no reports on the elbow joint without haemophiliacs. A 60-year-old woman who had undergone osteosynthesis for a left radial head fracture 8 years earlier came to our clinic with left elbow pain. X-rays and CT scans showed osteopenia and osteoarthritic changes throughout the elbow joint. MRI revealed joint effusion and synovia
