Lujan-Fryns syndrome (mental retardation, X-linked, marfanoid habitus).

The Lujan-Fryns syndrome or X-linked mental retardation with marfanoid habitus syndrome is a syndromal X-linked form of mental retardation, affecting predominantly males. The prevalence is not known for the general population. The syndrome is associated with mild to moderate mental retardation, distinct facial dysmorphism (long narrow face, maxillary hypoplasia, small mandible and prominent forehead), tall marfanoid stature and long slender extremities, and behavioural problems. The genetic defect is not known. The diagnosis is based on the presence of the clinical manifestations. Genetic coun

Recurrent symptomatic bradycardia due to secret ingestion of beta-blockers--a rare manifestation of cardiac Münchhausen syndrome.

The term Münchhausen syndrome was established in 1951 by Asher to describe a severe psychiatric illness in which patients simulate false symptoms and signs. We report on a female general practitioner who repeatedly ingested high doses of beta-blockers in order to simulate symptomatic sick-sinus syndrome. She had been admitted to intermediate care units in several hospitals before the correct diagnosis was made by finding the tablets in her toilet bag. Following psychiatric exploration and psychotherapy, she has been working in her community again for about a year. This is the first report on t

Gastric cancer acrometastases to all digits of one hand following closed intramedullary nailing.

Metastatic malignant tumor of the hand is unusual and the most common site of the primary cancer is lung. Acrometastases to the phalanges of the hand usually involve a single bone, while those proximal to the phalanges often involve multiple bones. Metastasis to the hand from gastrointestinal cancer, particularly from gastric cancer, is extremely rare. To our knowledge, only five cases have been reported in the literature to date and there has been no reported case following closed intramedullary nailing for metastatic diaphyseal fracture of an ipsilateral long bone. We present a very unusual

Huge lymphangioma of the tongue: a case report.

Lymphangioma of the tongue is relatively rare and may cause facial structural deformity. Using a combination of a V-shaped and central resection, we successfully treated a 6-year-old girl who had massive lymphangioma of the tongue. Postoperatively, her tongue was located completely within her mouth with good cosmetic results. Sensory and motor nerves to the tongue appeared to be intact. Her speech was also improved.

Unexpected findings in the routine histopathological examinations of appendectomy specimens A retrospective analysis of 1,970 patients.

INTRODUCTION: Diseases and tumors of the appendix vermiformis are very rare, except for acute appendicitis. This study aimed to examine rare findings in the histopathologic examinations of specimens of patients undergoing appendectomy due to the diagnosis of acute appendicitis. METHODS: The files of 1,970 patients undergoing appendectomy due to the diagnosis of acute appendicitis between March 2012 and March 2016 were retrospectively investigated. Rare findings were found in 59 (3%) patients, and these were evaluated in detail. Patients' age, gender, pathology reports, and post-operation foll

Severe Autoimmune Hemolytic Anemia Following Pneumococcal Vaccination in an Infant With Underlying Glucose-6-Phosphate Dehydrogenase (G6PD) Deficiency: Report of a Case and Review of the Literature.

Autoimmune hemolytic anemia (AIHA) and glucose-6-phosphate dehydrogenase (G6PD) deficiency are two distinct causes of hemolysis in children and a combination of both diseases is considered rare, especially in early infancy. We present such a rare case of severe AIHA in an infant with G6PD deficiency in the setting of Escherichia coli urinary tract infection and recent pneumococcal vaccination history, with the goal of analyzing potential links between them, examining the causative role of vaccines, and reviewing available literature.

Atrialized right ventricular myxoma in a patient with Ebstein's anomaly.

Ebstein's anomaly is a rare entity affecting around 1 in 200,000 live births and accounts for less than 1% of congenital heart diseases. Ebstein's anomaly with an associated right-sided myxoma is extremely rare, with only one other case report found in the literature. Previous reports have also noted cases of Ebstein's anomaly associated with left-sided myxomas. We describe a female patient with, to our knowledge, the first case of a histopathologically confirmed right ventricular myxoma in the setting of Ebstein's anomaly.

Congenital anonychia and brachydactyly of the left foot - Cooks syndrome variant: Case report and review of literature.

Cooks syndrome is characterized by familial congenital anonychia or onychodystrophy, hypoplasia or absence of distal phalanges of the hands and feet with brachydactyly of the fifth finger and digitalization of the thumb (triphalangism). It is listed as a "rare disease" by the Office of Rare Diseases of the National Institutes of Health. Here, we report a case of congenital anonychia and brachydactyly of the left foot, which possibly is a variant of Cooks syndrome with a positive family history of similar deformity.

Idiopathic juvenile osteoporosis. An unusual cause of back pain in an adolescent.

Idiopathic juvenile osteoporosis represents a rare bone disorder that affects prepubescent children of both sexes. The diagnosis is generally one of exclusion, ruling out other potential metabolic bone diseases. Here, the authors present a representative case of this rare disorder in an otherwise healthy male, along with the clinical history, physical presentation, and review of the available literature. Finally, it is important to note that patients with this disorder often have profound involvement of the spinal column with multiple compression fractures.

[A review of post-operative multiple organ failure in peripheral arterial diseases].

Two-hundred- and-fifty-one patients of peripheral arterial diseases underwent 281 operations for past 11 years in our institute, and multiple organ failure (MOF) occurred in 10 patients (4.0%). These patients were reviewed and were compared with in other diseases. Survival rate was 50% and all the patients with 4 or more organ disorders died. Incidence and survival rate were not significantly different from operative time, blood loss and blood transfusion. Significant difference was observed between elective operation (3.0%) and emergent operation (20.0%). Compared with other diseases, MOF occ

Pericarditis and Autoinflammation: A Clinical and Genetic Analysis of Patients With Idiopathic Recurrent Pericarditis and Monogenic Autoinflammatory Diseases at a National Referral Center.

Background Idiopathic recurrent pericarditis (IRP) is an orphan disease that carries significant morbidity, partly driven by corticosteroid dependence. Innate immune modulators, colchicine and anti-interleukin-1 agents, pioneered in monogenic autoinflammatory diseases, have demonstrated remarkable efficacy in trials, suggesting that autoinflammation may contribute to IRP. This study characterizes the phenotype of patients with IRP and monogenic autoinflammatory diseases, and establishes whether autoinflammatory disease genes are associated with IRP. Methods and Results We retrospectively analy

Rare causes of acute abdomen in a paediatric surgery department: a 5-years review.

AIMS: Acute abdomen is a common indication to surgery in paediatric emergency departments. Appendicitis is the most frequent diagnosis, followed by bowel obstruction and female genital conditions. Some rare aetiologies may lead to challenges in surgical management. Aim of this study is to analyse the distribution of rare causes of acute abdomen in patients undergoing emergency surgery in the past five years at our centre. METHODS: Records of patients undergone urgent abdominal surgery were retrospectively reviewed from 2015 to 2019. Information relative to age, sex, underlying pathology, and

Insulin autoimmune syndrome in a pregnant female: A rare case report.

RATIONALE: Insulin autoimmune syndrome (IAS) is an uncommon disorder characterized by hyperinsulinemic hypoglycemia related to insulin-binding autoantibodies. To the best of our knowledge, we report the first case of a pregnant female with IAS. PATIENT CONCERNS: The 26-year-old patient with Graves disease and 10 weeks pregnant developed IAS after approximately 6 months treatment with methimazole. The patient exhibited recurrent spontaneous hypoglycemia. DIAGNOSES: On evaluation, laboratory findings detected both high fasting insulin (>1000 mIU/L) and insulin autoantibodies. An oral glucose t

[Clinical characteristics and diagnostics of human spongiform encephalopathies: an update].

Human spongiform encephalopathies are rare transmissible neurodegenerative diseases of the brain and the nervous system that are caused by misfolding of the physiological prion protein into a pathological form and its deposition in the central nervous system (CNS). Prion diseases include Creutzfeldt-Jakob disease (CJD, sporadic or familial), Gerstmann-Straussler-Scheinker syndrome (GSS) and fatal familial insomnia (FFI). Prion diseases can be differentiated into three etiological categories: spontaneous (sporadic CJD), inherited (familial CJD, FFI, and GSS) and acquired (variant CJD and iatrog

Coexistence of rheumatoid arthritis and ankylosing spondylitis.

Rheumatoid arthritis (RA) and ankylosing spondylitis (AS) are chronic progressive inflammatory diseases, leading to joint damage and reducing the physical fitness of patients. They are among the most common rheumatic diseases. However, their etiology and symptomatology are different. Formerly, AS was often wrongly diagnosed as RA. Today there are no major diagnostic difficulties in differentiation between these diseases, thanks to modern laboratory tests and imaging. However, a problem may arise when the patient has symptoms typical for both diseases simultaneously. Cases of coexistence of RA

[Malignant and non-malignant uveitis-masquerade syndromes in childhood].

Diseases that can clinically simulate an uveitis are summarised under the name Masquerade syndromes. They constitute a heterogenous group of malignant and non-malignant entities that must be excluded in the differential diagnosis. Of the malignant diseases lymphomas are only rarely observed as the cause of a pseudo-uveitis, whereas especially leukaemic diseases and, in childhood, retinoblastomas and the extremely rare intraocular medulloepithelioma must be excluded. Of the non-malignant diseases in childhood, besides Coats' disease, a juvenile xanthogranuloma as well as persisting hyperplastic

The VASCERN PPL working group patient pathway for primary and paediatric lymphoedema.

Lymphoedema is caused by an imbalance between fluid production and transport by the lymphatic system. This imbalance can be either caused by reduced transport capacity of the lymphatic system or too much fluid production and leads to swelling associated with tissue changes (skin thickening, fat deposition). Its main common complication is the increased risk of developing cellulitis/erysipelas in the affected area, which can worsen the lymphatic function and can be the cause of raised morbidity of the patient if not treated correctly/urgently. The term primary lymphoedema covers a group of rare
