Free tissue transfer in pediatric patients.

The majority of reports on free tissue transfer involve adults; few have specifically addressed children. During the past 5 years, 20 free tissue transfers were performed in 19 pediatric patients. Patients ranged in age from 3 to 17 years, with a mean age of 10 years. Eight patients were 6 years and younger; 11 patients were 13 to 17 years old. Soft-tissue defects requiring reconstruction resulted from acute trauma in 12 patients, tumor ablation in 5 patients, infection in 1 patient, and hemifacial atrophy in 1 patient. Soft-tissue defects occurred in the lower extremity in 16 patients, the he

A two-year longitudinal study of falls in 482 community-dwelling elderly adults.

BACKGROUND: Falls are a common occurrence in elderly persons, including relatively healthy, community-dwelling men and women. A significant percentage of falls result in soft-tissue injuries. Although some risk factors for falls have been identified, more research is needed on risk factors for injurious falls. In addition, there is little information from prospective studies on the long-term consequences of falls other than injury. METHODS: Risk factors and consequences of falls were analyzed in a 24-month prospective study of 482 elderly (mean age 74 +/- 6.7 years) men and women living indep

Timing of Microsurgical Reconstruction in Lower Extremity Trauma: An Update of the Godina Paradigm.

BACKGROUND: Marko Godina, in his landmark paper in 1986, established the principle of early flap coverage for reconstruction of traumatic lower extremity injuries. The aim of this study was to determine how timing influences outcomes in lower extremity traumatic free flap reconstruction based on Godina's original findings. METHODS: A retrospective review identified 358 soft-tissue free flaps from 1979 to 2016 for below knee trauma performed within 1 year of injury. Patients were stratified based on timing of coverage: 3 days or less (early), 4 to 90 days (delayed), and more than 90 days (late

Free tissue transfer in patients with severe peripheral arterial disease: functional outcome in reconstruction of chronic lower extremity defects.

Chronic wounds of the lower extremity are difficult to treat, especially in patients with peripheral arterial disease (PAD). This patient population frequently suffers from multiple comorbidities, which further impairs wound healing. Local flaps are often not available, or are insufficient to cover these defects, and limbs are frequently amputated, although they could possibly be salvaged by free tissue transplantation. Although there is a growing evidence that free tissue transfer may be feasible in these patients with otherwise doomed extremities, previous studies have mostly focused on shor

Analysis of Functional Outcomes Following Surgical Management of Neglected Multiple Carpometacarpal Dislocations: A Series of four Cases and Review of Literature.

INTRODUCTION: Dislocation of the carpometacarpal (CMC) joint is a rare injury that results following high-velocity trauma. Although there are a few anecdotal reports of isolated CMC joint dislocations, there is scarce literature concerning dislocations involving multiple joints, especially those that are neglected and present late after injury. The injury is often missed when the patient presents to the surgeon since the swelling obscures the characteristic deformity, and the fracture may not be apparent on radiographs. When missed at the initial presentation, they can frequently result in pai

BaTiO3-based Piezoelectric Wireless Neuromodulation: From Molecular Mechanisms to Clinical Translation Challenges.

Neurological injuries remain difficult to treat due to the complex microenvironment and the limited intrinsic regenerative capacity of neural tissues. Recently, wireless neuromodulation based on piezoelectric nanomaterials, particularly barium titanate (BaTiO3), has emerged as a promising alternative to conventional implantable electrical stimulation. Polar BaTiO3 ceramics generally have higher piezoelectric charge coefficients than conventional Polyvinylidene Fluoride (PVDF) and Poly(vinylidene fluoride-co-trifluoroethylene) (P(VDF-TrFE)) films, are more resistant to degradation than ZnO, and

Transradial Amputation With Nerve Management: Case Presentation and Surgical Technique.

Transradial amputation of appropriate length preserves forearm rotation and facilitates prosthetic use; however, high rates of phantom limb pain and neuropathic medication use persist. This case describes a multidisciplinary approach to transradial amputation that integrates targeted muscle reinnervation, regenerative peripheral nerve interfaces, and bone and soft tissue management to optimize pain control and prosthetic success. A 29-year-old man with a severe ballistic injury and complex regional pain syndrome underwent transradial amputation with targeted muscle reinnervation and regenerati

Lisinopril-Induced Angioedema Triggered by Tattoo Cellulitis and Acute Kidney Injury: A Case Report.

Angiotensin-converting enzyme inhibitor-induced angioedema is a rare, potentially life-threatening disorder. This mostly manifests with swelling of the lips, tongue, and face with no known reports of angiotensin-converting enzyme inhibitor-induced angioedema from soft-tissue infections. We present herein a case of a 47-year-old male with a history of chronic kidney disease stage III who developed acute upper lip angioedema and severe acute kidney injury while on lisinopril/hydrochlorothiazide, following tattoo-related cellulitis. We propose that inflammatory activation from cellulitis amplifie

Arthroscopic Fixation of Minimally Displaced Avulsion Fracture of Ischial Tuberosity Using a "Double-Pulley" Suture Anchor Technique.

Avulsion fracture of the ischial tuberosity is an uncommon injury caused by a sudden, forceful eccentric contraction of the hamstrings that avulses the unfused secondary ossification center from the pelvis. Although open reduction and internal fixation are recommended when fragment displacement exceeds 2 cm, reports describing minimally invasive solutions are still lacking. In addition, for those patients who are engaged in high-intensity sports or physical activities, conservative treatment can compromise hamstring power, delay return to sport, and occasionally result in chronic ischial pain.

External Neurolysis and Hypothenar Fat Pad Flap With Early Nerve Gliding Exercise Regimen for Recurrent Carpal Tunnel Syndrome.

Carpal tunnel syndrome (CTS) is the most common compressive neuropathy of the upper extremity and is most often treated successfully with surgical release of the median nerve. However, some patients continue to experience symptoms or develop recurrence, which presents a management challenge. The pathophysiology of recurrent CTS may involve perineural fibrosis, iatrogenic injury, incomplete release, or unrelated neuropathic conditions such as diabetic or hereditary neuropathies. Management depends on patient history, examination, and clinical factors, with many requiring revisional surgery to r

Carbon ion radiotherapy and AI chemotherapy for soft tissue sarcoma of the temporomandibular joint: A case report of 5-year disease-free survival without wide resection.

We report a case of soft tissue sarcoma (STS) arising in the temporomandibular joint (TMJ), a rare site associated with considerable functional and aesthetic challenges when resection is required. A 33-year-old female with neurofibromatosis type 1 presented with progressive swelling in the right TMJ. Imaging revealed an enlarging, irregular region. Histopathological findings were suggestive of a high-grade STS, but definitive subclassification was not possible. The patient declined surgery due to potential facial nerve injury and cosmetic concerns. As an alternative, a combination of carbon io

Lymphatic endothelial cell RXRα is critical for 9-cis-retinoic acid-mediated lymphangiogenesis and prevention of secondary lymphedema.

Secondary lymphedema is a debilitating disease characterized by abnormal soft tissue swelling and caused by lymphatic system dysfunction. Despite a high prevalence of secondary lymphedema after cancer treatments, current management is supportive and there are no approved therapeutic agents that can thwart disease progression. We have previously demonstrated that 9-cis-retinoic acid (9-cisRA) has the potential to be repurposed for lymphedema as it mitigates disease by promoting lymphangiogenesis at the site of lymphatic injury. Although the efficacy of 9-cisRA has been demonstrated in previous
