Chronic scrotal pain is most commonly explained by the radiculitis that is caused by low back strain. The diagnosis is made chiefly by taking a careful history and by excluding other causes. Treatment directed to relieve sensory nerve root irritation at the T10 to L1 levels is often successful.

Although sclerotherapy as a treatment for hydroceles has gained in popularity during the last few years, there is no consensus on whether it can also be adopted for the treatment of spermatoceles. We describe 16 patients who underwent conservative surgical treatment of the ilioinguinal region after transscrotal orchiectomy had demonstrated a non-seminomatous germ cell tumor. Treatment consisted of an en bloc partial scrotectomy and removal of the remaining spermatic cord, usually through 2 incisions. Residual disease was present in 3 patients (19%) but malignant disease has not recurred in any. It is concluded that an inguinal lymphadenectomy is not warranted routinely in such patients.

We present our experience with a method of phalloplasty which can be applied by reconstructive surgeons to cases of penile loss from diverse etiologies. The complications with this method have been frequent, but not life-threatening. Achievement of the stated objectives is obtained in a sufficiently high proportion of cases to make this a method whorthwhile to use until a clearly superior method can be demonstrated.

Our brief experience of the use of ultrasonography and ultrasonic Doppler method for the diagnosis of intrascrotal contents were described. Five cases of testicular tumor, two cases of hydrocele and one case of epididymal tuberculosis were successfully diagnosed by ultrasonography. Only one case of chronic inflammation was misdiagnosed as a testicular tumor. Two cases of testicular torsion and two cases of acute epididymitis were easily differentiated by the ultrasonic Doppler method. We suggest that this is the most reliable technique for differentiation between these two diseases.

The clinical illnesses observed in three adolescents with Nocardia asteroides infection are described. The organism has been classified as a bacterium and placed in the Actinomycetaceae family. Sulfonamides remain the first choice for chemotherapy but alternative drugs are available and may be essential in successful therapy. Nocardial infections in children indicate the need for evaluation for an underlying immunological deficiency. Nocardia species should be considered as possible causative agents in immunocompromised patients with infection.

BACKGROUND: Historically, routine use of ultrasound in managing undescended testes has been contentious, but current guidelines from the British Association of Urological Surgeons (BAUS) and the European Association of Urology (EAU) recommend against its use due to limited clinical value. Inappropriate use of ultrasound can lead to delays in treatment, unnecessary costs, and suboptimal outcomes.
