Reconstruction of extensive posterior mid-thoracic soft-tissue defects after spinal surgery on irradiated skin.

A 56-year-old patient presented with neurological disorders resulting from an extrinsic medullary compression extending from T3 to T5 associated with a T4 corporeal invasion by a high-grade non-Hodgkin's malignant lymphoma. Treatment consisted in a laminectomy without spinal stabilization followed immediately by chemotherapy (VCAD) and locoregional radiotherapy (20 cycles of 50 Gy between T3 and T7 followed by nine sequences of 18 Gy). The evolution revealed unfavourable with reappearance of a compressive syndrome thus requiring surgical decompression combined with spinal stabilization. The im

Closure of palatal defects following excision of palatal pleomorphic adenomas.

AIM: The purpose of this study is to compare different palatal defect closure techniques following excision of palatal pleomorphic adenomas (PPA) in four cases and to review the associated dental literature. METHODS AND MATERIALS: Excision of all four PPA's was performed under local anesthesia. Three different closure techniques used among the cases included an intact mucosal flap, a pedicled buccal fat pad, or secondary healing. RESULTS: On average the defects healed completely at two months following surgery. While final healing was ideal, partial necrosis of the mucosal flap and minimal p

Endoscopic transnasal repair of choanal atresia.

OBJECTIVES: To present our experience with a new endoscopic technique for transnasal repair of choanal atresia. METHODS: Seventeen patients aged 2 months to 13 years with choanal atresia, bilateral in 6 and unilateral in 11, underwent endoscopic repair using a mucoperichondrial flap developed from the nasal septum. The bony stenosis was opened with a surgical curette or drill, and the raw surface was covered by the flap. All patients in these case series with bilateral atresia had been treated with dilatation at birth and had restenosis. RESULTS: A total of 23 choanae were operated. Follow-u

The clear corneal tongue: a mechanism for wound incompetence after phacoemulsification.

PURPOSE: To describe a mechanism for wound incompetence after phacoemulsification with corneal incisions resulting in early postoperative wound leakage. DESIGN: Observational case series. METHODS: Three patients who had uneventful phacoemulsification through a clear corneal incision were identified because of a postoperative wound leak. A corneal tongue, consisting of an everted triangular flap of posterior corneal stroma, the Descemet membrane, and endothelium, was observed in the wounds of all cases. RESULTS: One wound leak resolved after pressure patching. The other two necessitated woun

Postoperative mediastinitis following hemiarch replacement: successful treatment using omental flap transposition.

Although various strategies have been advocated for mediastinitis following cardiac operations, surgical results for this complication remain a significant concern. The condition would be more complicated with replacement of the ascending aorta using a prosthetic graft. We present the case of a 50-year-old female patient with acute aortic dissection who underwent hemiarch replacement using a prosthetic graft and concomitant coronary artery bypass grafting, and developed periprosthetic purulent collection 2 months later. Successful treatment involved debridement ofìnfected tissues and omental f

[Mandibular reconstruction in oncology: discussion of indications].

The aim of the study was to assess the results of segmental mandibular resections and the validity of mandibular reconstructions. The medical records of 95 patients treated between January 1, 1987 and December 31, 1992 were reviewed. There were 82 (86%) carcinomas, 9 ameloblastomas and 4 sarcomas. Among the 82 patients treated for carcinoma, the overall 2- and 5-year survival rates were 70% and 33% respectively. The functional results evaluated one year after surgery showed no significant difference according to the reconstruction procedure: myocutaneous flap, osteomyocutaneous flap, titanium

Retrospective study comparing robotic versus open approaches for genitourinary fistula repair.

INTRODUCTION: Genitourinary fistulas are abnormal communications between the urinary or genital tract and adjacent structures, often requiring surgical repair to restore function and quality of life. Robotic-assisted surgery has gained acceptance due to improved visualisation and surgical precision. This study compares the outcomes of robotic-assisted and open genitourinary fistula repair. PATIENTS AND METHODS: A retrospective review of patients undergoing genitourinary fistula repair between 2010 and 2020 was performed. Patients were divided into robotic (n = 26) and open (n = 24) groups. Su

Noma surgery.

OBJECTIVE: Noma is a devastating and destructive disease of the face for which there is a dearth of information regarding surgical options. Herein, we describe the facial deformities and patient characteristics in a patient population affected by noma and the surgical approaches used in treatment. METHODS: Retrospective case series of a Doctors Without Borders (Médecins Sans Frontières) intervention at Sokoto Children's Noma Hospital in northern Nigeria, the highest-volume noma hospital in the world. RESULTS: Twenty-two procedures were performed on 18 patients with noma, 44% of whom were chi

The association between complications and quality of life after mastectomy and breast reconstruction for breast cancer.

BACKGROUND: Medical treatment for breast cancer is associated with substantial toxicity and patient burden. There is less known about the impact of surgical complications. Understanding this impact could provide important information for patients when they are considering surgical options. METHODS: Between 2008 and 2009, the UK National Mastectomy and Breast Reconstruction Audit recorded surgical complications for a prospective cohort of 17,844 women treated for breast cancer at 270 hospitals; 6405 of these women were surveyed about their quality of life 18 months after surgery. Breast appear

Urethral Stricture Score is Associated with Anterior Urethroplasty Complexity and Outcome.

PURPOSE: Several surgical techniques are available to treat anterior urethral stricture. The choice of surgical technique largely depends on the severity of stricture disease. The U-score (urethral stricture score) is based on urethral stricture characteristics, namely length (1 to 3 points), number (1 or 2 points), location (1 or 2 points) and etiology (1 or 2 points), which are tallied to provide a total score of 4 to 9 points. Our aim was to identify whether the U-score system is predictive of the surgical complexity and outcome of anterior urethroplasty. MATERIALS AND METHODS: We retrospe

Early and late outcome after surgical treatment of acquired non-malignant tracheo-oesophageal fistulae.

OBJECTIVES: Tracheo-oesophageal fistula (TOF) is a rare, life-threatening condition. We report our results of surgical treatment and evaluation of the outcome of acquired non-malignant TOF. METHODS: Twenty-five patients (aged 49 ± 21 years) with TOF were operated on between 2001 and 2011. Tracheo-oesophageal fistula was due to prolonged intubation/tracheostomy (84%), was secondary to other surgery (8%) or trauma (4%) or was idiopathic (4%). The tracheal defect was 2.4 ± 1.3 cm long and was associated with tracheal stenosis in seven (28%) patients. Surgical treatment consisted of direct suturi

[Modified Warden Procedure for Correction of Partial Anomalous Pulmonary Venous Connection and Sinus Venosus Atrial Septal Defect in an Adult].

A 32-year-old woman was referred to our hospital for the surgical indication of sinus venosus-type atrial septal defect. Preoperative computed tomography scan revealed that the right upper pulmonary vein returned to the high superior vena cava. We performed a modified Warden procedure using a pedicle flap of the right atrial appendage along with a fresh autologous pericardium. Her postoperative course was uneventful with no venous obstruction or sinus node dysfunction. This technique is a useful surgical option for a partial anomalous pulmonary venous connection especially in adults.

Easing the Excision of Earlobe Keloid.

Earlobe keloids are commonly encountered in this era where ear piercing is routine in various traditions. Various medical and surgical methods have been tried for its management. Surgically, fillet flap surgery has better acceptability worldwide. Various difficulties are faced during keloid excision, and we hereby describe few innovative modifications (use of needle, hydrodissection, pressure dressing, bolster button) using cost-effective and simple materials (needles, dynaplast, buttons) to ease the excision of keloids. These have easy adaptability and also reduce the recurrence rate of earlo

Management of Frontal Sinus Cerebrospinal Fluid Leaks and Encephaloceles.

Encephaloceles and cerebrospinal fluid (CSF) leaks of the frontal sinus may result from congenital, traumatic, spontaneous, or neoplastic causes. Paramount to success is adequate preoperative planning with accurate history, physical exam, endoscopy, imaging, and testing to confirm location of the leak and origin of the disease. Generally, frontal sinus CSF leaks may be addressed endoscopically with favorable anatomy, proper surgical technique, and appropriate equipment. Open surgical approaches (eg, osteoplastic flap) are often required for superior/lateral defects or if the surgeon is not exp

Frontalis-based island pedicle flaps for the single-stage repair of large defects of the forehead and frontal scalp.

BACKGROUND: Medium- to large-sized surgical defects of the forehead and frontal scalp provide a challenge for the reconstructive surgeon. OBJECTIVES: To highlight the utility of a frontalis-based island pedicle flap (FIPF) for the single-stage, aesthetic reconstruction of such defects. METHODS: The design and detailed surgical technique required for a FIPF are described. RESULTS: The long-term results are illustrated. CONCLUSIONS: FIPFs, despite significant tissue dissection, are readily performed under local anaesthesia and are well tolerated by patients. The FIPF adds to the dermatologic

Evolution of techniques in hair transplantation: a 12-year perspective.

Few if any fields of medicine have undergone such dramatic advancements in technique and outcomes as surgical hair restoration over the past 12 years. Not very long ago, the procedure produced results that were, from a cosmetic perspective, acceptable at best; we are now capable of creating results that truly are natural and undetectable in appearance. These developments have clearly resulted in a procedure-follicular unit hair transplanting-far superior in outcome to the once popular other surgical hair restoration procedures of bald scalp reductions and scalp flap surgery, as well as micro-

[Sequelae of haemangiomas: surgical treatment].

The cure without after-effect of an hemangioma is classic but sometimes the spontaneous involution, a premature surgical treatment or an evolutionary complication can be at the origin of a definitive after-effect. In these cases, the treatment of the hemangioma, often multidisciplinary, will be begun in a late way during the late childhood or the adolescence. Surgical excision of an excess tissue or of a wide scar, lipoaspiration of a fat remainder, lasertherapy and fat tissue reinjection are simple and mostly sufficient procedures. In the most complex cases, notably at the facial site, a surg

[Our experience with the treatment of high perianal fistulas with the mucosal flap advancement technique].

The authors present their experience with the treatment of high transphincteric anal fistulas with the mucosal flap advancement technique. This technique, though by no means easy to perform, allows fistulas to be treated in a single surgical session in comparison to the technique in which setone is used or to the less well known transposition techniques, given the same long-term results in terms of continence and recurrence rate. After a brief overview of the problem, from the points of view of both aetiopathogenesis and classification, the principal surgical treatment techniques are described

Treatment of malignant melanoma of the lower eyelid using anterolateral thigh flap.

Malignant melanoma is a poor prognostic disease with the potential for high mortality despite early diagnosis and currently available treatment. Surgical judgment pertaining to the extent of primary resection and regional node dissections must be tempered by cosmetic considerations and by the anatomically diffuse pathways for lymphatic spread in certain lesions. We reported a case of malignant melanoma of the left lower eyelid, and presented the surgical excision and reconstruction using anterolateral thigh flap for skin defect. It is evident that comprehensive treatment using radical surgery
