The Potential of SARMs and Antimyostatin Agents in Addressing Lean Body Mass Loss From GLP-1 Agonists: A Literature Review.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have demonstrated substantial weight loss effects among patients with diabetes and obesity. However, given the rapid weight loss induced, there is concern about the total change in body composition, including lean body mass (LBM). Current literature on these effects contains significant heterogeneity, with some studies reporting a loss of 40%-60% of LBM and others reporting 15% or less. To combat this, selective androgen receptor modulators (SARMs) have become a popular candidate. Given their androgen receptor selectivity, SARMs have notabl

Cold temperatures, hot risks: perioperative hypothermia in geriatric patients - a narrative review.

Aging adversely impacts thermoregulatory function, thereby increasing the risk of intraoperative hypothermia. Age-associated alterations-including diminished thermal perception, impaired autonomic responsiveness, reduced thermogenic capacity due to sarcopenia, and decreased cardiovascular adaptability, exacerbate the vulnerability to hypothermia. Concomitant comorbidities and polypharmacy further compromise thermal homeostasis in geriatric patients. Anesthetic agents compound this risk by lowering the thresholds for vasoconstriction and shivering and attenuating the magnitude of thermal respon

Prevalence and coexistence of locomotive syndrome with reduced mobility and metabolic syndrome: a cross-sectional study of 35,059 Japanese adults.

Metabolic syndrome (MetS), marked by visceral obesity, hypertension, hyperglycemia, and dyslipidemia, elevates cardiovascular risk. Locomotive syndrome (LS), characterized by age-related mobility decline, often precedes frailty and sarcopenia. We investigated the prevalence and association between MetS and LS among Japanese middle-aged and older adults. This retrospective cross-sectional study was conducted using clinical records of 35,059 Japanese adults who underwent health check-ups between April 2021 and March 2022. MetS was diagnosed using the modified National Cholesterol Education Progr

The Role of Previous History of Muscle Wasting in Burn Outcomes-A Burn Care Quality Platform Study.

BACKGROUND: Burn patients can suffer prolonged hospital stays, infections, and wound breakdown. Given the complexity of burns, it is often difficult to determine which underlying factors contribute to complications. The Burn Care Quality Platform (BCQP) is the largest database of burn patients globally available, and it accounts for underlying or coinciding disease conditions present in burn patients. Muscle wasting conditions, such as sarcopenia, cachexia, and protein malnutrition, are suspected of causing worse outcomes. Prior analysis of BCQP data (2000-2017) demonstrated that patients with

Low, non-psychedelic doses of psilocybin as a novel treatment for MASLD, obesity and type 2 diabetes via 5-HT2B receptor-dependent mechanisms.

The therapeutic potential of low, non-psychedelic doses of psilocybin, a fungal tryptamine alkaloid, was investigated in metabolic disorders including obesity, type 2 diabetes mellitus (T2DM), and liver steatosis. Mice fed a high-fat/high-fructose diet received chronic treatment with psilocybin (0.05 mg/kg) for 12 weeks. Body weight, liver histology, insulin sensitivity, and skeletal muscle function were assessed, and hepatic and muscle tissues underwent transcriptomic and lipidomic analyses. The role of three serotonin receptors (5-HT2A, 5-HT2B, and 5-HT2C) in psilocybin-induced metabolic eff

Decrease in Psoas Muscle Mass and Density Following Liver Transplantation Is Greatest in Patients With the Highest Muscle Quantity and Density Pre-Transplant.

Sarcopenia is a known predictor of morbidity and mortality after liver transplantation (LT); it has been assessed with computed tomography (CT) derived Psoas Area Index (PAI) and mean Hounsfield units (mHU). While literature is abundant regarding the adverse outcomes of liver transplant in sarcopenic patients, a paucity of data exists describing the change in psoas muscle area and density from pre- to post-liver transplant. One hundred and four adult liver transplant recipients had pre- and post-transplant CT scans analyzed with respect to PAI and mHU. Mean PAI pre-transplant was 7.94 and 6.99

Trichosanthes kirilowii Maximowicz attenuates dexamethasone induced atrophy in C2C12 myotubes through a Sirtuin 1 associated mechanism.

Glucocorticoids such as dexamethasone (DEX) are commonly used clinically but can induce skeletal muscle atrophy with prolonged or high-dose exposure. Trichosanthes kirilowii Maximowicz (TK), a traditional medicinal herb, has known pharmacological effects, but its impact on muscle atrophy remains unclear. This study investigated the anti-atrophic potential of TK in DEX-induced muscle atrophy models. In C2C12 cells, TK improved cell viability and restored myotube diameter and number, downregulated atrophy markers MuRF-1 and Atrogin-1, and upregulated myogenic markers MyoD, MyoG, and MHC. TK also

Immunoproteasomes in Skeletal Muscle Pathologies: Emerging Roles, Conflicting Evidence, and Future Directions.

Skeletal muscle pathologies, including sarcopenia, inflammatory myopathies, and various muscular dystrophies, are strongly influenced by chronic low-grade inflammation and impaired proteostasis. Immunoproteasomes (IMPs), inducible proteolytic complexes activated by pro-inflammatory cytokines, are emerging as regulators linking immune signaling to protein quality control. Evidence suggests that IMPs have paradoxical, context-dependent roles in skeletal muscle. On one hand, they can support proteostasis and muscle regeneration under stress; on the other, persistent activation may sustain cytokin

Cornflower Extract and Its Active Components Alleviate Dexamethasone-Induced Muscle Wasting by Targeting Cannabinoid Receptors and Modulating Gut Microbiota.

Sarcopenia, a decline in muscle mass and strength, can be triggered by aging or medications like glucocorticoids. This study investigated cornflower (Centaurea cyanus) water extract (CC) as a potential protective agent against DEX-induced muscle wasting in vitro and in vivo. CC and its isolated compounds mitigated oxidative stress, promoted myofiber growth, and boosted ATP production in C2C12 myotubes. Mechanistically, CC reduced protein degradation markers, increased mitochondrial content, and activated protein synthesis signaling. Docking analysis suggested cannabinoid receptors (CB) 1 and 2

Muscle mass as a modifier of stress response in acute ischemic stroke patients.

Stroke triggers a systemic inflammatory response over the ensuing days after the cerebral insult. The age and comorbidities of the stroke population make them a vulnerable population for low muscle mass and sarcopenia, the latter being another clinical condition that is closely associated with inflammation, as shown by increased levels of pro-inflammatory biomarkers, including neutrophil-to-lymphocyte ratio (NLR). In this study, we evaluated the relationship between post-stroke NLR changes and muscle mass in a prospective cohort of acute ischemic stroke patients (n = 102) enrolled in the Muscl
