Outcomes of Volar Plating Distal Radius Fractures Based on Surgical Timing.

BACKGROUND: While complexity of distal radius fractures varies, volar plating is the most prevalent surgical option in adult injuries. The time between date of injury and surgical intervention varies according to several factors, including the timing of presentation and the surgeon's availability. This study aims to understand the impact of a delay in surgical intervention on operative time, patient-reported outcomes, and reoperation rates. METHODS: A retrospective review was performed on patients treated with volar plating of distal radius fractures from 2017 to 2020 at a single institution

Football injuries: current concepts.

Football is one of the most popular sports in the United States and is the leading cause of sports-related injury. A large focus in recent years has been on concussions, sudden cardiac death, and heat illness, all thought to be largely preventable health issues in the young athlete. Injury prevention through better understanding of injury mechanisms, education, proper equipment, and practice techniques and preseason screening may aid in reducing the number of injuries. Proper management of on-field injuries and health emergencies can reduce the morbidity associated with these injuries and may

Shoulder outcomes measures.

General health as well as disease- or condition-specific outcome measures have long been used to assess patients with shoulder conditions. Currently, a variety of validated measures is available. Shoulder outcomes measures may be general (eg, American Shoulder and Elbow Surgeons; Constant; Disabilities of the Arm, Shoulder, and Hand), disease-specific (eg, Rotator Cuff Quality of Life, Western Ontario Rotator Cuff Index), or condition-specific (eg, Oxford Shoulder Instability Questionnaire). The results of shoulder arthroplasty and arthritis treatment can be assessed with the Hospital for Spec

Correlation between external rotation of the glenohumeral joint and deformity after brachial plexus birth palsy.

The purpose of this study was to determine whether physical examination findings correlate with glenohumeral joint dysplasia after brachial plexus birth palsy. Thirty-three children with residual palsy underwent magnetic resonance imaging evaluation of both shoulders. Glenoscapular angles and humeral head measurements were calculated. Physical examination findings were recorded about the shoulder. Glenohumeral external rotation was measured during firm stabilization of the scapulothoracic joint. In the uninvolved shoulders, glenoscapular version averaged -8 degrees (range -16 degrees to -2 deg

The course of the suprascapular nerve in the supraspinatus fossa and its vulnerability in muscle advancement.

Twenty-four cadaveric shoulders were evaluated to assess damage to the suprascapular nerve in relation to Debeyre's advancement of the supraspinatus muscle for rotator cuff repair. In all cases the neurovascular pedicle was tethered at the suprascapular notch and at the periosteum of the supraspinatus fossa. The medial motor branches were directed to the trigonum spinae or the superior angle of the scapula (group 1). The dorsal branches crossed the muscle at the bottom of the supraspinatus fossa (group 2) or directly entered the muscle (group 3). The lateral group remained in the supraspinatus

Indirect MR arthrography of the unexercised glenohumeral joint in patients with rotator cuff tears.

RATIONALE AND OBJECTIVES: To evaluate the diagnostic utility of indirect MR arthrography of the unexercised glenohumeral joint in patients with rotator cuff tears confirmed by arthroscopy or arthrotomy as the gold standard. METHODS: Twenty-six patients underwent conventional MR imaging and indirect MR arthrography of the stationary glenohumeral joint using a wrap-around surface coil. Unenhanced T1-weighted spin echo/T2-weighted fast spin echo sequences and T1-weighted gradient echo sequences, adding spectral fat suppression after intravenous administration of contrast medium, were performed i

Morphologic changes in long head of biceps brachii in rotator cuff dysfunction.

Morphologic changes in the long head of the biceps brachii (LHB) and bicipital groove associated with cuff tears were studied in 170 cadavers. In specimens with minimum or moderate cuff tears, the primary finding was relative stenosis at the bicipital groove induced by enlargement of the LHB. However, this stenosis was not apparent in specimens with massive cuff tears and in these specimens, the medial wall of the groove exhibited wear and tear, a potential cause of LHB instability. We suggest that the long head of the biceps brachii muscle can potentially compensate for inadequate rotator cuf

"Bursal reactions" in rotator cuff tearing, the impingement syndrome, and calcifying tendinitis.

Subacromial bursal specimens from 63 patients undergoing surgery for rotator cuff tearing (n = 43), the impingement syndrome (n = 14), and calcifying tendinitis n = 6) were studied to characterize the reactions that develop at the tendinopathy "lesional" sites. Intensity of the bursal reactions and production of type III collagen vary considerably, with the highest incidence of both seen in patients with rotator cuff tears. The intensity of bursal reactions correlated with the degree of formation of perivascular new collagen and type III collagen expression. In 22 of the 63 patients the bursal

High-resolution ultrasound and MRI in the evaluation of pectoralis major injuries.

The pectoralis major muscle is the largest muscle of the anterior chest wall. The primary function of the muscle is to adduct and internally rotate the arm at the shoulder. The pectoralis major muscle is broken down into two main components or "heads" based upon muscle fiber origin: clavicular and sternal. Pectoralis major muscle injury results from direct trauma or indirect force overload. The inferior sternal head fibers are the most commonly torn. The pectoralis major tendon most commonly is torn at the humeral insertion. Magnetic resonance imaging and high-resolution ultrasound have value

Muscle assessment in brachial plexus injury: A reliability study.

PURPOSE: Motor outcome following a brachial plexus injury is frequently measured to evaluate the success of surgical interventions. We aimed to identify whether the manual muscle testing using the Medical Research Council (MRC) method in adults with C5/6/7 motor weakness was reliable and whether its results correlated with functional recovery. METHODS: Two experienced clinicians examined 30 adults with C5/6/7 weakness following proximal nerve injury. The examination included using the modified MRC to assess motor outcome in the upper limb. Kappa statistics were calculated to evaluate inter-te

Reducing chest injuries in automobile collisions: rib fracture timing and implications for thoracic injury criteria.

The purpose of this study was to quantify the biomechanical response of the human thorax during dynamic shoulder belt loading representative of that seen in a severe automotive collision. Two post-mortem human surrogates (PMHSs) (one male and one female) were instrumented with 26 single-axis strain gages on the ribs, sternum, and clavicle. The thorax of each PMHS was placed on a custom spine support bracket designed to support the thorax on either side of the spinous process, thereby allowing free motion at the costovertebral joints. In addition, the support bracket raised the thorax above the

Prevalence of musculoskeletal disorders at the NFL Combine--trends from 1987 to 2000.

PURPOSE: The National Football League holds an annual combine where individual teams evaluate college football players likely to be drafted. As part of the combine, the players are evaluated with a medical history, physical exam, and review of imaging studies, and then they are rated medically as to their ability to participate in the NFL. The purpose of this study was to review the prevalence of musculoskeletal disorders in this population and to test the hypothesis that fewer players were medically disqualified over calendar time from 1987 to 2000. METHODS: The available summary data for al

Risk of avascular necrosis following short term megadose methylprednisolone treatment.

We conducted a prospective cohort study to determine whether administration of large doses of the corticosteroid methylprednisolone following spinal cord injury as recommended in the National Acute Spinal Cord Injury Study-2 (NASCIS-2) protocol results in an increased incidence of avascular necrosis (AVN) of the femoral or humeral head. All subjects were patients treated by a spinal cord injury physician in an Acute Spinal Cord Injury Unit between 1989 and 1996 where some received the megadose steroids while others did not. Patients younger than 15 years and older than 75 years were excluded,

Allograft Medial Patellofemoral Ligament Reconstruction Using Patellar Suture Anchors and Stepwise Medial Imbrication.

Patellar instability commonly affects adolescents and young athletes and is associated with injury to the medial patellofemoral ligament (MPFL). MPFL reconstruction is a well-established treatment for recurrent instability; however, complications related to graft tensioning, fixation, and incomplete restoration of medial soft-tissue restraints persist. We describe a surgical technique for allograft MPFL reconstruction that emphasizes stepwise imbrication of the medial knee layers, anatomic femoral tunnel placement, and all-suture fixation to the patella. This approach restores both the static

Third Trimester foetal complications in pregnancy with diabetes.

Diabetes in pregnancy starts affecting the foetus even in the pre-conception period. The complications encountered in third trimester are foetal macrosomia and intrauterine foetal demise; birth of a macrosomic baby further leads to shoulder dystocia, birth trauma, brachial plexus injury. Additionally, pregnancies with overt/pregestational diabetes may be complicated with foetal growth restriction, congenital abnormalities diagnosed in third trimester and foetal hypertrophic obstructive cardiomyopathy. Even minor degrees of hyperglycaemia is associated with adverse pregnancy outcome. Optimizing

Spontaneous Clostridium perfringens myonecrosis of the shoulder: a case report.

Clostridium perfringens is a rare, life-threatening infection. We present an atypical evolution of spontaneous Clostridium perfringens myonecrosis in a monitored healthy patient. After a head injury, the hemodynamically stable patient became febrile and grew cultures of Clostridium perfringens with no identifiable focus. Antibiotics were initiated. Seventy-two hours after culture results, the patient complained of shoulder pain and imaging revealed periscapular subcutaneous emphysema. Multiple surgical débridements of the necrotic supraspinatus muscle were required to eradicate the infection.

Reoperation for thoracic outlet syndrome.

The clinical history and operative findings in a group of 60 patients who underwent reoperation for thoracic outlet syndrome (TOS) are presented. The patients were severely disabled by arm, shoulder, and neck pain and presented with physical findings pointing to scar fixation of the brachial plexus in the neck (upper tract recurrence) or at the thoracic outlet (lower tract recurrence). The causes of recurrence of TOS as discovered at operation are outlined. Basic principles governing the surgical management of recurrent TOS are elimination of the known causes of recurrence, thorough neurolysis

Obstetric traction injuries of the brachial plexus. Natural history, indications for surgical repair and results.

Of a consecutive series of 70 babies with obstetric traction injury to the brachial plexus we were able to review 40 who had had only conservative treatment. We found that if recovery in the upper roots had not started by three months of age then, at follow-up there was a significant residual functional deficit. In a further 22 babies who showed no recovery of the upper roots by three months, exploration and sural nerve grafting were performed; 20 of these children were reviewed at one year. Good recovery of the deltoid was seen in 80% and of the biceps in 55%, while 25% had good external rota
