Chronic homocysteine exposure causes changes in the intrinsic electrophysiological properties of cultured hippocampal neurons.

Homocystinuria is an inborn error of metabolism characterized by plasma homocysteine levels up to 500 μM, premature vascular events and mental retardation. Mild elevations of homocysteine plasma levels up to 25 μM, which are common in the general population, are associated with vascular disease, cognitive impairment and neurodegeneration. Several mechanisms of homocysteine neurotoxicity have been investigated. However, information on putative effects of hyperhomocysteinemia on the electrophysiology of neurons is limited. To screen for such effects, we examined primary cultures of mouse hippoca

Restoring functional status: a long-term case report of severe lung and ventilatory muscle pump dysfunction involving recurrent bacterial pneumonias.

BACKGROUND AND PURPOSE: Prolonged mechanical ventilation contributes to immobility and deconditioning making efforts to safely discontinue ventilator support desirable. This case report documents how implementing physical therapy treatment interventions, based on the Guide to Physical Therapist Practice, can help to restore a person's functional status even after multiple years of mechanical ventilation dependency. CASE DESCRIPTION: A patient (female; aged 63 years) with severe restrictive and obstructive ventilatory impairment has survived 34 recurrent pneumonias involving 6 bacterial pathog

Integrated, home-based treatment for MDR-TB and HIV in rural South Africa: an alternate model of care.

SETTING: Treatment outcomes for multidrug-resistant tuberculosis (MDR-TB) in South Africa have suffered as centralized, in-patient treatment programs struggle to cope with rising prevalence and human immunodeficiency virus (HIV) co-infection rates. A new treatment model is needed to expand treatment capacity and improve MDR-TB and HIV outcomes. OBJECTIVE: To describe the design and preliminary results of an integrated, home-based MDR-TB-HIV treatment program created in rural KwaZulu-Natal. METHOD: In 2008, a decentralized center was established to provide out-patient MDR-TB and HIV treatment

Multimodal alexia: neuropsychological mechanisms and implications for treatment.

Letter-by-letter (LBL) reading is the phenomenon whereby individuals with acquired alexia decode words by sequential identification of component letters. In cases where letter recognition or letter naming is impaired, however, a LBL reading approach is obviated, resulting in a nearly complete inability to read, or global alexia. In some such cases, a treatment strategy wherein letter tracing is used to provide tactile and/or kinesthetic input has resulted in improved letter identification. In this study, a kinesthetic treatment approach was implemented with an individual who presented with sev

Role of the clinical microbiology laboratory in infection control--a Danish perspective.

Clinical microbiology laboratories in Denmark are located in hospitals and staffed by clinical microbiologists who are clinically trained medical doctors. Each county has its own clinical microbiology unit, serving a population of 0.3-0.6 million. The responsibilities of clinical microbiology unit cover many different aspects of infection control. They include detection of outbreaks of hospital-acquired infections, screening for multi-resistant organisms, advice to clinicians about disinfection, sterilization and isolation procedures, and the rational use of antibiotics. Effects of the halothane genotype and slaughter weight on texture of pork.

The objective of this study was to investigate the effects of the halothane (HAL) genotype, slaughter weight (SW), and the HAL x SW interaction on compositional and textural traits of raw and cooked pork. Pigs were bred to exhibit one of the three HAL genotypes (NN, Nn, and nn) with otherwise equivalent genomes. The nn halothane reactors are known to typically produce PSE pork, whereas NN pigs do not typically produce PSE pork. Pietrain x Large White gilts and boars, all with verified Nn genotype (by DNA test), were mated to obtain F2 littermates of the three HAL genotypes. These pigs were sla

Effects of unilateral cardiac sympathetic denervation on the ventricular fibrillation threshold.

A train of gated stimuli scanning the entire vulnerable period was delivered to the right anterior or left posterior ventricular surface to study the ventricular fibrillation threshold in anesthetized and vagotomized dogs. Heart rate was held constant by atrial pacing. Measurements were obtained in control conditions and after surgical removal of one stellate ganglion. To avoid the shortcomings associated with an irreversible procedure like stellectomy, control fibrillation threshold measurements were also alternated with determinations during reversible blockade by cooling of one stellate gan

Hybrid 12-Month Exoskeleton Training with Percutaneous Epidural Stimulation After Spinal Cord Injury.

Background: Exoskeleton-assisted walking (EAW) is an effective method of rehabilitation that improves ambulation as well as independence in individuals with chronic spinal cord injury (SCI). Percutaneous spinal cord epidural stimulation (SCES) is another strategy that has shown promising results for promoting locomotor recovery in persons with SCI. We hypothesized that the hybrid application of SCES with EAW for 12 months would further enhance motor performance after SCI. Methods: Four males with chronic motor complete SCI underwent permanent implantation with percutaneous SCES. Participants w

MedHopQA: A Disease-Centered Multi-Hop Reasoning Benchmark and Evaluation Framework for LLM-Based Biomedical Question Answering.

Evaluating large language models (LLMs) in the biomedical domain requires benchmarks that can distinguish genuine reasoning from pattern matching and that remain discriminative as model capabilities improve. Existing biomedical question answering (QA) benchmarks are limited in this respect: multiple-choice formats allow models to succeed by answer elimination rather than inference, and widely circulated exam-style datasets are subject to performance saturation and training data contamination. Multi-hop reasoning. i.e., the ability to integrate information across multiple sources to derive an a

Mitochondrial dysfunction in chemical anemia: can nursing led exercise programs improve red blood cell production?

INTRODUCTION: Chemical anemia, a common consequence of chemotherapy and environmental toxins, is conventionally attributed to bone marrow suppression. This traditional view may oversimplify the underlying pathology, potentially overlooking critical cellular mechanisms that could serve as novel therapeutic targets. OBJECTIVES: This review aims to (1) propose a paradigm shift in understanding chemical anemia by reframing it as a disorder of mitochondrial dysfunction within erythroid precursors and (2) evaluate the potential of structured exercise as a multi-targeted countermeasure to restore er

HRR gene promoter methylation-guided nomogram predicts PARP inhibitor efficacy and progression-free survival in high-grade serous ovarian cancer.

BACKGROUND: High-grade serous ovarian cancer (HGSOC) remains a lethal gynecologic malignancy with limited tools for accurately predicting PARP inhibitor (PARPi) efficacy. While promoter methylation of homologous recombination repair (HRR) genes is a potential biomarker, its clinical utility remains underexplored. This study aimed to systematically evaluate HRR gene promoter methylation profiles to predict PARPi sensitivity in HGSOC patients. METHODS: We conducted a retrospective analysis of 96 HGSOC patients receiving PARPi maintenance therapy (olaparib or niraparib). The promoter methylation
