Post-traumatic syringomyelia: outcome predictors.

OBJECTIVE: To identify risk factors that predispose to post-traumatic syringomyelia (PTS) and describe the outcome of surgical management. METHODS: Retrospective cohort study of 27 patients with post-traumatic syringomyelia. Spinal cord injury of these patients spanned the period from 1963 to 2008. All data were collected retrospectively using available medical records and radiological images. RESULTS: There were 24 males and 3 females. The level of initial spine injury was thoracic in 21, cervical in 4, and lumbar in 2. The average age (±SD) at diagnosis of PTS was 40±13 years. The mean fol

[Histopathological study of aging of the posterior portion of human cervical vertebral bodies and discs--with special reference to the early ossification of the posterior longitudinal ligament].

UNLABELLED: Ossification of the posterior longitudinal ligament ( OPLL ) of the cervical spine is a disease causing spinal canal stenosis and, henceforth , spinal cord compression. This ossification is found approximately in three percent of the adult Japanese, but it is also found to be present among Chinese, Korean and other South-East Asians, whereas the incidence among Caucasians was reported to be significantly low. The etiology of this interesting ossification is still unknown although multidisciplinary studies have been carried out in Japan in recent years. The purpose of the present st

The association of operative versus nonoperative treatment for lumbar spondylolisthesis and stenosis with future metabolic conditions and allostatic burden: an emulated target trial.

BACKGROUND CONTEXT: Lumbar spinal stenosis adversely impacts mobility and function. Persistent lack of exercise and ambulatory capacity may contribute to the development of metabolic conditions. PURPOSE: To determine the effect of surgical intervention, as compared to nonoperative treatment, on the development of metabolic conditions, chronic pain, frailty, and allostatic load (AL). STUDY DESIGN: Retrospective emulation target trial. PATIENT SAMPLE: A total of 2,521,827 patients were included, of which 329,314 (13.1%) underwent surgery. OUTCOME MEASURES: Development of metabolic conditions

Effects of pain in lumbosacral stenosis and lifestyle-related factors on brain-derived neurotrophic factor expression profiles.

This study investigated the role of brain-derived neurotrophic factor (BDNF) in patients with degenerative lumbar stenosis, focusing on its expression and correlation with pain intensity. The study examined 96 patients with lumbar stenosis and 85 control participants. BDNF levels in the yellow ligamentum flavum were measured using reverse transcription quantitative polymerase chain reaction (RT-qPCR), enzyme-linked immunosorbent assay (ELISA), and western blot analysis. The results showed significantly higher BDNF expression at both messenger ribonucleic acid (mRNA; fold change = +1.35 ± 0.23;

GPT4LFS (generative pretrained transformer 4 omni for lumbar foramina stenosis): enhancing lumbar foraminal stenosis image classification through large multimodal models.

BACKGROUND CONTEXT: Lumbar foraminal stenosis (LFS) is a common spinal condition that requires accurate assessment. Current magnetic resonance imaging (MRI) reporting processes are often inefficient, and while deep learning has potential for improvement, challenges in generalization and interpretability limit its diagnostic effectiveness compared to physician expertise. PURPOSE: The present study aimed to leverage a multimodal large language model to improve the accuracy and efficiency of LFS image classification, thereby enabling rapid and precise automated diagnosis, reducing the dependence

Y-Style Unilateral Biportal Endoscopic Far-Lateral Approach for L5S1 Foraminal Stenosis.

OBJECTIVE: High iliac crest can pose an anatomical challenge for classic paraspinal unilateral biportal endoscopic far-lateral approach (C-UBEFLA). To overcome this, we devised a modified technique, the Y-style unilateral biportal endoscopic far-lateral approach (Y-Style-UBEFLA), which introduces a lateral working portal. The name derives from the morphological resemblance of the portal arrangement to the letter "Y". To compare the effectiveness of the Y-Style-UBEFLA with the C-UBEFLA in treating L5S1 foraminal stenosis (L5S1FS). METHODS: A retrospective review was conducted on 47 patients di

Post-operative gigantic lumbar pseudomeningocele: A case report.

Postoperative pseudomeningocele is a rare, but still existing, complication after spinal surgeries. It may be asymptomatic or presented with back pain, radicular pain or headaches. Many pseudomeningoceles resolve spontaneously, others require revision surgery with dural repair. We present a female patient who underwent duroplasty treatment for a massive postoperative lumbar pseudomeningocele measuring 22.57 cm in length after broad laminectomy. A 71-year-old female with previous thoracolumbar T10-L5 instrumentation surgery, underwent a L2-5 broad laminectomy due to severe canal stenosis at the

Deep-learning-reconstructed high-resolution 3D cervical spine MRI for foraminal stenosis evaluation.

OBJECTIVE: To compare standard-of-care two-dimensional MRI acquisitions of the cervical spine with those from a single three-dimensional MRI acquisition, reconstructed using a deep-learning-based reconstruction algorithm. We hypothesized that the improved image quality provided by deep-learning-based reconstruction would result in improved inter-rater agreement for cervical spine foraminal stenosis compared to conventional two-dimensional acquisitions. MATERIALS AND METHODS: Forty-one patients underwent routine cervical spine MRI with a conventional protocol comprising two-dimensional T2-weig

Treatment of posttraumatic lumbar interspinous ligament calcification with partial resection of spinous processes and calcified interspinous ligaments: case report.

The authors report on the first surgical treatment for traumatic interspinous ligament calcification, with significant radiographic and symptomatic improvements at long-term follow-up. Heterotopic ossification occurs following traumatic injury but does not typically affect the interspinous ligaments. While these ligaments can calcify with age, this is rarely seen in patients younger than 50 years of age. The authors present the unusual case of a 31-year-old man who suffered traumatic fractures of thoracic and lumbar spinous processes. He developed progressive low-back pain that failed to respo

[The DIAM spinal stabilisation system to treat degenerative disease of the lumbosacral spine].

PURPOSE OF THE STUDY: Each dynamic stabilisation should preserve motion at the operated segment as well as reduce a load on the disc and intervertebral joints. One of the methods to achieve this is the implantation of interspinous spacers between lumbar spinous processes. In this study, the patients treated with the DIAM interspinous spacer (Medtronic, USA) were prospective- ly followed up with the aim to evaluate clinical outcomes and post-operative complications. MATERIAL: Patients with a degenerative disease of the lumbosacral spine were indicated for the operation. They suffered from axia
