Improving Guideline-Directed Medical Therapy in Hospitalized HFrEF Patients: A Multidisciplinary Quality Improvement Initiative.

BACKGROUND: Guideline-directed medical therapy (GDMT) reduces mortality and rehospitalization in heart failure with reduced ejection fraction (HFrEF), yet many hospitalized patients are discharged without optimal therapy. PROJECT RATIONALE: A baseline audit at our community teaching hospital showed fewer than half of eligible patients were discharged on ≥3 GDMT pillars, driven by cost barriers, deferred titration, and inconsistent education. PROJECT SUMMARY: A multidisciplinary Plan-Do-Study-Act intervention standardized GDMT workflows, incorporating pharmacist-led admission review, structur

Emotion regulation strategy use and daily levels of affective arousal, affective valence and dissociation in a clinical sample.

Clinical models and cross-sectional research suggest that habitual use of disengagement-oriented emotion regulation strategies is associated with high levels of affective arousal, negative valence, and dissociation, whereas engagement-oriented strategies show the opposite effect. However, intensive longitudinal evidence remains scarce. This study examined whether habitual use of 10 emotion regulation strategies, assessed using the Process Model of Emotion Regulation Questionnaire (PMERQ), predicts affective arousal and valence, and dissociation in daily life. We analysed ecological momentary a

scGraphVerse: a modular workflow for single-cell gene network inference.

MOTIVATION: Inferring gene networks from single-cell RNA sequencing data is challenging due to high sparsity, dimensionality, and technical noise. Current pipelines lack the multi-dataset integration and comprehensive post-processing analysis. RESULTS: scGraphVerse is an R package that integrates multiple algorithms (GENIE3, GRNBoost2, ZILGM, PCzinb, and JRF) with extensive evaluation and visualization tools. Its modular workflow supports early, late, and joint integration strategies for multi-dataset analysis, providing standardized input/output interfaces and biological interpretation tools

Phase 1 study of intracranial T cell therapy with pemetrexed for refractory brain metastases and glioblastomas.

Therapeutic options remain limited for advanced intracranial malignancies, including brain metastases and glioblastomas, particularly in patients ineligible for surgery, radiotherapy, or systemic therapies. This phase 1 study evaluates the safety and feasibility of intracranial administration of low-dose pemetrexed followed by RetroNectin-activated killer cells (RAKs) and interleukin 2 via an Ommaya reservoir (collectively referred to as SCOPE-T) in 25 patients with refractory intracranial malignancies. Nineteen patients completed at least one treatment cycle, and 17 received the maximum teste

Evaluating Cognitive Workload With the Auditory Stroop Task While Using a Lower Limb Prosthesis: a Feasibility Study.

This study explored the feasibility of using the auditory Stroop task to assess cognitive workload in a dual-task paradigm. Performance on two tasks-counting backward by 3 and the auditory Stroop task-was measured across two conditions: participants' own passive device and a direct EMGcontrolled (dEMG) device. Results based on individual participants' data indicated that the auditory Stroop task effectively captured more consistent dual-task costs under cognitively demanding conditions, whereas the backward counting task exhibited minimal and less consistent performance deterioration. Particip

Efficient sequence alignment against millions of prokaryotic genomes with LexicMap.

The size of microbial sequence databases continues to grow beyond the abilities of existing alignment tools. We introduce LexicMap, a nucleotide sequence alignment tool for efficiently querying moderate-length sequences (>250 bp) such as a gene, plasmid or long read against up to millions of prokaryotic genomes. We construct a small set of probe k-mers, which are selected to efficiently sample the entire database to be indexed such that every 250-bp window of each database genome contains multiple seed k-mers, each with a shared prefix with one of the probes. Storing these seeds in a hierarchi

Visualizing intraorganellar ultrastructures, dynamics, and interactions with open-access background-free Lock-in-SIM.

Structured illumination microscopy (SIM) is a powerful method for fast and gentle live-cell super-resolution imaging. However, its susceptibility to reconstruction artifacts from out-of-focus blur and background imposes substantial barriers to analyze the dynamics of densely packed volumetric intraorganellar ultrastructures that are typically in a size range of SIM's spatial resolution. To address this limitation, we have developed Lock-in-SIM, an open-access two-dimensional SIM framework that eliminates background and maximizes the recovery of sub-diffraction information with the highest poss

Use of Mitochondrial Delayed Luminescence Measurements From Brain Tissue to Optimize Photobiomodulation Prarameters in Alzheimer's Disease Mice.

The therapeutic potential of photobiomodulation (PBM) for Alzheimer's disease (AD) was evaluated by examining β-amyloid accumulation, microglial activation, and memory function. Mitochondrial delayed luminescence (m-DL), an indirect mitochondrial marker, was measured by irradiating 2 J/cm2 of 808 nm near-infrared light to the exposed brain surface of anesthetized 5XFAD mice. Based on m-DL findings, behavioral PBM was applied transcranially to the intact scalp using the same fluence at 30, 40, and 80 Hz with respective duty cycles and durations. The 80 Hz setting produced the longest m-DL decay

Integrating axis quantitative trait loci looks beyond cell types and offers insights into brain-related traits.

Genome-wide association studies have identified many loci for brain disorders, but most non-coding variants fail to colocalize with bulk expression quantitative trait loci. Single-cell expression quantitative trait loci studies capture cell-type-specific regulation but are often underpowered. We developed Bulk And Single cell expression quantitative trait loci Integration across Cell states (BASIC) to combine bulk and single-cell expression quantitative trait loci through "axis-quantitative trait loci," which decompose bulk-tissue effects along orthogonal axes of cell-type expression. BASIC be

Giant Intracranial Chondroma.

Intracranial chondroma is extremely rare. The treatment of choice is total tumor excision when resectable. A 30-year-old female patient presented with a giant intracranial chondroma that had enlarged for 18 years. In this case, we totally removed the tumor and reconstructed the meninges, cranium, and left ear using fascia lata, a combination of titan mesh and cement, and the posterior auricular artery axial flap, respectively. The aesthetic outcome was acceptable. No recurrence was identified after 7 months of follow-up. The patient was satisfied with the outcome. The lack of access to high-te

Meta-analysis of genetic regulation of RNA editing in the human brain identifies new genes underlying neurological disease.

Adenosine-to-inosine (A-to-I) RNA editing is among the most abundant post-transcriptional modifications, whose role in homeostasis and disease of the central nervous system (CNS) has been extensively reported. Regulation of A-to-I editing by cis-acting genetic variants (edQTLs) is understudied, especially in the brain. As a part of the BigBrain Project (n=4,471 individuals), which represents a >10-fold increase in sample size from previous edQTL studies, we harmonized A-to-I sites across datasets to perform a meta-analysis that identified 38,731 edQTLs in 3,554 genes. Integrating other QTL sum

Phage therapy with nebulized cocktail BX004-A for chronic Pseudomonas aeruginosa infections in cystic fibrosis: a randomized first-in-human trial.

Cystic fibrosis is a monogenetic disease complicated by recurrent bacterial lung infections that require chronic antibiotics. Pseudomonas aeruginosa is an increasingly antibiotic-resistant pathogen associated with cystic fibrosis morbidity and mortality. Here, we describe the development of a three-phage cocktail (BX004-A) designed to target a wide range of P. aeruginosa strains. We evaluated BX004-A in Part 1 of a first-in-human double-blind placebo-controlled phase 1b/2a clinical trial, which included nine adult cystic fibrosis patients chronically infected with P. aeruginosa (NCT05010577).

A Meta-Analysis of the Effects of Transcranial Direct Current Stimulation on Anger and Aggression.

Anger and aggression are causes of significant suffering. Psychological methods to prevent and reduce anger and aggression have been partially successful; however, there is room for novel interventions, such as those informed by neuroscience. One such intervention is anodal transcranial direct current stimulation (tDCS), which involves administering a weak electrical current to the brain to stimulate cortical activity. In this meta-analysis, we synthesized 93 effect sizes from 25 sham-controlled experiments. We predicted that tDCS would reduce anger and aggression. The overall results showed n

Polygenic scores for disease risk are not associated with clinical outcomes in Parkinson's disease.

Polygenic risk scores (PRS) in Parkinson's disease (PD) are associated with disease risk. Recently, pathway-specific PRS have been created to take advantage of annotations inking variants to biological pathways or cell types. Here, we investigated 8 biological pathways or regions of open chromatin using pathway-specific PRS: alpha-synuclein pathway, adaptive immunity, innate immunity, lysosomal pathway1, endocytic membrane-trafficking pathway, mitochondrial pathway, microglial open chromatin single nucleotide polymorphisms (SNPs), and monocyte open chromatin SNPs. We analysed 7,402 PD patients

Digital phenotyping of Parkinson's disease via natural language processing.

Frontostriatal degeneration in Parkinson's disease (PD) is associated with language deficits, which can be identified using natural language processing, a remarkable tool for digital-phenotyping. Current evidence is mostly blind to the disorder's cognitive phenotypes. We validated an AI-driven approach to capture digital language markers of PD with and without mild cognitive impairment (PD-MCI, PD-nMCI) relative to healthy controls (HCs). Analyzing the connected speech of participants, we extracted linguistic features with CLAN software. Classification was performed using SVM and RFE. Discrimi
